The contact owned a 2008 Dodge Avenger. The contact stated that while her grandson was driving at an unknown speed, he lost control of the vehicle. The contact was informed by the police officer, that the vehicle flipped 4 times and the driver was not wearing his seat belt, which caused the driver to be ejected from his seat and was found half inside the cab and half outside the driver's side window. The contact stated all the other air bags deployed, except the front driver side airbag. The contact was informed the driver was deceased when the police and the front passenger sustained injuries on her left shoulder, chest pains, a laceration on her right leg, and scratches on her left leg. The contact stated the front passenger did seek medical attention at the hospital. A police report was filed. The vehicle was not drivable. The insurance company deemed the vehicle a total loss. The vehicle was towed to her residence. The vehicle was not diagnosed. A dealer was not contacted. The manufacturer had not been informed of the failure. The failure mileage was unknown.

WEDNESDAY OCTOBER 10TH 2018 APPROXIMATELY 10:30 A.M. I WAS ON MY WAY TO SMOOTHIE KIND IN COOPER CITY LOCATED INSIDE OF A PLAZA. AS I TURNED INTO THE PARKING SPOT WITH MY FOOT ALREADY ON THE BRAKE TO SLOW DOWN INTO SAID PARKING SPOT THE VEHICLE ACCELERATED ON ITS OWN. I PUMPED THE BRAKES ONCE NOTHING STOPPED THE VEHICLE AND IT WENT OVER THE MEDIUM BETWEEN PARKING SPOTS AND HIT A UNOCCUPIED VEHICLE ON THE LEFT FRONT. THEN STOPPED JUST BEFORE HITTING A TREE. I'AM NOT SURE WHAT HAPPENED BUT THEIR SEEMS TO BE AN ELECTRICAL MALFUNCTION BECAUSE MY MILEAGE HAS DISAPPEARED SINCE.
ON MARCH 5, 2010 (FRIDAY) - APPROXIMATELY 5:30 P.M., WHILE DRIVING ON SUNRISE HIGHWAY IN LONG ISLAND, NEW YORK, AN INDICATOR LIGHT WENT ON INSIDE MY 2008 DODGE AVENGER, IN THE SHAPE OF A LIGHTNING BOLT IN MY CAR. LITERALLY 30 SECONDS LATER, AND BEFORE THERE WAS TIME TO REACT AT ALL, MY BRAKES STOPPED WORKING AND THERE WAS NO WAY TO STOP MY VEHICLE. I HAD JUST ABOUT MADE IT TO THE EXIT RAMP WHEN I REALIZED BY LOOKING IN THE REARVIEW MIRROR THAT THERE WAS SMOKE COMING FROM MY CAR. AS I HAD STEERED ONTO THE EXIT RAMP, I OPENED THE DOOR AND SAW FLAMES SHOOTING FROM THE UNDERNEATH PART OF MY CAR. WITHOUT THINKING TWICE I JUMPED FROM THE MOVING VEHICLE (WHICH AT THE TIME STILL WAS GOING AT ABOUT 20 MPH). I WAS NOT HURT PHYSICALLY BUT WAS EXTREMELY SHAKEN UP. ANOTHER TRAVELER WHO HAD PULLED UP ON THE SERVICE ROAD TO HELP ME, INFORMED ME THAT THE CAR WAS ON FIRE FOR AT LEAST 1/4 OF A MILE ON THE HIGHWAY AND THAT HE HAD BEEN ATTEMPTING TO FLAG ME DOWN. AFTER I HAD ESCAPED THE VEHICLE, THE CAR CONTINUED IN A DIAGONAL PATH TOWARD ONCOMING RUSH HOUR HIGHWAY TRAFFIC. THE FIRE CONTINUE TO BURN AND HAD BECOME VERY BAD, TO THE POINT WHERE THE ENTIRE FRONT HALF OF THE CAR WAS ABLAZE. THE ONLY THING THAT STOPPED THE OUT OF CONTROL VEHICLE WAS A WIRE FENCE IN THE DIVIDER BETWEEN THE SERVICE ROAD AND MAIN HIGHWAY. THE INSURANCE COMPANY AND MANUFACTURER ARE CURRENTLY IN THE PROCESS OF INVESTIGATING. THE FIRE DEPARTMENT TOOK AT LEAST 15-20 MINS TO RESPOND WHICH CAUSED THE VEHICLE TO BECOME SEVERELY DAMAGED TO THE POINT WHERE IT IS NOW EXTREMELY DIFFICULT TO ASCERTAIN A CAUSE. MY INSURANCE ONLY COVERED MY CAR LOAN AND NOW I AM OUT MONEY FOR THE DOWN PAYMENT ON A NEW VEHICLE. THIS SITUATION IS VERY SERIOUS. THERE COULD HAVE BEEN CHILDREN OR ELDERLY PERSONS IN THE VEHICLE WHO COULD HAVE DIED. PLEASE HELP ME INVESTIGATE THIS ISSUE. I CONSIDER MYSELF LUCKY TO STILL HAVE MY LIFE AND EVALUATE THE SITUATION AS HAVING INITIATED A DUTY TO KEEP OTHERS SAFE FROM DANGERS OF THE SAME. *TR
I WAS IN A CAR ACCIDENT AND THE AIRBAG DID NOT DEPLOY AND THE SEATBELT DID NOT RESTRAIN ME. I HIT THE STEERING WHEEL REALLY HARD. I HAVE CHEST PAINS, AND MIGRAINES. TL* THE CONTACT OWNED A 2008 DODGE AVENGER. WHILE DRIVING 50 MPH, THE CONTACT DROVE OFF THE ROAD AND CRASHED INTO A CEMENT BARRIER IN AN EFFORT TO AVOID A CRASH WITH ANOTHER VEHICLE. THE AIR BAGS FAILED TO DEPLOY AND THE SEAT BELTS FAILED TO OPERATE AS INTENDED. THE CONTACT SUSTAINED A SEVERE CONCUSSION, BLEEDING ON THE BRAIN, A SPRAINED ANKLE, A FRACTURED NOSE, FRACTURED RIBS, AND SEVERE BACK TRAUMA. MEDICAL ATTENTION WAS REQUIRED. TAKATA RECALL- I HAVE BEEN IN TWO ACCIDENTS WITH MY DODGE AVENGER 08. FIRST WAS WHEN I HIT A DEER MY ENTIRE BODY WAS SLAMMED INTO THE STEERING COLUMN, KNEW OUT TEETH AND BROKE BONES MONTHS AGO, NO AIRBAG DEPLOYED NOR DID NOT SEAT BELT ENGAGE. THE CAR WAS IN MOTION TRAVELING ABOUT 40 MILES AN HOUR. THE SECOND ACCIDENT WAS RUN OFF THE ROAD BY A VAN TRAVELING IN THE MIDDLE OF THE ROAD COMING AT ME. I HAD TO SERVE HARD RIGHT, I ENDED UP IN A DEEP DITCH IN WHICH I SLAMMED INTO A CEMENT WASHY AGAIN NO AIRBAG, NO SEAT BELT, HIT MY HEAD SO HARD I WAS KNOCKED UNCONSCIOUS, SEVERE FACIAL TRAUMA, BACK, NECK AND HEAD. MY SON WAS TRAVELING EAST ON INTERSTATE 64 WHEN ANOTHER CAR CAME UPON HIM TRAVELING AT A HIGH RATE OF SPEED AND SLAMMED ON HIS BRAKES. THE ROAD WAS WET AND THE OTHER CAR STARTED SLIDING OUT OF CONTROL AND HIT MY SONS CAR IN THE PASSENGER FRONT DOOR AND PUSH THE WHOLE CAR INTO A ROCK WALL. THE AIRBAGS DID NOT DEPLOYED AT ALL.

I WAS EXITING OUT OF A PARKING LOT IN TOWN AND TRAFFIC WAS BACK UP AND AT A STANDSTILL. AS I ATTEMPTED TO TURN LEFT A CAR TRIED PASSING THE STOPPED TRAFFIC IN A NO PASSING ZONE. THE VEHICLE HIT THE FRONT OF MY CAR, TOTALING IT. THE BUMPER WAS ON THE GROUND , RADIATOR SIDEWAYS, HOOD CRINKLED, FENDERS DESTROYED. MY AIRBAGS DID NOT DEPLOY. I WAS CUT OFF BY ANOTHER DRIVER WHO FAILED TO STOP AND YIELD TO ONCOMING TRAFFIC (ME). I BRAKED HARD TO AVOID REAR ENDING HIM, BUT HE TURNED INTO MY LANE WHEN I WAS TOO CLOSE. I WAS DRIVING EAST BOUND AND I WAS T-BONED ON THE DRIVER SIDE BY A CAR GOING SOUTH BOUND ON WHOM RAN THE LIGHT. ON 1/12/12 AT ABOUT 545AM I WAS DRIVING TO WORK ON A RAINY DAY. WHILE TRAVELING ON 95 NORTH BOUND AT ABOUT 55MPH MY WIPER BLADES STOP WORKING AND SO DID MY STEERING WHEEL. MY STEERING COLUMN WAS NOT CONTROLING MY WHEELS AND I HYDROPLAINED WHILE TRYING TO STOP MY CAR. MY CAR SPUN AROUND NUMEROUS TIMES BEFORE SLAMMING INTO THE MEDIAN WALL. I HAVE HAD MANY MALFUNCTIONS WITH THIS VEHICLE BUT THIS ONE TOPPED THEM ALL. I HAVE BEEN OUT OF WORK SINCE THIS DAY WITH INJURIES CAUSED BY THIS ACCIDENT, I COULD HAVE BEEN KILLED. I WAS DRIVING MY 2008 DODGE AVENGER TO WORK ON 01/12/12. IT WAS RAINING THAT MORNING. I ATTEMPTED TO MERGE TO MY RIGHT WHEN SUDDENLY MY WINDSHIELD WIPERS STOPPED AND MY STEERING WHEEL BEGAN TO TURN 360 DEGREES HOWEVER MY TIRES WERE NOT MOVING. I BANGED IN THE MEDIAN. THE CONTACT LOST CONTROL OF THE VEHICLE AND CRASHED INTO A WALL. THE AIR BAGS DID NOT DEPLOY UPON IMPACT. THE CONTACT WAS INJURED AND A POLICE REPORT WAS FILED. THE CONTACT REFERENCED NHTSA CAMPAIGN ID NUMBER: 07V240000 (AIR BAGS:FRONTAL) BUT THE VEHICLE WAS NOT INCLUDED IN THE RECALL. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. IN ADDITION, THE CONTACT STATED THAT PRIOR TO THE CRASH, THERE WERE SEVERAL FAILURES IN WHICH THE VEHICLE CONTINUOUSLY STALLED WITHOUT WARNING. A LOCAL MECHANIC REPLACED THE ELECTRONIC THROTTLE CONTROL. THE DEALER OFFERED TO ASSIST THE CONTACT WITH THE STALLING DEFECT. THE FAILURE MILEAGE WAS 66,000 AND THE CURRENT MILEAGE WAS 84,000.
