IT WAS SEPTEMBER 23RD AROUND 11PM, I WAS DRIVING ON A HIGHWAY, UPON TURNING INTO THE EXIT LANE MY CAR HIT SOME FORM OF POTHOLE WHICH MADE MY TOUCH SCREEN AND DASHBOARD TURN OFF AND BACK ON. I'VE HAD ISSUES WITH MY TOUCH SCREEN SHUTTING OFF AND BACK ON IN THE PAST, ONCE I'VE REPLACED THE BATTERY IT WORKED FINE UNTIL THAT DAY, THEN IT STARTED ACTING UP AGAIN SO BEFORE ALL OF THIS I DID SET UP A CAR APPOINTMENT FOR IT TO GET CHECKED OUT 2 DAYS EARLIER. SO ANYWAY, AS I'M DRIVING, I ARRIVE TO MY DESTINATION. I SAT IN THE CAR FOR A WHILE ON MY PHONE AND DECIDED TO TURN ON MY AIR. WHEN THE AIR CAME OUT IT SMELLED VERY SMOKEY. SO I TURNED IT OFF AND ABOUT 5 MINUTES LATER, A VERY LIGHT MIST BEGAN TO COME OUT OF THE DRIVERS SIDE HOOD. SO I DECIDED TO JUST HEAD HOME INSTEAD BECAUSE I DIDN'T WANT TO BE STUCK SOMEWHERE AND I NOT HAVE A RIDE HOME, MY HOUSE WASN'T THAT FAR FROM MY LOCATION. SO THE FURTHER I'M DRIVING I NOTICED THE SMOKE BEGAN TO PICK UP A BIT SO I CUT MY HAZARD LIGHTS ON. I'M ABOUT 5 MINUTES AWAY FROM MY HOUSE AT THE POINT. SO I'M DRIVING DOWN THE RAMP TO MERGE INTO THE HIGHWAY AND THE SMOKE GOT SO THICK TO WHERE I COULD NO LONGER SEE SO I'M PUSHING DOWN THE BREAKS TO SLOW THE CAR DOWN AND FIND MY WAY TO THE SIDE OF THE ROAD BUT THEN MY CAR LOCKS UP AND SHUTS DOWN. I COULDN'T TURN THE WHEEL, GAS OR BREAK OR ANYTHING. WHEN I COULD GLIMPSE THROUGH THE SMOKE I NOTICED A FIRE WAS COMING FROM MY HOOD, SO INTO MOVING TRAFFIC ON THE HIGHWAY GOING ABOUT 40 MILES AN HOUR I HOPPED OUT OF MY CAR AND IT ROLLED A LITTLE OVER 100 FEET TO ALMOST THE YELLOW LINE (IT WAS STILL BLOCKING ONE LANE). WITNESSES CALLED THE COPS AND THE FIRE DEPARTMENT PUT THE FLAMES OUT.I REACHED OUT TO MY CAR COMPANY AND THEY SAID SINCE MY CAR WASN'T INSURED, THAT BASICALLY I'M OUT OF LUCK. TL* THE CONTACT OWNS A 2013 DODGE DART. WHILE DRIVING AT AN UNKNOWN SPEED, THE VEHICLE STALLED. THE CONTACT WAS ABLE TO EVENTUALLY RESTART THE VEHICLE. THE FAILURE RECURRED ON NUMEROUS OCCASIONS. THE VEHICLE WAS TAKEN TO A DEALER WHERE IT WAS DIAGNOSED THAT THE KEY FOB AND THE IGNITION NEEDED TO BE REPLACED. THE VEHICLE WAS REPAIRED; HOWEVER, THE FAILURE PERSISTED. IN ADDITION, WHILE DRIVING 45 MPH, THE CONTACT NOTICED A BURNING ODOR COMING FROM THE VEHICLE. UPON INSPECTION, THE CONTACT NOTICED THAT THE FRONT PASSENGER SIDE DASHBOARD AND WIRING HARNESS IN THE GLOVE BOX WERE ON FIRE. THE CONTACT INDEPENDENTLY EXTINGUISHED THE FIRE. NO INJURIES WERE SUSTAINED. THE VEHICLE WAS TOWED TO A DEALER WHERE IT WAS DIAGNOSED THAT THE WIRING HARNESS, CARPET, AND DASHBOARD NEEDED TO BE REPLACED. THE VEHICLE WAS REPAIRED. THE CONTACT ALSO STATED THAT WHILE DRIVING 30 MPH AND APPROACHING A STOP LIGHT, THE CONTACT ATTEMPTED TO DEPRESS THE BRAKE PEDAL, BUT THE BRAKE PEDAL FAILED TO DEPRESS. AS A RESULT, THE VEHICLE FAILED TO STOP. THE CONTACT APPLIED THE EMERGENCY BRAKE BUT THE VEHICLE VEERED AND CRASHED INTO A GUARDRAIL. THE AIR BAGS FAILED TO DEPLOY. THE BRAKE WARNING LIGHT ILLUMINATED. THE CONTACT SUSTAINED FACE INJURIES THAT DID NOT REQUIRE MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED FROM THE CRASH SCENE TO A DEALER, BUT WAS NOT DIAGNOSED OR REPAIRED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE VIN WAS INCLUDED IN NHTSA CAMPAIGN NUMBER: 15V800000 (SERVICE BRAKES, HYDRAULIC); HOWEVER, THE PART NEEDED TO REPAIR THE VEHICLE WAS UNAVAILABLE. THE FAILURE MILEAGE WAS 28,000.

MY ASSESSMENT, WHICH IS BASED ON ENGINEERING EXPERTISE NOT EMPIRICAL EVIDENCE, SUGGESTS THAT, RATHER THAN RESPONDING TO DRIVER CONTROLLED BY-WIRE" INSTRUCTIONS TO BRAKE PREPARATORY TO A HARD RIGHT TURN INTO OUR DRIVEWAY, THE VEHICLE TOOK CONTROL OF THE THROTTLE, BRAKE, AND (DRIVE-BY-WIRE ?) CLUTCH PROPELLING ITSELF 20 METERS FURTHER DOWN-ROAD TO THE RIVER SHORELINE. PERHAPS READING THE BRAKE COMMAND AS AN ACCELERATION INSTRUCTION (OR ???) THE VEHICLE CONTINUED FORWARD AND DROVE ITSELF OFF THE ROAD, TUMBLING 2 METRES ONTO A BEACH, THEN FROM THE BEACH ONTO THE RIVER ICE WHICH EXTENDS OUT TOWARDS MID-RIVER. IT CONTINUED ACROSS THAT ICE ONLY TO END ITS FORWARD PROGRESS AT THE ICE/WATER BOUNDARY WHERE IT PLUNGED INTO OPEN WATER WITHIN 100 METERS OF HOME. THESE EVENTS EFFECTIVELY ENDING A JOURNEY HOME WITH DEATH. THE OTTAWA CORONER REPORTS NO INDICATION (SUCH AS BLOOD ALCOHOL LEVELS / MEDICAL EVENTS) TO INDICATE THE DRIVER CONTRIBUTED TO THIS OUTCOME IN ANY WAY. IF THE DRIVER IS NOT RESPONSIBLE THEN RESPONSIBILITY FOR THIS EVENT LIES WITH THE VEHICLE AND THE VEHICLE ALONE. IT IS WORTH NOTING THAT (1) MY SON IS A PROFESSIONAL AZ LICENSED DRIVER (2) HAVING FISHED IT OFTEN, IS INTIMATELY FAMILIAR WITH THE ROAD/FERRY BOUNDARY IMMEDIATELY ADJACENT TO OUR HOME (3) WERE MECHANICALLY OPERATED STEERING ALL THAT REMAINED UNDER HIS CONTROL HE WOULD HAVE HAD THREE CHOICES: RIGHT INTO THE RIVER; STRAIGHT AHEAD INTO THE RIVER, OR LEFT ONTO A BEACH FORMED BY THE DEPOSITION OF RIVER SILT OVER TIME (THE CAR WENT LEFT OFF THE ROAD) (4) THE RECORD LEFT BY HIS BODY SHOWS ARM AND FIST POSTURE THAT SUGGEST DEPLOYMENT OF THE AIRBAGS LITERALLY FORCED HIS HANDS OFF THE STEERING WHEEL. THE PARKING BRAKE HAS FAILED ON TWO OCCASIONS, ONCE WHILE STATIONARY WHEN IT WAS AT THE DEALERSHIP. IT CRASHED INTO A CAR IN THEIR PARKING LOT IN 2016. THE VEHICLE HAS BEEN IN AND OUT OF THE DEALERSHIP OVER THE LAST FOUR YEARS. IT WAS IN THE DEALERSHIP IN 2015-2016 THIRTEEN TIMES FOR OIL LEAKS AND THE ENGINE EITHER DYING OR ALMOST SEIZING WHILE IN MOTION. THIS RESULTED IN MY VEHICLE STARTING ON FIRE IN LATE 2016 EARLY 2017. THE REPAIR FOR THE FIRE DAMAGE WAS COVERED BY MY INSURANCE COMPANY AND IT WAS SUBSEQUENTLY FIXED BY A DIFFERENT DEALERSHIP AFTER THAT. MORE RECENTLY, IT WAS IN MY DRIVEWAY AND ROLLED INTO MY TRUCK AND DAMAGED THE FRONT OF THE VEHICLE'S BUMPER WHILE STATIONARY WITH THE PARKING BRAKE WAS ENGAGED. THE SYMPTOMS WITH THE ENGINE HAVE ALSO STARTED TO REAPPEAR WHILE IN MOTION IN 3RD OR 4TH GEAR... IT STARTS TO ACT LIKE IT'S GOING TO COMPLETELY DIE. NEEDLESS TO SAY, I HAVE IT BACK IN THE DEALERSHIP FOR DIAGNOSTICS. SOME OF THE RECALLS LISTED ON THIS WEBSITE SOUND LIKE THINGS I HAVE EXPERIENCED, PLEASE LET ME KNOW IF THERE IS ANYTHING I CAN DO. MY TIME IS THE MAIN THING I CARE ABOUT AT THIS POINT AND DON'T WANT TO SPEND ANOTHER 2 YEARS ENDLESSLY IN AND OUT OF THE DEALERSHIP. I JUST WANT A CAR THAT OPERATES NORMALLY.

ON 9/23/19 I WAS STOPPED AT A RED LIGHT AND WAS HIT IN THE REAR AND WAS PUSH INTO THE VEHICLE IN FRONT OF ME. THE FORCE OF IMPACT WAS SO EXTREME MY CAR WAS TOTALED AND I HAD TO BE TAKEN TO THE HOSPITAL BY AMBULANCE. NOT EVEN ONE AIR BAG DEPLOYED!!! THEY SAY THIS CAR HAS 10 AIR BAGS AND NOT ONE OF THEM DEPLOYED. I COULD HAVE BEEN KILLED BUT BY THE GRACE OF GOD I WASN'T. I WANT ANSWERS AS TO WHY NOT EVEN ONE OF MY 10 AIR BAGS DID NOT DEPLOY. THE ENTIRE REAR OF MY CAR WAS PUSHED INTO THE BACK SEAT, THE FRONT OF THE CAR THE HOOD WAS PUSHED TO THE WINDSHIELD, LIKE I SAID THE CAR IS TOTALED. THIS VEHICLE WAS TRAVELING ABOUT 35 MPH WHEN A VEHICLE NEXT TO IT TRAVELING THE SAME SPEED DECIDED TO MAKE A U-TURN IN FRONT OF IT ON A CITY ONE-WAY ROAD. PLEASE INVESTIGATE THIS ASAP BEFORE THE CAR IS DISPOSITIONED BY THE INSURANCE.
