I am making this complaint in the public interest The car I am complaining about is mangled and burned and the VIN is unavailable so I have used a dummy VIN to file this complaint. This car has just shown up in the wreckers yard. https://www.copart.com/lot/68650122/2016-tesla-model-x-ca-van-nuys It is almost certain that the occupants have been severely injured or killed. 2016 model Teslas are notorious for suspension breakages. The front RH tire has a deep gouge over the entire circumference. This is proof that the suspension failed and gouged the tire BEFORE the crash and therefore actually caused the crash. Image of the gouged tire is attached. You have hundreds of valid complaints filed for these Teslas. How many more have to die before you take action? Please note that details of the crash such as date and number of injuries are not precisely known but it will be easy for you to find out.
A TESLA MODEL X HAS JUST CRASHED AND BURST INTO FLAMES. THE TOP BALL JOINT HAS POPPED APART. EVEN IF THE SUSPENSION WAS HIT IN THE CRASH THE BALL JOINT SHOULD NOT HAVE POPPED. YOU HAVE SEVERAL OTHER COMPLAINTS ON RECORD OF TESLA BALL JOINTS COMING APART. IT IS QUITE POSSIBLE THAT THE BALL-JOINT FAILURE CAUSED THE CRASH. ALSO THE TESLA HAS ONCE AGAIN SELF-IGNITED. CRASH TESTS NEED TO BE DONE WITH A BATTERY THAT IS HOT FROM RECENT CHARGING OR FROM FAST DRIVING TO FIND OUT WHY TESLAS BURST INTO FLAMES SO OFTEN. PLEASE INVESTIGATE
HERE IS A REPORT OF A TESLA MODEL X CRASH IN CHINA. THE DRIVERS AIRBAG FAILED TO DEPLOY RESULTING IN VERY SERIOUS INJURIES. THE REAR SEAT OCCUPANTS WERE UNABLE TO OPEN THE FALCON DOORS. THE EMERGENCY DOOR RELEASE IS NOT OBVIOUS TO OCCUPANTS AND REQUIRES THEM TO REACH UP BEHIND THE SPEAKER GRILL TO FIND THE PULL. THE CAR CAUGHT FIRE AND WAS BURNED TO ASHES. BYSTANDERS WERE UNABLE TO DOUSE THE FIRE USING REGULAR EXTINGUISHERS.. THE SUDDEN SWERVE, FAILURE OF AIR-BAG AND FAILURE OF DOOR RELEASE MECHANISMS IS CONSISTENT WITH FAILURE OF THE 12 VOLT ELECTRICAL SYSTEM OR GROUND CONNECTION AS HAS BEEN REPORTED ON MANY OTHER OCCASIONS. I BELIEVE THAT THIS CAR IS EXTREMELY UNSAFE. PLEASE INVESTIGATE. CONCERNED CITIZEN.
HERE IS A TESLA MODEL X IN THE JUNKYARD. HTTPS://WWW.COPART.COM/LOT/53118179/PHOTOS IT APPEARS TO HAVE BURNED UP IN THE JUNKYARD PRESUMABLY BECAUSE THE BATTERY PACK WAS DAMAGED IN A CRASH AND IT DECIDED TO WAIT A WHILE BEFORE SELF COMBUSTING. BOTH BACK WHEELS HAVE SNAPPED OFF DESPITE THERE BEING NO SIGNIFICANT IMPACT TO THAT PART OF THE CAR. THE BROKEN SUSPENSION ARM FROM ONE OF THE BACK WHEELS HAS BEEN PLACED ON THE PASSENGER SEAT ( SEE ATTACHED PHOTOGRAPH) AND IS STILL RECOGNIZABLE DESPITE THE FIRE. THERE HAVE BEEN HUNDREDS OF CASES OF TESLA SUSPENSION BREAKAGES BOTH FROM OWNERS AND FROM CONCERNED CITIZENS. RATHER THAN INVESTIGATING THE CAUSE OF THESE FAILURES MR. Our insured advised he had his foot firmly on the brake, and without warning the vehicle accelerated forward striking the vehicle in front of him. Was stopped at a red light behind the car in front of me. I was stopped at a appropriate distance from the car in front of me. I had my right foot on the brake pedal, when the car accelerated on its own. It launched its self into the car ahead of me. I looked down and my foot was still on the brake pedal. None of the air bags inside the car deployed.
The contact owned a 2016 Tesla Model X. The contact stated that while his wife was driving approximately 15 MPH from a traffic light, the vehicle was T-boned by another vehicle attempting to run the red light. The contact stated that the other vehicle crashed into the driver’s side front fender panel of the vehicle. The contact stated that the driver’s door was not impacted in the crash. The contact stated that the air bags deployed as needed. However, the contact’s wife was stuck in the vehicle, and unable to exit the vehicle because the doors failed to unlock. The contact stated that the vehicle had lost all electrical functions, causing the doors to remain closed. The contact stated that his wife was unable to move over to the passenger’s side front or rear to exit the vehicle. The contact stated that some pedestrians nearby were able to unlock the door from outside for his wife to exit the vehicle. The contact was concerned that his wife could have been killed if the vehicle had caught on fire or there was some other form of an emergency. The contact stated that his wife sustained a knee injury. Medical attention was not needed at the scene; however, the contact’s wife had since started receiving physical therapy for pain in her knee. The contact was unsure if the other driver had sustained any injuries. A Police report was filed. The vehicle was towed to a tow lot and was deemed a total loss. The manufacturer was not notified of the failure. The failure mileage was approximately 60,000.
I was rear ended while my Tesla was on auto pilot and the system never warned me or prevented the accident

JUDGING BY THE CONDITION OF THE WRECK IT SEEMS LIKELY THAT THE DRIVER WAS KILLED OR INJURED. THE TOP BALL-JOINT HAS POPPED APART. EVEN IN A CRASH IT IS UNUSUAL FOR A BALL-JOINT TO BE POPPED APART LIKE THIS. IT IS HIGHLY PROBABLE THAT THE BALL JOINT POPPING CAUSED LOSS OF CONTROL WHICH THEN RESULTED IN THIS CRASH. THIS CAR HAS DONE 95,000 MILES. IT IS HIGHLY LIKELY THAT THE BALL-JOINT WAS WORN DOWN SUFFICIENTLY TO ALLOW THE BALL TO POP OUT. MANY OF THESE BALL JOINT SEPARATIONS HAVE ALREADY BEEN REPORTED TO NHTSA AND TO THE CHINESE EQUIVALENT ORGANISATION SAMR/DPAC. ON JANUARY 28, 2019 I BACKED OUT OF MY DRIVEWAY IN MY 2016 TESLA X, BRAKED, STOPPED, PUT IT IN DRIVE AND BARELY, SLOWLY STARTED DOWN MY LANE WHEN THE CAR SUDDENLY ACCELERATED AT FULL SPEED DOWN THE LANE. I TRIED THE BRAKE TWICE BUT IT DID NOTHING TO SLOW THE ACCELERATION.APPROXIMATELY 75 YARDS DOWN THE LANE THE TESLA ENTERED THE CROSS STREET BELOW AND CLIPPED ANOTHER CAR, DEMOLISHED A LARGE BANK OF MAILBOXES ACROSS THE STREET, TOOK BRIEFLY TO THE AIR SEVERAL FEET DUE TO THE HILLSIDE THERE, HIT A LARGE CEDAR TREE, GLANCED OFF AND FINALLY STOPPED NOSE DOWN, HIGH SIDED ON A LARGE LANDSCAPE BOULDER. BUT FOR THE BOULDER, I WOULD HAVE CONTINUED DOWN INTO THE DEEP RAVINE BEHIND THE NEIGHBOR'S HOUSE. THE AIRBAGS HAD DEPLOYED DURING THESE EVENTS. I WAS TAKEN TO THE HOSPITAL BY AMBULANCE AND DIAGNOSED WITH A T-12 SPINAL COMPRESSION FRACTURE, WHICH IS STILL PAINFUL A YEAR LATER. THE CAR WAS TOTALED. THE EYE BOLT THE 2016 TESLA MODEL X THAT IS USE TO LOAD A TESLA ON TO A TOW TRUCK IS DEFECTIVE.THE LOADING MANUAL IS WRONG AS WELL. 08/24/2018 AROUND 10 AM MOUNTAIN TIME IN UTAH. THE LOCATION WAS AT SERVICE KING IN SALT LAKE CITY, UTAH. VEHICLE VIN # [XXX] 2016 MODEL X. THE EYE BOLT USED TO LOAD THE VEHICLE ON TO A WRECKER FOR TRANSPORT IS DEFECTIVE. THE LOADING MANUAL DOES NOT ILLUSTRATE A SAFE METHOD TO LOAD VEHICLE USING THIS EYE BOLT. I WAS LOADING THE VEHICLE VIA THE INSTRUCTIONS OF THE TESLA MODEL X MANUAL AND AS SOON AS I GOT THE VEHICLE TO THE SPOT TO PUT A CHALK BEHIND WHEEL, THE BOLT BROKE AND THE VEHICLE CAME OFF THE DECK AND ALMOST RAN OVER A KID WORKING IN THE BODY SHOP. I GOT SLIGHTLY INJURED TRYING TO STOP VEHICLE FROM HITTING THE WALL. THE BODY SHOP REFUSES TO REPORT THIS SAFETY ISSUE TO TESLA. WHEN I TRIED TO REPORT THIS TO TESLA I WAS HUNG UP ON 2 TIMES. BOTH LOWER BALL JOINT STEMS HAVE SNAPPED. THESE ARE STEEL COMPONENTS THAT ARE STRONGER THAN THE ALLOY PARTS THEY ARE ATTACHED TO. SOMETHING IS VERY WRONG IF THE BALL AND SOCKET CAN POP APART LIKE THIS EVEN IF IT DID HAPPEN AFTER THE CRASH. THE WHEEL HAS BEEN SLICED IN HALF AS IF IT WAS MOUNTED ON A LATHE AND A CUTTING TOOL FORCED INTO THE RIM. IT LOOKS HIGHLY PROBABLE THAT THE WHEEL WAS SLICED IN HALF WHILST IT WAS STILL ROTATING AND THAT ALMOST CERTAINLY HAPPENED BEFORE THE CRASH. THERE IS NO OTHER MAKE OF CAR IN WHICH NEAR NEW CARS ARE SHOWING UP IN THE JUNKYARDS WITH BROKEN SUSPENSIONS AND SLICED WHEELS AT ANYWHERE NEAR THE FREQUENCY THAT TESLA CARS DO. TAKATA RECALL I DROVE MY TESLA MODEL X 2016 TODAY TO WORK AND WHEN I WAS ABOUT TO PARK THE CAR IN THE PARKING LOT (AROUND 6 MILES PER HOUR MAY BE) IT SUDDENLY ACCELERATED AND HIT THE CONCRETE WALL AND BOUNCED BACK AROUND 8 FEET. SINCE IT WAS FOR PARKING I CAN SURELY SAY THAT I DID NOT ACCELERATE THE CAR. THE STEERING AIR BAGS AND KNEE AIR BAGS CAME OFF AND ALSO THE PASSENGER SIDE KNEE AIR BAGS CAME OFF AS WELL. I SEARCHED ONLINE AND THERE SEEMS TO BE A CLASS ACTION SUITE ON THIS ISSUE BUT TESLA IS NOT ACCEPTING IT AS THE GLITCH IN THEIR SOFTWARE OR SOME OTHER COMPONENT. I FELT LIKE THE ACCELERATOR GOT PRESSED THE WAY WHEN THE CAR WAS IN CRUISE MODE. UNLESS I WANTED TO HIT THE WALL INTENTIONALLY THERE WAS NO NEED FOR ME TO PRESS THE ACCELERATOR TO SPEED FROM ALMOST ZERO TO WHATEVER THE HIGH SPEED IT ATTAINED AT THE TIME OF HITTING THE WALL.
I HAD PULLED INTO A PARKING LOT, PROCEEDED TO PULL INTO A SPOT ADJACENT TO A CINDER BLOCK BUILDING. I HAD MY FOOT LIGHTLY ON THE GAS PEDAL, THEN AS I MADE THE TURN INTO THE SPOT, MY FOOT WAS ON THE BRAKE - THE CAR LURCHED FORWARD AND SPED UP AND THE BRAKES DID NOT STOP IT. I WENT RIGHT INTO THE CONCRETE BUILDING, HEAD ON - AIR BAGS DEPLOYED. THE FRONT END CRUSHED AND THE 2 AIRBAGS ON THE DRIVERS SIDE DEPLOYED AND WERE SMOKING. I READ ON LINE THAT THERE HAVE BEEN NUMEROUS INCIDENCES OF THIS HAPPENING WITH THE TESLA. SPONTANEOUS ACCELERATION WITH MY FOOT NOT ON THE GAS PEDAL. THE CAR WOULD NOT STOP BY THE BRAKES! I COULD HAVE BEEN SERIOUSLY INJURED OR HIT ANOTHER PERSONA OR VEHICLE. THE CAR HIT THE BUILDING AS WELL AS A NATURAL GAS PIPE THAT WAS RUNNING ALONG THE BUILDINGS SIDE AT THE LEVEL OF MY FRONT BUMPER. I FILED A POLICE REPORT.
