What component or system failed or malfunctioned, and is it available for inspection upon request? Response: Frame, Suspension, Airbags / Yes it is available for inspection. How was your safety or the safety of others put at risk? Response: Safety was put at risk due to frame & suspension failure due to severe rust. Also due to failure of deployment of AirBags. Rusted Frame Failure subsequently caused safety of Injured Party and other motorists to be at risk due to the Injured Party’s 4Runner to dart across oncoming lane of traffic with no warning other than a swaying motion felt immediately before losing control. Once the Injured Party lost control of the 4Runner it crossed into oncoming traffic, progressing into the drainage ditch on the oncoming traffic side of highway. Once the 4Runner impacted the drainage ditch at a 90° angle, the 4Runners momentum dispersed by causing the 4Runner to overturn 3 times landing on its wheels in the adjoining field. Has the problem been reproduced or confirmed by a dealer or independent service center? Repaonse: Independent Service Center has confirmed the problem with the Rusted Frame, Rusted Suspension, and Air Bag failure. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Response : Yes Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Repaint: No warning lamps prior to failure. Symptoms indicative of failure were an unknown minor swaying sensation while driving. Injured party was under the impression that there may have been a tire low on air, which was discovered to be incorrect later.
The contact owned a 2004 Toyota 4 Runner. The contact stated that the driver’s and front passenger air bags failed. The contact stated that when at complete a stop at the red traffic light, the vehicle in the rear failed to stop and crashed into the contact's vehicle rear-end, pushing the vehicle into the roadway. The driver’s and front passenger air bags failed to deploy and caused the front passenger's head to hit the dashboard, suffered injuries to the head, six broken back vertebrae, three broken ribs, head concussion. The contact was the driver and had no injuries to report. The front passenger who was her husband was airlifted to the hospital. The State Trooper mentioned that the other driver might have been on their phone however it was unknown due to them suffering fatal injuries. A police report was filed. The vehicle was towed. The contact advised that the vehicle was destroyed. The manufacturer was not contacted. The failure mileage was 238,000.
THE AIRBAGS FAILED TO DEPLOY UPOND IMPACT FROM A TRACTOR TRAILER. CAR COMPLETELY TOTALED.
AIR BAGS FAILED TO DEPLOY. I HAD SENT IN A COMPLAINT AFTER THE INCIDENT. CAR WAS TOTALED AND DRIVER WAS INJURED WITH A FRONT END COLLISION. TOYOTA REFUSED TO LOOK AT CAR FOR AIRBAG DEFECTS. I CAN SEND IMAGES IF NEEDED. RECALLED FOR TOO MUCH FORCE? HOW ABOUT JUST DEFECTIVE? THIS IS AT LEAST AS DANGEROUS AS DEPLOYING WITH TOO MUCH FORCE IN MY OPINION. *TR
TL* THE CONTACT OWNS A 2004 TOYOTA 4-RUNNER. THE CONTACT WAS DRIVING 2 MPH UPHILL WHEN THE VEHICLE SUDDENLY ACCELERATED AND CRASHED INTO A TREE. THERE WERE NO WARNINGS PRIOR TO THE FAILURE. THE CONTACT WAS INJURED BUT THERE WAS NO POLICE REPORT FILED. THE VEHICLE WAS INSPECTED BUT NO TECHNICIAN WAS ABLE TO EXPLAIN WHAT CAUSED THE CRASH. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE CURRENT MILEAGE WAS APPROXIMATELY 110,000 AND THE FAILURE MILEAGE WAS 87,643.
I WAS DRIVING MY CHILDREN TO SCHOOL AND A CAR COMING IN THE OPPOSITE DIRECTION TRIED TO MAKE A LEFT HAND TURN IN FRONT OF ME, SHE DID NOT HAVE TIME TO MAKE THE TURN( I DIDN'T EVEN HAVE TIME TO PUT ON BRAKES) WHICH ENDED UP IN A HEAD ON COLLISION. I WAS GOING AROUND 45MPH AND MY AIRBAG DID NOT DEPLOY. NO ONE WAS SERIOUSLY HURT BUT MY HEAD AND UPPER BODY DID GET BRUISED AND BANGED UP PRETTY BAD. THE MANUFACTURER WAS NOT CONTACTED TO REPORT THE FAILURE. THE CONTACT STATED THAT THE AIR BAGS SHOULD HAVE DEPLOYED IN THE TYPE OF FRONTAL CRASH HE WAS INVOLVED IN. THE FAILURE MILEAGE WAS 62,000.
FRONT END COLLISION WITH SEMI WHERE AIR BAGS FAILED TO DEPLOY. HE SAID/SHE SAID SITUATION. TRUCKER STATED HE WAS BROADSIDED BY DRIVER WHO RAN RED LIGHT. AIR BAGS DID NOT DEPLOY. DOES THE SPEED OF THE CAR TRAVELING MAKE A DIFFERENCE? DRIVER OF CAR HAD SPRAINED NECK AND CONCUSSION AND WAS WEARING SAFETY BELT. HAD BRUISES FROM SAFETY BELT ON SHOULDER. HEAD-ON COLLISION WHILE STOPPED AT LIGHT BY PANEL VAN TRAVELING AT 25 MPH, POLICE ESTIMATE, FRONT AIR BAGS FAILED TO DEPLOY. *AK
DT*: THE CONTACT STATED WHILE DRIVING 50 MPH, THE VEHICLE WAS INVOLVED IN A COLLISION WITH A GUARDRAIL. THERE WAS A 5 SECOND DELAY IN THE DEPLOYMENT OF THE AIR BAG, CAUSING THE CONTACT AND A PASSENGER TO HIT THE WINDSHIELD. BOTH OCCUPANTS INCURRED HEAD INJURIES, LACERATIONS, MUSCULAR INJURIES, AND THE CONTACT INCURRED OCULAR INJURIES. THE VEHICLE WAS CONSIDERED A TOTAL LOSS, AND A POLICE REPORT WAS TAKEN. THE VEHICLE WAS NOT INSPECTED BY A SERVICE DEALER OR PRIVATE REPAIR SHOP.
TRAVELING 55 MPH ON CA INTERSTATE WHEN CAR DIRECTLY IN FRONT OF ME ABRUPTLY STOPPED. THERE WAS NOT ENOUGH TIME FOR ME TO STOP, SO I IMPACTED THE REAR OF THE VEHICLE HEAD ON AT APPROXIMATELY 45 MPH. NOT ONE OF THE AIRBAGS IN MY VEHICLE DEPLOYED. CONTACTED TOYOTA ABOUT THIS AND THEY SAID IT IS THEIR "OPINION THAT THE AIRBAGS SHOULD NOT HAVE DEPLOYED IN THIS SITUATION." I ASKED THEM WHERE THE FRONTAL IMPACT SENSORS WERE, AND WAS TOLD DIRECTLY UNDER THE HEADLIGHTS IN THE BUMPER. WELL, BOTH SENSORS WERE IN THE IMPACTED AREA ON MY VEHICLE, THE ENTIRE BUMPER AND ASSEMBLY IS BEING REPLACED. THE COST TO REPAIR THE DAMAGE IS $10,000.00 THE OWNERS MANUAL SAYS THE AIRBAGS WILL DEPLOY IN A FRONTAL CRASH AT APPROXIMATELY 15 MPH, I WAS DOING AT LEAST 45 MPH! TOYOTA REFUSES TO ADMIT THE FAILURE OF THE AIRBAGS, AND NOW I WILL BE DRIVING AN UNSAFE VEHICLE. *JB
Unknown: we were on [XXX] when the car went out of control. It first spun one way and then the other way and then we hit the concrete abutment where the guardrail connects to it. We hit it head on. It was as if we were on ice or oil, unable to gain control or brake. We checked the road and it was dry. Other cars were going by with no problem. The airbags didn’t deploy. MY 4RUNNER'S FRAME BROKE WHILE DRIVING DUE TO RUST. THERE'S A RECALL FOR TACOMA'S BUT NOT 4 RUNNERS. I THANKFULLY WAS NOT INJURED BUT NOW I'M WITHOUT A VEHICLE. WHEN I BOUGHT MY 4 RUNNER I WAS UNDER THE IMPRESSION TOYOTA WAS #1 WHEN IT CAME TO SAFETY FOR THEIR CUSTOMERS. PLEASE HELP ME

DROVE 4RUNNER 10 MINUTES, PARKED, TUNED OFF AND STARTED BACK UP JUST FINE. 4.) 4RUNNER SAT FOR 3 HOURS, STARTED 4RUNNER, BUZZER/WARNING SOUND EMITTED AND BRAKE AND ABS LIGHT WERE ON. 5.) TRIED DISCONNECTING BATTERY TO SEE IF SYSTEM WOULD RESET AFTER SPEAKING TO TOYOTA DEALER TO NO AVAIL. 6.) BACKED 4RUNNER OUT OF PARKING SPOT, LIGHTS AND BUZZER STILL IN EFFECT, AND 4RUNNER HAD NO BREAKS. 7.) TOOK 4RUNNER TO DEALER, DEALER CLEARED CODES, BATTERY TEST, RESET BRAKE SYSTEM, TEST DROVE, ALL WORKED. 8.) DROVE 4RUNNER APPROXIMATELY 10-11 MILES, HIT BRAKES, BRAKES FAILED, ABS AND BRAKE LIGHT ON, WARNING/BUZZER SOUND, CRASH - NO FATALITIES. 9.) CODES FROM DEALER C1223, C1251, C1256; BRAKE FLUID LEVEL GOOD. 10.) DEALER REPLACED ABS AND MASTER CYLINDER. $3,356 REPAIR COST. 11.) DEALER (FROM MULTIPLE DEALERS IN THE ORLANDO, FL MSA) INDICATED THIS IS A COMMON OCCURRENCE AND ONLY BY UNACCEPTABLE ODDS DO YOU AVOID CRASH, DEATH OR SERIOUS INJURY. 12.) TOYOTA HAS NO SAFEGUARDS FOR EARLY INDICATION PROBLEM COULD OR MAY OCCUR. 13.) TOYOTA HAS NO MAINTENANCE SPECIFICATIONS FOR PREVENTING OR RECOGNIZING A POSSIBLE FATAL MALFUNCTION IN THE BRAKING SYSTEM. 14.) TOYOTA HAS NO ECONOMICAL SOLUTION TO RESOLVE/REPLACE THE ABS PUMP/MASTER CYLINDER AND IS IN THE TOP 5 MOST EXPENSIVE PARTS OF THE VEHICLE. 15.) NUMBER 11V113000 (VEHICLE SPEED CONTROL) HOWEVER, THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS 164,000.
I EXPERIENCED A TERRIFYING "ACCELERATION PROBLEM" THIS PAST WEEK AS I WAS PARKING MY CAR AT A LOCAL STORE. I'D ALREADY SLOWED DOWN TO ALMOST A HALT, BUT AS I PUT MY FOOT ON THE BRAKE TO COME TO A COMPLETE STOP, IT SUDDENLY LURCHED FORWARD WITH SHOCKING SPEED. I INSTINCTIVELY THREW THE GEAR INTO PARK; IT STOPPED AS IT CRASHED INTO THE STEPS LEADING TO THE STORE ENTRANCE. THE CAUSE HAD NOTHING TO DO WITH A FLOOR MAT (I DON'T HAVE ANY) NOR "STICKING" GAS PEDAL (THE CAR HAD ALREADY COME ALMOST TO A HALT BEFORE IT SUDDENLY ACCELERATED). THE BRAKES WERE USELESS AND FELT LIKE THEY WERE DISENGAGED; ACCELERATION MAY HAVE OCCURRED AFTER DEPRESSING THE BRAKE PEDAL (THIS IS STILL CLOUDY IN MY MIND). I BOUGHT THE CAR NEW, IN 2004. THIS IS THE FIRST TIME I'VE HAD ANY PROBLEM. I WONDERED WHAT I MIGHT HAVE DONE TO CAUSE THE PROBLEM AFTER SO MANY YEARS WITHOUT ANY. I REALIZED I'D NEVER USED THE CRUISE CONTROL FEATURE, UNTIL RECENTLY, ONLY IN THE PAST FEW MONTHS. I WAS NOT USING THE CRUISE CONTROL FEATURE WHEN THE INCIDENT OCCURRED, BUT THE SUDDEN FORWARD SURGE FELT EXACTLY LIKE WHAT HAPPENS WHEN THE CAR IS ON CRUISE CONTROL AND DROPS BENEATH THE SPEED AT WHICH IT'S SET; IT SURGES FORWARD, ON "ITS OWN." I BELIEVE THERE'S A CAUSAL RELATIONSHIP. MY CONCERN IS TWO-FOLD: FIXING A POTENTIALLY DEADLY PROBLEM IN MY OWN TOYOTA, AND HELPING IDENTIFY THE CAUSE OF THE PROBLEM SO IT CAN BE FIXED BY TOYOTA BEFORE ANOTHER TRAGEDY. *TR
VEHICLE WAS BEING PARKED ALONGSIDE THE CURB WHEN IT SUDDENLY JUMPED THE CURB, THE SIDEWALK ONTO THE UPHILL GRASSY LANDSCAPE, HIT AND SEVERED A TREE, AND ALMOST CRASHED THE HOUSE INCHES AWAY, THEN BOUNCED BACK TO THE STREET- ALL IN ABOUT 3 SECONDS. THE TIRES SPUN VIOLENTLY AND LEFT A THICK RESIDUE OF RUBBER AND THICK SMOKE THROUGHOUT. THE BRAKE WAS HARD AND STUCK , AS I MANAGED TO PUT THE STICK TO NEUTRAL AND THEN TO PARK. I WAS TRULY SHOCKED AND SHAKEN THAT I COULD HAVE HURT OR KILLED MYSELF OR OTHERS. THIS IS THE SECOND TIME THIS HAPPENED- THE FIRST IN JULY 22, 2009 WHEN THE SAME ACCELERATION HAPPENED WHEN I WAS MOVING THE VEHICLE INTO THE GARAGE, SURGED IN ABOUT 2 SECS. CRASHING INTO THE STORAGE AND THE GARAGE WALLS. REFERENCE: TOYOTA CLAIM NO.[XXX] (2009) AND NO.[XXX] (2012). IN BOTH CASES TOYOTA DISOWNED ANY RESPONSIBILITY. WHILE TOYOTA ASSURES ME OF THE SAFETY OF THE VEHICLE, I CANNOT IN MY RIGHT MIND CONTINUE TO DRIVE THIS VEHICLE NOR PASS IT ON TO SOMEBODY ELSE EXCEPT TO RETURN IT TO TOYOTA.
