Hello, My name is [XXX] and I had the 2022 Tesla Model Y performance. I had an accident on [XXX]. Car just took control itself, we hit the tree. It was in a residential area, it happened so fast that I could not react to prevent it. After we hit the tree, the car caught fire, It was a nightmare for me, I had my best friend with me at the time of the accident in the front passenger seat. Both of us were unconscious, by the time I got awake, the car was in flames, my friend was still unconscious but then he got awake after I tried with the local neighbor to pull him out from the car. Unfortunately my friend passed away after an accident in 5 hours. I had autosteer assistance on my car as a subscription. Plus my car had a problem with the side cameras, from time to time it was giving me errors - Door Pillar error message. Cops were trying to get information from the car, but according to Tesla representative my car was out of network from June 30, 2022. And I never got any notifications that my car is out of network. I have a case against me VS State of New Jersey, the prosecutor's office charged me with vehicle manslaughter, at the time of accident i gave a statement to the police officer, in December 12, 2023 it was Nationwide recall on Tesla cars, and this recall it was exactly for the same problem what I described to the police officer at the time of accident, so my problem is, I can't retrieve any information from Tesla regarding what happened at the time of accident, DUE TO TESLA had my car out of network for 25 days prior. I can't prove my innocence because of that I'm facing serious charges right now. My complain is about my car being out of TESLA NETWORK FOR 25 DAYS, WITHOUT PROPER NOTIFICATION ON THE APP OR EMAIL, WHILE MY CAR WAS OPERATED EVERYDAY ON THE DAILY BASIS. Same fatal error I've been reporting for over 6 months and about 10 "OVER THE AIR UPDATES" Yes we all really 'NEEDED' more ways to make fart and poop sounds and open our rainbow LED [XXX] by voice command BUT WHAT ID REALLY LIKE IS FOR LITERALLY ANYONE IN ALL OF AMERICA TO DO THEIR JOB! Maybe if someone would do an investigation into to how much FRAUD there is at literally every level of Tesla from the very top all the way to the pathetic worms at the bottom that somehow find the one vertebrae they have left to continue lying to protect the bank account of a lying megalomaniac. Maybe they would investigate how Tesla vehicles can be used as remote operated drones to kamakazi themselves and kill anyone in the vehicle you don't like or even worse they could hack it thru the radio and use it to crash into literally anyone on the road as described by [XXX] himself and the FBI when asked about drone technology. I mean if [XXX] is allowed to spy on all of planet earth without their consent, freeze their car radio, miss every auto payment for 8 months, keep your car in an overflow lot for 3 days on hidden camera and never work on it once 5 times, make your car stop on the highway 50 time, stop your car from charging at home with your own charger in your own house then why does anyone follow any rules? Why don't we all just go out in the street and rob and kill and rape anyone whos dumb enough to believe that a car can drive itself? Really tho why do I even bother doing any of this everyone on the internet just calls me a stupid, fat, [XXX], [XXX], [XXX] fan,[XXX]. So like why would you EVER buy a Tesla. It's made by SLAVES FOR F SAKE! I'm a slave for Uber, Tesla and Apple. Slavery is real. I lost $28,000 last year doing 1,000 Uber and Lyft rides in an electric car that's supposed to save money on gas but actually costs more than gas if you use a supercharger. Two incidents: 1) Non-accident: My wife was slowing to make a right turn at a stop light and the steering locked up for about 3 seconds, where she wasn't able to regain manual steering control. We did not have full self-driving mode at the time. Luckily, she was only going about 10 mph, and was able to brake before running into oncoming traffic. 2) Accident: Our son was driving on a country road, along a section with a slight curve. We had a free trial for full self-driving mode during the previous month, but it ended two weeks before the accident, so he could not have had it in self-driving mode. It appears that he didn't make the curve, but as evidenced by a single long tire skid mark, continued almost perfectly straight, across the oncoming lane, and ran into a redwood tree. The car rolled and ended up upside down, against the bank back on the right side of the road, with the lithium batteries on fire. Our son and his passenger sustained severe injuries and neither has a memory of what caused the accident. Naturally we suspect the possibility that the steering locked up again. But it also seems likely that the AEB and ABS were not functioning correctly, as if they were, there should have been several short skid marks for the tires on both sides of the car, rather than only a single long skid mark. Additionally, there's no shoulder on that road, and there is a clump of bushes that come very close to the white line, so it's possible that a steering auto-correct initiated the incident. We have not yet been able to recover EDR or log data from the vehicle.
We live on a road with a steep embankment. My wife backed across our driveway and when she put the car from reverse to drive it accelerated uncontrollably. She hit our mailbox, then a power pole across the street, a driveway across the street then over the embankment where she hit a retainer wall another wooden railing and finally came to rest against our neighbors house. If she wouldn’t have caught the edge of the house she would have went down approximately 150 foot embankment. Not only the car accelerated on its own it is supposed to automatically brake, it didn’t and the airbags never deployed. This all this happened right after it went through an automatic update. The car was totaled, with my wife getting a concussion and a lot of muscle injuries. There was no warning lights prior to event and we reported it to Tesla and all they did was send camera images which we had. The car is now in possession of Wawanesa Insurance because it was a total loss
The contact owned a 2022 Tesla Model Y. The contact stated that while the contact’s daughter was driving at approximately 70 MPH under significant rainy weather conditions, with autopilot mode engaged, the auto pilot became inoperable. The contact stated that the driver attempted to steer the steering wheel; however, the steering could not be steered, causing the driver to lose control of the vehicle. The vehicle subsequently collided with a concrete wall and might have collided with another vehicle. There was no warning light illuminated. The contact stated that all the air bags of the vehicle deployed during the crash. As a result of the incident, the driver sustained a nose injury and facial lacerations. The driver was transported to a local Emergency Room for medical treatment. A police report was filed. The vehicle was towed and deemed a total loss. A Tesla service center was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure, but no assistance had been provided. The failure mileage was approximately 55,000.
On [XXX], while using Full Self-Driving (FSD) to drive to a hospital in Oak Park, Illinois, the vehicle experienced sudden unintended acceleration during the disengagement of FSD. As the vehicle approached the hospital area, I initiated disengagement of FSD to manually enter the parking area. During—or immediately after—the disengagement, the vehicle suddenly accelerated at full power without warning, causing an immediate loss of control and a collision with a nearby fixed structure. No system warnings or alerts were issued prior to the event, and the acceleration was so abrupt that manual intervention was not possible. This occurred in a low-speed, street-level environment where no acceleration was intended or expected and does not correspond to any normal driver input. Based on review of preliminary incident data provided by Tesla, there appear to be irregularities during the FSD-to-manual control transition, including missing or invalid input values, gaps in accelerator/brake/steering signals, and speed readings inconsistent with the crash severity. These indicators suggest potential instability in the real-time control system during disengagement. This report is submitted due to concern that sudden unintended acceleration during FSD disengagement may represent a safety defect with potential risk to drivers and the public. Vehicle Information: [2022, Model - Y, ] Incident Summary: While driving with FSD engaged, the vehicle hydroplaned and the steering wheel started spinning uncontrollably and despite attempts, couldn't regain control of the car, and it spun while crossing lanes. A truck hit the front of the car and ended up on shoulder.

The contact attempted to apply the brake pedal, but the brakes failed, and the vehicle accelerated forward at a higher speed. The vehicle crashed into an unoccupied vehicle parked in the garage, causing the other vehicle to crash into the garage wall and into the laundry room inside the home. The Tesla's air bag deployed. The contact confirmed that no self-driving or other feature was activated at the time of the failure. The vehicle was towed and deemed totaled. The contact sustained injury to their neck, knee, and hands. Medical attention was sought via ambulance. A police report was filed. The manufacturer was informed of the failure and opened a case. The failure mileage was 45,417.
I was driving the car on supervised self drive and going home around 9 pm. When I reached my house I tapped the break to disengage the FSD and take over so that I can pull into our driveway. After coming to a stop the car all of a sudden just took off on its own at almost 60 mph on a residential street. I was repeatedly pressing on the brake to try to control the car. But it seemed like the car had a mind of its own and was not responding to me at all. approximately 8 seconds (based on Tesla dashcam recordings) later after travelling a distance of approximately 700 feet the car hit 2 parked cars on the street, which eventually activated the brake and brought the car to a stop at the front yard of a house across the street from where the parked cars were hit.

I just got T-Boned in my 2022 Model Y performance. I got out of the vehicle which wasn't in terrible condition, in fact the left side of the vehicle was still perfect and the doors worked fine. I had my dog with me and everyone was okay, but I opened my door and the back passenger door to grab a jacket and check on my dog. I shut the doors to make sure he didn't run out and was a bit out of it because of the accident impact so I took a moment to compose myself.. All of the car's lights flickered and then the electric cut off leaving the doors completely locked and inoperable even though they had just been used moments before and were not damaged in the accident. I had to climb in through a shattered glass window to get my dog as all the doors wouldn't open from the outside and I was unable to access the glove box, nor pry it open because the electricity was out and the impact caused that door to make the side panel to unlock the glove box manually accessible. If the car DID catch fire or an occupant were bleeding out: Anyone inside, unconscious, would be unable to use the manually release and would be burned alive while people pull on doors that won't open that were undamaged in the accident and working 60 seconds before. From what I have read the only solution would be to jump the car using the 12V battery in the Frunk of which the Tow company and Emergency Responders were not familiar with.
I was sideswiped by another vehicle which just joined our direction of traffic coming out of a cutout. As soon as I saw the vehicle moving fast from inside the cutout to join our direction and into my lane, I immediately decided to move to the lane next to me. I was at the leftmost lane at the time. Halfway into the lane I just felt the violent impact of the other vehicle hitting us. My wife and I were shaken so bad, we were shocked and terrified at what just happened and what else might happen to us. I felt like we were travelling on the front wheels with our rear end up in the air. Next thing we knew, the car stopped in front of a tree. Our iphones and the our car's phone called 911. We were pulled out of the car and taken immediately into an ambulance and Orange Global Hospital, where we stayed for about 2 days. X rays, CT Scan and MRI were taken. NOTE: A ROD STICKING OUT FROM THE FLOOR HIT MY WIFE during the accident. This rod was connected to the wheel and was not supposed to be stick out inside the car during an accident! I BELIEVE THAT A DEFAULT IN CAR DESIGN CAUSED THIS ROD TO HIT MY WIFE , WHICH CAUSED SO MUCH PAIN, DOCTORS COMPARED HER BRUISED RIBS TO BROKEN RIBS!!

Safety Risks: The sequence of malfunctions, particularly the recurring issues with the Rear Drive Motor unit followed by a severe loss of control and airbag system failure, presents an extreme safety risk. The absence of warnings prior to these failures compounds the danger, suggesting systemic safety and reliability flaws. Conclusions & Reporting: The vehicle's history of critical malfunctions within a short mileage interval demands an in-depth examination by the manufacturer and safety regulatory authorities. I was stopped at a light, and I as I continued my trip after the green. I had put the car in auto pilot. As I approached the curve there deer to my left that got my attention. The vehicle didn't follow the road, nor did it announce with any warnings, or lane assists that the vehicle was departing the lane. Impact was made into a broken guard rail. Guard Rail impaled the vehcile from the left side into the cabin impacting my left leg causing serious injuries.

The contact stated that while his daughter was driving at approximately 65-70 MPH in rainy weather, the contact's daughter released her foot from the accelerator pedal to decelerate; however, she lost control of the vehicle. The vehicle crashed into a guardrail and went into a ditch. The vehicle then inadvertently reversed onto the highway. The contact's daughter slammed her foot on the brake pedal, and the vehicle stopped immediately. No warning light was illuminated. No air bags were deployed. A police report was filed. The contact's daughter suffered body aches, and medical attention was received. The vehicle was towed to an unknown location. The contact was unaware if the vehicle was deemed totaled by the insurance company. The manufacturer was not made aware of the failure. The failure mileage was unknown. The VIN was unavailable.
I was driving my Tesla Model Y on 02-26-2022 with my wife in the passenger seat. We were on our way to pick up our son and his two friends from the movie theater in Lincoln Square North, Bellevue Square, Washington. As we entered the Lincoln Square parking garage approximately around 6 30pm and turned into the narrow passage leading down to the several floors of parking lots at approximately 5 miles an hour, the Tesla on its own, and to my alarm and surprise suddenly picked up tremendous speed and went careening down the parking driveway and slammed to a stop into the concrete boundary of the Pillar on parking level P2. The distance travelled from start to finish was to the best of my very rough guesstimate 60ft to 70 ft. My repeated attempts at slamming the brakes did not stop the car. The airbags deployed. •With the impact the car was completely totaled •The front right wheel broke free and hit the rear fender of a car parked in the Parking Garage in P2. - No one in the parking garage was impacted My wife was in great pain and very shaken. She was helped out of the car by a good Samaritan. Lincoln Square security reached us almost immediately. We called 911 and the fire brigade and police came to the accident site within minutes. The police inspected the vehicle and filed a case report. Post the accident, I took my wife to the ER. She was badly bruised with the impact of the air bags and seat belt restraint. We continue to get treated to manage the pain and the bruising. The experience was a horrific one and my wife has been badly impacted …it will take time for her to begin driving again. We have not been contacted by Tesla regarding this accident as of March 9, 2023.
