TL*THE CONTACT OWNS A 2006 TOYOTA RAV4. WHILE DRIVING 45 MPH, THE DRIVER STRUCK A CONCRETE POST AND NONE OF THE AIR BAGS DEPLOYED. THE WEATHER WAS RAINY, WINDY, AND VERY COLD. THE DRIVER SUFFERED CONGESTIVE HEART FAILURE AND DIED. THE VEHICLE HAS BEEN REPAIRED, BUT THE CONTACT DOES NOT WANT TO DRIVE THE VEHICLE AND DOES NOT BELIEVE THAT IT IS SAFE TO DRIVE. THE DEALER STATED THAT THE VEHICLE HAS MAJOR FAILURES AND SHOULD NOT BE DRIVEN. A POLICE REPORT WAS FILED. THE CONTACT HAS PICTURES. THE POWERTRAIN WAS UNKNOWN. THE CURRENT AND FAILURE MILEAGES WERE 41,000.

"TAKATA RECALL", I HAD AN ACCIDENT IN WHICH MY VEHICLE FRONT END COLLIDED WITH A TREE. MY AIR BAG DID NOT DEPLOY, AND UPPER SEAT BELT DID NOT LOCK CAUSING INJURY TO MY FACE. I WAS ON A CITY STREET IN MOTION. I WAS TAKEN TO HOSPITAL VIA AMBULANCE.
TL* THE CONTACT OWNS A 2006 TOYOTA RAV4. THE CONTACT WAS WAITING TO ENTER A HIGHWAY WITH THE BRAKES DEPRESSED WHEN THE ENTIRE INSTRUMENT PANEL ILLUMINATED. THE VEHICLE THEN ABNORMALLY ACCELERATED AND SWERVED INTO TRAFFIC AS ANOTHER VEHICLE CRASHED INTO THE DRIVER SIDE OF HER VEHICLE. THE CONTACT SUSTAINED INJURIES TO THE HANDS AND ARMS. THE VEHICLE WAS TOWED TO THE DEALER BUT NOT REPAIRED. THE FAILURE AND CURRENT MILEAGE WAS 65,132.
I WAS HEADING THROUGH AN INTERSECTION AND STRUCK AN ONCOMING VEHICLE, WHICH FAILED TO YIELD TO ME. I STRUCK HIM IN THE FRONT PASSENGER SIDE SECTION OF A FORD RANGER. THE VEHICLE HAS NOT BEEN REPAIRED YET BUT IS WAITING FOR THE OTHER INSURANCE COMPANY. *TR
TL*THE CONTACT OWNS A 2006 TOYOTA RAV4. WHILE DRIVING APPROXIMATELY 40 MPH, THE VEHICLE CRASHED INTO THE FRONT END OF ANOTHER VEHICLE. THE VEHICLE WAS DESTROYED AND THE AIR BAGS FAILED TO DEPLOY. THE CONTACT WAS INJURED AND A POLICE REPORT WAS FILED. THE MANUFACTURER WAS NOT NOTIFIED. THE VIN WAS INVALID. NO OTHER VEHICLE INFORMATION WAS AVAILABLE.
While my son was driving me the vehicles steering wheel locked and he was unable to steer the vehicle. It caused us to go off the road down a steep hill and wreck. I’ve never had any problems with this vehicle so it was completely unexpected. If there had been anyone in our path we wouldn’t of been able to stop from hitting them. It has not been inspected but there are witnesses to the accident
THIS COMPLAINT IS RELATED TO DISCOVERY SRX P215/70R16 COOPER TIRES WHICH WERE BOUGHT ON OCTOBER 23 2018. THESE TIRES ARE IDENTICAL TO RECALL 'NHTSA RECALL NO. - 19T-005 AFFECTING COOPER DISCOVERER' AND COOPER EVOLUTION' TIRES' ONLY THEY ARE THE 16 INCH TIRES. WE BELIEVE THAT THE 16 INCH TIRES SHOULD ALSO BE INCLUDED IN THIS RECALL. HERE ARE THE ISSUES THAT WE HAVE HAD WITH THESE TIRES: 1.10-23-18 PURCHASED DISCOVERY SRX P215/70R16 COOPER TIRES WITH ROAD HAZARD FROM BLAINS FARM & FLEET IN VERONA WI VEHICLE MILES 106292 2.10-28-18 REAR PASSENGER TIRE BLEW FOR NO REASON, CHANGED AND RETURNED TO BLAINS FOR REPLACEMENT (NO RECEIPT WAS GIVEN) 3.11-25-18 WHILE DRIVING HOME AND ROUNDING A CORNER, THE CAR VEERED INTO A CURVE AND DESTROYED THE FRONT PASSENGER TIRE, RIM, AND WHEEL COVERS ON BOTH WHEELS. REAR TIRE WAS ALSO REPLACED BECAUSE OF BULGE ON TIRE. WE PAID THE $770.46 BECAUSE WE THOUGHT IT WAS OUR FAULT BUT NOW ARE WONDERING IF THIS WAS A TIRE ISSUE TOO. MILEAGE 107274 4. 02/21/2019 FOUND BULGE IN REAR PASSENGER TIRE, REPLACED TIRE AT BLAINS FARM AND FLEET 5.2/27/19 HAD ALIGNMENT CHECKED BECAUSE TIRE FELT LIKE THEY WERE NOISY AND WONDERING IF THERE WAS ANOTHER PROBLEM WITH THE TIRES. 6.04/16/21 FOUND BULGE IN DRIVERS SIDE FRONT TIRE AND WILL HAVE REPLACED. CALLED DAVID AT COOPER TO EXPLAIN ALL OF THE ISSUES WE HAVE HAD SAY THAT THE FAILURES ALIGN WITH RECALL 19T-005 EXCEPT FOR THE SIZE OF THE TIRE. HE SAID THEY COULD NOT DO ANYTHING EXCEPT WANTED US TO TAKE THE TIRE WITH THE BULGE TO THE DEALER, WHICH WILL DO.

THE VEHICLE WAS NOT REPAIRED. THERE WERE NO INJURIES AND A POLICE REPORT WAS FILED. THE CONTACT CALLED THE MANUFACTURER AND PROVIDED CASE NUMBER: 1707072335. NO FURTHER ASSISTANCE WAS OFFERED. OSTEOPOROSIS AND I READ THAT THIS DRUG BRINGS ON OR MAKES WORSE ANY OSTEOPOROSIS CONDITION MINE IS EXTREMELY SEVER AND MINE IS SEVER. THERE WERE NO INJURIES AND THE POLICE WERE NOT CALLED. THE CONTACT STATED THAT THE FAILURE RECURRED THREE ADDITIONAL TIMES. THE VEHICLE WAS TAKEN TO THE DEALER BUT WAS NOT REPAIRED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 16,000. THE VIN WAS NOT AVAILABLE.
WHILE DRIVING ON A SUBURBAN NJ ROAD AT AROUND 30-40MPH, THE VEHICLE LOST STEERING CONTROL OUT OF THE BLUE. THE CAR VEERED INTO THE LEFT LANE, I ATTEMPTED TO RIGHT THE CAR AND SIMULTANEOUSLY HIT THE BREAKS. THE ABS RESPONSE KICKED IN, AND THE CAR VEERED DRASTICALLY TO THE RIGHT, GOING OFF THE ROAD, ACROSS THE SIDEWALK, AND FINALLY COMING TO A STOP IN THE WOODS. THE REPAIR SHOP DISCOVERED THAT THE CAUSE OF ACCIDENT WAS A BROKEN TIE ROD OF THE BACK RIGHT WHEEL, THAT HAD BEEN BROKEN FOR A WHILE BECAUSE THE SLEEVE ENCASING HAD RUSTED OVER. THE DAMAGE TO THE CAR TOTALLED $10,000, AND THE SHOP WOULD HAVE TOTALED THE CAR IF THE MILAGE HADNT BEEN SO LOW. *TR
ON WEDNESDAY, JANUARY 30TH, AROUND 6 P.M. EST, I WAS DRIVING HOME FROM WORK, GOING APPROXIMATELY 40-45 MPH, WHEN SUDDENLY I LOST CONTROL OF MY VEHICLE (2006 RAV4, STANDARD NOT SPORT). FORTUNATELY, I WAS ABLE TO GET THE CAR OFF TO THE SHOULDER AFTER ABOUT 30 SECONDS OF SWERVING AND INABILITY TO CONTROL THE CAR. THERE WAS NO PHYSICAL DAMAGE TO MY PERSON, CAR OR ANYONE ELSE, BUT AS YOU CAN PROBABLY IMAGINE, I WAS ? AND CONTINUE TO BE- VERY SHAKEN UP OVER WHAT HAPPENED, AND HOW MUCH WORSE IT COULD HAVE BEEN HAD I BEEN ON A MORE CONGESTED HIGHWAY, GOING AT A FASTER SPEED. A WOMAN, WHO WAS DRIVING BEHIND ME, STOPPED AND HELPED ME OUT AND WITNESSED HOW OUT OF CONTROL MY CAR WAS. SHE THOUGHT I HAD HIT A DEER (IT WAS VERY DARK, FOGGY AND RAINY, AS WELL). IRONICALLY, I HAD JUST RECEIVED A RECALL NOTICE FROM TOYOTA NOT EVEN A FEW DAYS EARLIER. I WAS AWAY THE PREVIOUS WEEK AND IT ARRIVED WHILE I WAS OUT OF TOWN. I HAD THE RECALL IN MY PHYSICAL POSSESSION FOR A TOTAL OF 4 DAYS, ONE OF WHICH WAS NOT A BUSINESS DAY. I HAD MY TIRES CHANGED AND ALIGNED OVER A YEAR AGO (I?D HAVE TO GET BACK TO YOU ON THE EXACT DATE, BUT I?M FAIRLY CERTAIN IT WAS DEC. 2011 OR JANUARY 2012). AT THIS TIME, ACCORDING TO THE TOYOTA RECALL NOTICE, AN ADDITIONAL STEP NEEDED TO BE TAKEN IN ORDER TO PREVENT BACKLASH AT THE THREADED PORTION OF THE ARM, FOLLOWED BY RUST. IF THIS OCCURRED, THREAD WOULD WEAR, CAUSING THE ARM TO SEPARATE, WHICH COULD RESULT IN THE LOSS OF THE VEHICLE. THE SAME NIGHT OF THE INCIDENT, I HAD MY CAR TOWED TO THE DEALERSHIP WHERE I PURCHASED IT AND THE NEXT MORNING, JANUARY 31ST, THEY CONFIRMED THAT WHAT HAD HAPPENED TO MY CAR WAS INDEED BECAUSE OF THE RECALL. THIS MEANS THAT I HAD BEEN DRIVING MY CAR AROUND TO MY OWN DETRIMENT FOR OVER A YEAR, WITHOUT ANY IDEA THAT THERE WAS A POTENTIAL SAFETY HAZARD TO MYSELF OR MY SIX YEAR OLD DAUGHTER. *TR
2006 TOYOTA RAV4 SUDDENLY ACCELERATED UNCONTROLLABLY IN A RESIDENTIAL DISTRICT; DRIVER APPLIED STANDARD BRAKE WHICH DID NOTHING TO SLOW ACCELERATION, SO THE DRIVER TRIED TO BREAK THE VEHICLE'S MOMENTUM BY SCALING A LARGE CHAIN-LINKED FENCE ON THE DRIVER'S SIDE. THE CAR WAS AIRBORN A FEW SECONDS, DRIVER PULLED EMERGENCY BRAKE, HIT A STOP SIGN, LANDED, THE VEHICLE STOPPED, AND SMOKE WAS COMING FROM THE LEFT SIDE OF THE FRONT HOOD. THE IMPACT OF LANDING BROKE THE DRIVER'S BACK, CAUSED SEVERAL BULGING LUMBAR DISCS, AND BROKE THE LEFT FOOT. TO CORRECT THE FAILURE THE OWNER OF THE VEHICLE HAS CONTACTED TOYOTA WHO IS SENDING OUT THEIR "SMART TEAM" TO DOWNLOAD INFORMATION FROM THE CAR'S BLACK BOX OR EVENT DATA RECORDER WHICH CONTAINS A HISTORY OF THE DRIVER'S ACTIONS AND THE HISTORY OF THE CAR'S ACTIONS. VEHICLE: 2006 TOYOTA RAV 4 PROBLEM: STUCK ACCELERATOR DATE OF INCIDENT: DECEMBER 24, 2009 MY WIFE WAS LEAVING WORK ON THE 24TH OF DECEMBER. OUR CAR WAS PARKED FACING UPHILL. AS SHE WAS LEAVING HER PARKING SPACE, SHE DEPRESSED THE ACCELERATOR SLIGHTLY AND THE CAR "TOOK OFF" ON ITS OWN. SHE ENDED UP HITTING A CAR ON THE OTHER SIDE OF THE ROAD, THEN GOING OVER TO THE OTHER SIDE, OVER A SIDEWALK, SNOW BANK AND THEN HITTING A BUILDING. I HAVE REPORTED THIS INCIDENT TO TOYOTA WHO WENT TO MY LOCAL DEALER TO CHECK IT OUT. THEY CLAIM THAT THEY COULD NOT FIND ANYTHING AND SENT A LETTER TO MY HOUSE STATING THAT FACT. THE WEATHER CONDITIONS THAT DAY WERE SLOPPY SNOW. AS MENTIONED, THE CAR WAS FACING UPHILL SO THE SLIPPERY CONDITIONS ACTUALLY ACTED IN HER FAVOR. I WROTE UP A FULL REPORT WITH PHOTOS THAT I SENT TO BOTH TOYOTA AND STATE FARM. I WOULD BE HAPPY TO E-MAIL THIS REPORT TO YOU AS WELL. THEN AFTER 3 DAYS AFTER THEY GAVE ME A CASE NUMBER, THEY TOLD ME THAT I HAVE TO HAVE ANOTHER ACCIDENT IN THE RAV4, TAKE IT TO A DEALERSHIP AND SEE IF THEY CAN MAKE IT HAPPEN AGAIN. I COULD BE DIED THEN!!. WHEN I HAD THE ACCIDENT, NOT ONLY DID I HAVE DAMAGE TO MY CAR, BUT I DID DAMAGE TO A BUILDING BECAUSE MY CAR WHEN OUT OF CONTROL AND BACK IN TO THE SIDE OF A BUILDING. A POLICE REPORT WAS DONE, BUT IT WAS NOT MENTION IN THE POLICE REPORT. THE DAMAGE TO THE BUILDING HAD TO BE MORE THAN A THOUSAND DOLLARS AND COMPENSATION FOR THE PEOPLE INSIDE IT. IT HAPPEN AT A GAS STATION IN BUFORD, GEORGIA. IT SEEMS LIKE TOYOTA DO NOT WANT ANYONE TO KNOW THAT THEIR GAS PEDALS HAS BEEN STICKING FOR MORE YEARS THAN THEY OWN UP TO. I AM LEASING MY RV4 AND HAVE ONE MORE YEAR LEFT IN MY LEASE. PLEASE PRAY THAT I CAN CONTROL MY GAS PEDAL WHILE I AM TRYING TO COMPLETE THIS LEASE WITHOUT GETTING KILLED. *TR
