TL* THE CONTACT OWNED A 2004 TOYOTA SEQUOIA. WHILE DRIVING AT APPROXIMATELY 40 MPH OVER A MANHOLE, THERE WAS A LOUD POPPING SOUND AND THE FRONT PASSENGER SIDE WHEEL FOLDED UNDER THE VEHICLE. AS A RESULT, THE CONTACT CRASHED INTO A DITCH ON THE SIDE OF THE ROAD. THE FRONT PASSENGER SUSTAINED INJURIES TO THE RIGHT KNEE AND REQUIRED MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE VEHICLE WAS DESTROYED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND STATED THAT THE VIN WAS INCLUDED IN NHTSA CAMPAIGN NUMBER: 07V013000 (SUSPENSION). THE FAILURE MILEAGE WAS APPROXIMATELY 175,000.

THEY HAD SWITCHED ME INTO A JEEP WHICH WAS EMMITING SOME TYPE OF CHEMICAL THAT MAKES YOU CHOKE AND BLEED IF YOU'RE PREGNANT (I'M PREGNANT). SO I SWITCHED BACK TO THE VAN THAT I HAD BEFORE BUT THIS VEHICLE STARTED DOING THE SAME. MY VEHICLE HAS NOW SINCE BEEN WRECKED (NOV. 2, 2012) AND SITS AT LANCE TOWING COMPANY IN LAWRENCEVILLE, GA. HOWEVER, THIS IS NOT AN ISOLATED INCIDENT AS I HAVE NOTICED THAT WHEN TRANSFER TRUCKS PASS BY MY THEY BLOW THEIR PIPES WHICH GIVE OFF THE SAME EFFECT BUT STRONGER IF YOU'RE TRYING TO OPEN THE WINDOWS TO GET FRESH AIR. I HAVE NOW HAD TO RESORT TO WEARING A MASK TO PROTECT MY HEALTH AND THE HEALTH OF MY UNBORN CHILD. WAS DRIVING MY 2004 TOYOTA SEQUOIA AT 35-40MPHIN HEAVY TRAFFIC WHEN THERE WAS A LOUD BANG, A DROP IN THE FRONT LEFT OF THE TRUCK AND THE SOUND OF METAL SCRAPPING THE PAVEMENT. I TURNED THE WHEEL TO THE RIGHT AND HIT THE BRAKES TO AVOID GOING OVER THE MEDIAN INTO ON COMING TRAFFIC. I THEN SKIDDED ACROSS 2 LANES OF TRAFFIC AND HIT A CAR TRYING TO ENTER THE PARKWAY. AN EYEWITNESS SAID HE SAW THE WHEEL COME DISLODGED FOR NO REASON. LUCKILY THE LADY WHO WAS IN THE OTHER CAR ONLY HAD MINOR INJURIES AND OTHER THAN A SO FAR MINOR CASE OF WHIP LASH EVERYONE IS OK. UPON INSPECTING THE VEHICLE WITH THE POLICE OFFICER IT LOOKED AS IF THE WHEEL AND AXLE WERE RIPPED FRON THE TRUCK. UNFORTUNATELY THE SEQUOIA GAVE ME NO INDICATIONS OF ANYTHING GOING WRONG OR I WOULD HAVE TAKEN IT TO THE SHOP. *TR

BOTH THE DRIVER AND PASSENGER WERE KNOCKED UNCONSCIOUS AT THE TIME. THE PASSENGER AND DRIVER AWOKE AND THE CRASH WAS STILL OCCURRING. THE VEHICLE CONTINUED TO PICK UP SPEEDS; HOWEVER THE CONTACT DID NOT NOTICE THE EXACT SPEEDS. THE VEHICLE WAS ON THREE WHEELS AND CONTINUED TO PROCEED. THE PASSENGER WAS KNOCKED UNCONSCIOUS A SECOND TIME. THE PASSENGER AWOKE TO SOMEONE FLASHING LIGHT INTO THE VEHICLE. THE CONTACT SUSTAINED A BROKEN TIBIA, SEVEN BROKEN RIBS, FRACTURED STERNUM AND A FRACTURED VERTEBRAE. THE DRIVER SUSTAINED A CONCUSSION, BRUISED FACE AND A SMALL LEFT HAND FRACTURE. BOTH WERE TAKEN TO A NEARBY HOSPITAL. A POLICE REPORT WAS FILED AT THE SCENE OF THE CRASH. THE VEHICLE WAS DESTROYED. THE FAILURE AND CURRENT MILEAGES WERE 84,199.

THE VEHICLE VEERED TOWARD TRAFFIC IN THE OPPOSITE DIRECTION SO I FOUGHT TO HOLD THE STIRRING FROM TURNING IN TO ONCOMING TRAFFIC OR ROLLING OFF THE ROAD DOWN THE SIDE OF THE FREEWAY. MY CHILDREN SCREAM AND CRIED HYSTERICALLY AS THEY WATCHED AND EXPERIENCE OUR VEHICLE VIOLENTLY THRASHED ABOUT. SMOKED AND SPARKS BELLOWED UP ALONG THE PASSENGER AND FRONT SIDES OF THE VEHICLE. I GOT THE VEHICLE TO THE SIDE OFF THE ROAD, OUT OF TRAFFIC INCHES FROM A 20' (TWENTY-FOOT) DROP OFF. THE VEHICLE CAME TO REST ON THE WHEEL WHICH HAD SEPARATED FROM THE VEHICLE CAUSING THE DRIVER (ME) TO LOOSE STEERING CONTROL THEREFORE ENDING IN THE MOST TERRIFYING EXPERIENCE FOR MY CHILDREN AND MYSELF AS WELL AS A NEAR TRAFFIC COLLISION. VEHICLE WAS TOWED TO TOYOTA OF EL CAJON. THEY CALLED AND SAID THE VEHICLE HAS APPROX $3900 IN DAMAGES. HE SAID THE ARM BROKE AND THE WHEEL SEPARATED FROM THE VEHICLE. HE ASSURED ME HE HAD EVEN LOOK FOR RECALLS ON THE VEHICLE AND THERE WERE NONE. HE EXPLAINED THE VEHICLE HAD 109,000 (ONE-HUNDRED-THOUSAND) MILES AND THIS WAS JUST ONE OF THOSE THINGS. I TOLD HIM I NOT AGREE BECAUSE I ONCE OWNED A RAV 4 WITH 364,000 (THREE-HUNDRED-SIXTY-FOUR-THOUSAND) MILES AND NEVER EXPERIENCED SUCH. THE SEQUOIA WAS PURCHASED NEW MAY OF 2004, IRREGARDLESS OF THE NUMBER OF MILES THIS SHOULD NOT HAVE OCCURRED. THEIR WERE 6 PASSENGERS IN OUR SEQUOIA THAT DAY AND EVERYONE WAS WEARING THEIR SEAT BELT. MY HUSBAND THE DRIVER SUSTAINED THE FOLLOWING INJURIES: HEAD, AND LEFT ARM CONTUSIONS AND ABRASIONS AND COMPLAINED OF HEADACHE. HE ALSO SUFFERED NECK AND BACK PAIN. MYSELF: I SUFFERED A CONCUSSION FROM HITTING THE RIGHT FRONTAL AND TOP PORTION OF MY HEAD, SEVERE NECK AND BACK PAIN & HEADACHE, LEFT WRIST & HAND CONTUSION WITH TENDON & LIGAMENT DAMAGE. BILATERAL KNEE PAIN WITH LIGAMENT DAMAGE. MULTIPLE ABRASIONS AND UNSTABLE VITAL SIGNS. MY MOTHER-IN-LAW WHO WAS SITTING BEHIND MY HUSBAND IN THE 2ND ROW SEATING. SHE SUSTAINED MULTIPLE CONTUSIONS TO HIPS AND SHOULDERS AND SUSTAINED 2 PELVIC FRACTURES. COMPLAINED OF HEAD, NECK, AND BACK PAIN. MY FATHER-IN-LAW WHO WAS SITTING DIRECTLY BEHIND PASSENGER WHO SUSTAINED 3 NECK FRACTURES ONE SECTION OF HIS NECK VERTEBRAE WAS CRUSHED. HE ALSO HAD A SEVERE LACERATION/CRUSH INJURY TO HIS RIGHT THIRD DIGIT. HE HAD MULTIPLE CONTUSIONS AND ABRASIONS. COMPLAINED OF HEAD, NECK, AND BACK PAIN. OUR YOUNGEST DAUGHTER WAS IN THE 3RD ROW SEATING SITTING IN THE MIDDLE. SHE SUSTAINED MULTIPLE CONTUSIONS AND ABRASIONS AND COMPLAINED OF HEADACHE, BACK & NECK PAIN ALONG WITH ABDOMINAL PAIN. I DO THINK THE SEQUOIA HAS THE SAME PROBLEM AS THE SIENNA BECAUSE TWICE THIS HAS HAPPENED TO ME ONCE IN MY DRIVEWAY AND THE SECOND TIME WAS AT A GAS STATION. I HAD TO JUMP IN THE VEHICLE BECAUSE IT STARTED TO ROLL BACK ALMOST 2 MINS AFTER I GOT OUT OF THE VEHICLE AND CAUSED SOME DAMAGE TO MY DOOR WHILE I WAS TRYING TO JUMP BACK INTO THE VEHICLE BEFORE IT CAUSED MAJOR DAMAGE. ABOUT 5 MINUTES LATER, WE HEARD A REPEATED CLICKING SOUND AND THE BOAT AND TRAILER CAME ROLLING BACK DOWN THE LAUNCH RAMP INTO THE WATER NARROWLY MISSING MY SON AND I. THE ONLY THING THAT STOPPED THE TRUCK AND TRAILER WAS THE FACT THAT THE BOAT TRAILER JACKKNIFED AGAINST THE LEFT REAR BUMPER OF THE TRUCK. THE REGIONAL INSPECTOR, JIM TEELY FINALLY INSPECTED THE SEQUOIA ON JULY 15TH. ACCORDING TO JIM TEELY, WHOM I HAVE SPOKEN WITH ON THE TELEPHONE, THE INSPECTION FOUND THAT THERE WAS NOTHING MECHANICALLY WRONG WITH THE VEHICLE. THE INSPECTION THAT WAS DONE WAS CURSORY AT BEST. UNTIL THE TRANSMISSION HAS BEEN PHYSICALLY EXAMINED BY TOYOTA I AM NOT COMFORTABLE TOWING MY BOAT AND ESPECIALLY NOT PARKING THE SEQUOIA (TOWING OR NOT) ON AN INCLINE. THEY HAVE NOT OFFERED A SATISFYING STATEMENT ON WHY THE CAR WAS NOT RECALLED, AND WHY THE WEAR WAS NOT IDENTIFIED IN THE LAST SEVERAL SERVICES I HAVE HAD PERFORMED AT THE DEALERSHIP. THIS ACCIDENT COULD HAVE COST ME, MY WIFE AND CHILDREN, AND POSSIBLY ANOTHER VEHICLE'S OCCUPANTS THEIR LIVES. I AM THANKFUL THAT THE CIRCUMSTANCES OF THE SEPARATION WERE ON A TAME NEIGHBORHOODS ROAD, BUT I AM FEARFUL THAT SEVERAL THOUSAND 2004 OR LATER SEQOUIAS MAY BE AFFECTED AND UNIDENTIFIED. WE HAVE HAD 2 REAR END COLLISIONS IN OUR 2004 TOYOTA SEQUOIA IN LESS THAN 10 MONTHS OF OWNERSHIP. WE HAVE NEVER HAD A REAR END COLLISION IN 12 YEARS OF DRIVING OTHER MODEL SUVS, AND 20+ YEARS OF DRIVING ALL TYPES OF VEHICLES. I BELIEVE THERE IS A SAFETY DESIGN FLAW IN THE REAR BRAKE LIGHTS OF THE SEQUOIA. THE LARGE RED LIGHTS ON THE RIGHT AND LEFT SIDE OF THE TAILGATE DO NOT FUNCTION AS BRAKE LIGHTS. THEY ONLY LIGHT UP WHEN THE HEADLAMPS ARE ILLUMINATED. THEREFORE, THE LARGEST RED LIGHT SURFACE AREA ON THE BACK OF THE CAR DOES NOT ILLUMINATE WHEN BRAKES ARE APPLIED. I BELIEVE THIS LEADS DRIVERS TO MISS THE FACT THAT THE BRAKES HAVE BEEN APPLIED. THE ONLY DRIVER EYE-LEVEL BRAKE LIGHTS ARE ON THE SIDE PANELS AND THE HIGH-MOUNT LIGHT OVER THE TAILGATE, WHICH IS FAIRLY SMALL. THE PROPORTIONAL SIZE OF THE SIDE EYE-LEVEL BRAKE LIGHTS IS SIGNIFICANTLY SMALLER THAN THE RED LENSES ON THE TAILGATE. WE REQUEST THAT NHTSA LOOK INTO THIS MATTER, BECAUSE THE DESIGN DEFIES COMMON SENSE AND WE BELIEVE HAS LED TO TWO REAR-END COLLISIONS IN LESS THAN A YEAR. WE ARE SERIOUSLY CONSIDERING SELLING THIS VEHICLE BECAUSE I HAVE LOST CONFIDENCE THAT IT IS SAFE FOR MY FAMILY.

NEW 2004 TOYOTA SEQUOIA LEASED ON 8/8/04 WITH 78 ODOMETER MILES, SUSTAINED DAMAGES TO THE FRONT PASSENGER-SIDE STEERING ARM AT ABOUT 2,379 MILES AT 7:30 A.M. ON 11/5/04 IN PLANTATION, FLORIDA, AND IMMEDIATE/COMPLETE SEPARATION OF PASSENGER-SIDE FRONT WHEEL. AT THE TIME OF THE INCIDENT, THE ASPHALT-PAVED ROADWAY WAS FREE OF ANY ABNORMAL CONDITIONS, AND THE WEATHER WAS CLEAR. THE VEHICLE HAD BEEN IN MOTION FOR ONLY A FEW FEET AFTER A FULL STOP, SECONDS BEFORE THE BREAKDOWN. EXCEPT FOR BODY DAMAGES RESULTING FROM THE 5,000-LB VEHICLE FALLING ON THE ROAD AFTER THE WHEEL SEPARATION, THERE IS NO DAMAGE THAT WOULD SUGGEST THAT THE BREAKDOWN WAS THE RESULT OF A COLLISION. BATTERY IN 2004 TOYOTA SEQUOIA WENT DEAD. PUT THE BATTERY ON CHARGE. AFTER CHARGING WAS COMPLETE I PUT THE KEY IN THE IGNITION TO START THE CAR. I STARTED THE CAR AND SMOKE POURED OUT FROM UNDER THE HOOD. I IMMEDIATELY TURNED THE IGNITION OFF AND OPENED THE HOOD TO FIND THE ABS COMPUTER SMOKING WITH FLAMES. I QUICKLY PUT OUT THE FLAME WITH A THICK BLANKET AND NOTICED SMOKE SHOOTING THROUGH A HOLE BURNED IN THE COMPUTER. LAST WEEK, (3/17/16) MY NEIGHBOR WAS TAKING HER THREE CHILDREN TO SCHOOL IN HER 2004 TOYOTA SEQUOIA. SHE WAS TRAVELING ON A NEW ROAD AT APPROXIMATELY 30 MPH WHEN THE FRONT END OF THE VEHICLE DROPPED ON DRIVER'S SIDE. SHE THOUGHT SHE HAD A FLAT TIRE SO SHE STEERED TO THE SIDE OF THE ROAD. IT APPEARS THE DRIVER'S SIDE, LOWER BALL JOINT FAILED (SEE PHOTOS). I CALLED HENDRICK TOYOTA OF APEX, NC AND TALKED WITH MR. JOHN SPECHT (919-335-1678). HE CHECKED THE VIN AND SAID THE LOWER BALL JOINTS HAD A RECALL AND WERE CHANGED ON AUG. 15, 2007. HE ALSO SUGGESTED I CONTACT TOYOTA TO REQUEST THEIR ASSISTANCE TO COVER THE COST TO REPAIR THE VEHICLE (CASE NO.[XXX]). HE ADVISED THE VEHICLE BE BROUGHT IN FOR THE AIRBAG RECALL TOO. VEHICLE WAS TOWED HOME ON A DOLLY TO PREVENT ANY DAMAGE TO THE TRANSMISSION. THE VEHICLE HAS 215,444 MILES AND WAS NOT USED FOR OFF-ROAD DRIVING. THE VEHICLE RECENTLY PASSED STATE INSPECTION WITH NO PROBLEMS NOTED ABOUT THE FRONT END. IF THE LOWER BALL JOINTS WERE ALREADY REPLACED IN A RECALL (2007) WHAT DID THEY REPLACE THEM WITH, AND HOW DID THEY CORRECT THE PROBLEM? THIS FAILURE COULD HAVE CAUSED A DEADLY ACCIDENT AT HIGHER SPEED.
