TL*THE DRIVER OWNED A 2010 TOYOTA PRIUS. THE CONTACT STATED THE DRIVER CRASHED INTO ANOTHER VEHICLE, AT 35 MPH. AS A RESULT OF THE ACCIDENT, THE VEHICLE WAS DESTROYED AND THE REAR PASSENGER WAS KILLED DUE TO A SEVERE NECK INJURY. THE CONTACT BELIEVED THE HEAD RESTRAINT WAS NOT POSITIONED PROPERLY. THE CONTACT CALLED THE MANUFACTURER, BUT RECEIVED NO RESPONSE REGARDING THE HEAD RESTRAINT. THE FAILURE MILEAGE WAS UNKNOWN. UPDATED 03/15/11*LJ
TL* THE CONTACT'S BROTHER OWNED A 2010 TOYOTA PRIUS. THE CONTACT STATED HER BROTHER DIED IN A HEAD ON CRASH. WHILE HE WAS DRIVING AT APPROXIMATELY 65MPH NORTH BOUND, A TRUCK WAITING AT A TRAFFIC LIGHT WAS REAR ENDED BY ANOTHER TRUCK PUSHING THE TRUCK INTO THE CONTACT'S BROTHER LANE OF TRAFFIC. THE CONTACT STATED WITNESSES ADVISED HER THAT BRAKES FAILED ON HER BROTHER'S VEHICLE AND HE WAS NOT ABLE TO BRING THE VEHICLE TO STOP WHEN HE SAW THE TRUCK APPROACHING. THE POLICE REPORT WAS NOT AVAILABLE. THE VIN WAS UNKNOWN. THE FAILURE AND THE CURRENT MILEAGE WAS 21,000.
TL*THE CONTACT OWNS A 2010 TOYOTA PRIUS. THE CONTACT WAS DRIVING APPROXIMATELY 20 MPH WITH THE BRAKE ENGAGED WHEN THE VEHICLE ACCELERATED. THE CONTACT CRASHED INTO THE REAR END OF A NISSAN. THE WEATHER WAS NOT A FACTOR. THE CONTACT OBSERVED SMOKE COMING FROM THE VEHICLE. THE REAR SUSPENSION WAS DAMAGED AFTER THE CRASH OCCURRED. THE INSURANCE COMPANY AUTHORIZED THE VEHICLE TO BE TOWED TO AN IMPOUND LOT. THE VIN WAS NOT AVAILABLE. THE APPROXIMATE FAILURE AND CURRENT MILEAGES WERE 36,000.
I was coming to an intersection where two cars were waiting for a right turn. I pressed on the brakes and nothing happened. The car continued to move without slowing down. The brakes felt "blocked" and unresponsive. I engaged the manual break at the last moment and that slowed me down. But I still hit the car in front of me which in turn hit another car in front of it. The police report says "following too close". But I was not following other cars. The two cars were already at the intersection. I was just coming to this intersection and attempted to slow down as usual. Except that the brakes did not respond. My car was a total loss.
ABS system displayed airbag symbol displayed brake symbol displayed seat belts light flashing airbag system showing off. Toyota dealership is aware of the problem with the ABS system along with the items mentioned. accident could have been avoided contact if the ABS system was working. Airbags did not deploy. Advise Toyota corporation on 27 February 2023 file number 230-227-00 2210 in regards to the ABS they were also advised of the airbag symbol displayed in the seat belt fasten flashing on driver side when an individual is seated in the car and buckled in. Vehicle has not been inspected yet however on Monday 6th of March it will be inspected by copart, by the direction of the USAA insurance company.

THE CONTACT STATED THAT WHILE HER HUSBAND WAS DRIVING AT 40 MPH, THE VEHICLE IN FRONT SUDDENLY STOPPED, THE BRAKE PEDAL WAS DEPRESSED HOWEVER IT FAILED TO ENGAGE CAUSING HIM TO REAR END THE OTHER VEHICLE. THE CONTACT MENTIONED THAT HER HUSBAND WAS ALREADY DEPRESSING THE BRAKE PEDAL WHEN THEY COMPLETELY FAILED. THE AIR BAGS DID NOT DEPLOY. THE CONTACT STATED THAT THE FRONT SEAT PASSENGER IN THE OTHER VEHICLE WAS TRANSPORTED TO THE HOSPITAL BY AMBULANCE DUE TO FEELING NAUSEOUS. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO AUTO BODY REPAIR FACILITY WHERE IT WAS AWAITING INSPECTION BY THE INSURANCE COMPANY. THE CONTACT STATED THAT THE VEHICLE WAS A POSSIBLE TOTAL LOSS AAS THE RADIATOR AND ENGINE WAS DAMAGED. THE VEHICLE HAD BEEN SERVICED AT CLEARWATER TOYOTA (21799 US HWY 19 N, CLEARWATER, FL 33765 800-667-905) FOR A BRAKE ISSUE. THE DEALER REFUSED TO REPAIR THE BRAKES UNDER AN EXTENDED WARRANTY DUE TO AN INCORRECT CONSISTENT FAILURE CODE. THE MANUFACTURER STATED THAT THE SENSOR WAS NOT ALERTING ENOUGH FOR THE REPAIRS TO BE COVERED. THE FAILURE MILEAGE WAS 200,000.
WHEN I WAS DRIVING ON WEST PACES ROAD IN ATLANTA, I TRIED TO BRAKE AND THE BARAKES DID NOT WORK . THEREFORE I WENT DIRECTLY INTO THE BACK OF THE CAR IN FRONT OF ME. I MUST PRECISE THAT THE BRAKES PAD WERE NEW, THE BRAKE LIQUID WAS OK. I WAS DRIVING AT 30 MILES AN HOUR.

THE BRAKE WAS APPLIED BUT FAILED TO ENGAGE AS THE VEHICLE COLLIDED WITH A NEIGHBOR'S FENCE AND TRAILER. THE BRAKE WARNING WAS LIGHT ILLUMINATED DURING THE FAILURE. THE CONTACT STATED THAT THE PASSENGER SUSTAINED BRUISING TO THE KNEE AS WELL AS HE ALSO SUSTAINED BRUISING TO THE KNEE THAT REQUIRED NO MEDICAL ATTENTION. A POLICE REPORT WAS NOT FILED. THE VEHICLE WAS TAKEN TO LAKELAND TOYOTA (2925 MALL HILL DR, LAKELAND, FL 33810) WHERE THE BRAKES WERE REPAIRED. THE CONTACT STATED THAT THE MANUFACTURER WAS NOTIFIED OF THE FAILURE AND THE VEHICLE WAS INSPECTED BY A TOYOTA ENGINEER ASSISTANCE FIELD INSPECTOR. THE FAILURE MILEAGE WAS APPROXIMATELY 197,356.
AIR BAG DID NOT DEPLOYED AT THE TIME OF ACCIDENT. DAMAGED/RIP APART FRONT BUMPER...CAR IS TOTAL LOSS AS PER INSURANCE ADJUSTER. OTHER COLLIDED HONDA CR-V HAD ALL ITS AIR BAGS DEPLOYED. NONE ON MY PRIUS. ACCIDENT HAPPENED AT MAJOR INTERSECTION ,WHEN GREEN LIGHT TURNED ON I PADDLE MY CAR , OTHER CAR HONDA CR-V CAME AT A SPEED TO MAKE LEFT TURNED AND HIT MY FRONT BUMPER.
AIR BAGS FAILED TO DEPLOY IN CAR CRASH: MY 2010 PRIUS WAS STUCK BY ANOTHER CAR WHO RAN A RED LIGHT, IN THE FRONT RIGHT (PASSENGER SIDE). THE CAR THAT STRUCK ME WAS TRAVELING APPROX 35MPH AND PUSHED MY CAR (ALSO TRAVELING APPROX. 35MPH) INTO THE PATH OF A WOODEN TELEPHONE POLE THAT SUSPENDED THE TRAFFIC LIGHTS OVER THE INTERSECTION. MY CAR TRAVELED ABOUT 20-30 FEET FURTHER AND STOPPED WHEN MY CAR HIT THE POLE HEAD ON. THE TELEPHONE POLE BROKE IN TWO IN THE IMPACT. NONE OF MY AIR-BAGS DEPLOYED. THE POLICE REPORT IS INCORRECT REGARDING THE AIRBAGS. I HAVE CONTACTED THE POLICE DEPARTMENT TO AMEND THE REPORT. AS SEEN IN THE ACCOMPANYING PHOTOGRAPHS, THE AIR BAGS DID NOT DEPLOY, CONTRARY TO THE POLICE REPORT.
TL* THE CONTACT OWNED A 2010 TOYOTA PRIUS. THE CONTACT STATED THAT WHILE DRIVING AT 25 MPH, ANOTHER VEHICLE CRASHED INTO THE FRONT PASSENGER'S SIDE. THE FRONT DRIVER'S SIDE AIR BAGS DEPLOYED AS A RESULT THE CONTACT SUSTAINED A LEG INJURY. THE CONTACT WAS HOSPITALIZED AND THE DRIVER FOR THE OTHER VEHICLE HAD UNKNOWN INJURIES. A POLICE REPORT WAS FILED AND THE VEHICLE WAS TOWED TO A TOW YARD. THE VEHICLE WAS TOTALED BY THE CONTACT'S INSURANCE COMPANY. THE CONTACT STATED THAT SHE JUST RECEIVED A RECALL LETTER NUMBER: 16V487000 (AIR BAGS) HOWEVER, THE ACCIDENT OCCURRED BACK IN APRIL OF 2016. THE DEALER AND MANUFACTURER WERE NOT CONTACTED. THE FAILURE MILEAGE WAS 40,000.
I WAS ON THE FREEWAY IN TRAFFIC, GOING 60 MPH. THERE WAS ROAD CONSTRUCTION COMING UP AHEAD. I DEPRESSED THE BRAKE PEDAL AND NOTHING HAPPENED. THE CAR DIDN'T SLOW DOWN AT ALL AND I CRASHED INTO ANOTHER VEHICLE. MY CAR WAS MANUFACTURED WHEN THE 'VOLUNTARY SAFETY RECALL' BY TOYOTA WAS SUPPOSED TO ADDRESS THIS, BUT I RECEIVED NO NOTIFICATION, NOR DID THOUSAND OAKS TOYOTA, WHERE I HAD THE CAR SERVICED, COMMUNICATE ABOUT IT. *JS
CAR IN TWO ACCIDENTS OVER TWO YEARS BODY WAS NOT PROPERLY WELDED BACK TO THE FRAME! BRAKES WERE NOT PROPERLY REALIGNED NEITHER WAS THE STEERING COLUMN TRANSMISSION BANDS WERE NOT OPERATING AND WOULD NOT SHIFT AND NUMEROUS OTHER PARTS FOR LEAKAGE FROM BRAKE LINES TO TRANSMISSION LINES ALL LINES WERE BROKEN AND SO ARE THE SEALS ON THEM!
THE BRAKES SEEM TO DISENGAGE FOR A SECOND AT RANDOM TIMES WHILE PRESSING ON THEM IN A NORMAL MANOR. I DRIVE EVERY DAY ABUT 20 MILES AND THIS SEEMS TO HAPPEN ABOUT ONCE A MONTH. I DIDN'T THINK MUCH OF IT UNTIL A FEW DAYS AGO WHEN I HAD TO SLAM ON MY BREAKS TO TRY AND AVOID AN ACCIDENT. DURING THAT INCIDENT THEY DISENGAGED FOR A SECOND AGAIN. THIS WAS A SECOND TOO LONG AND I WAS UNABLE TO AVOID A COLLISION. I LOOKED THIS UP AFTER MY ACCIDENT AND I SURPRISINGLY SAW SIMILAR REPORTS ON GOOGLE FOR MY CAR. BUT IT LOOKS LIKE TOYOTA IS SAYING THAT IT DOESN'T AFFECT MY VIN NUMBER? I'M HERE TO SAY THAT IT DOES.
*TAKATA RECALL I WAS INVOLVED IN A MAJOR CAR ACCIDENT DURING LUNCH HOUR ON THE 405 SOUTH FWY IN LOS ANGELES, CA. I WAS STRUCK BY A SEMI TRUCK WHO WAS MERGING INTO MY LANE AND WHILE I TRIED TO GET OUT OF HIS WAY BY SPEEDING UP. I WAS CLIPPED BY THE TRUCK AND MY CAR CAREENED INFRONT OF THE TRUCK AND WAS PUSHED BEFORE HE APPLIED HIS BRAKES. MY CAR THEN SPUN OUT OF CONTROL ACROSS ALL FWY LANES. I WAS HIT BY TWO OTHER ON COMING CARS BEFORE COMING TO A STOP IN THE CARPOOL LANE. ALL CARS INCLUDING THE TRUCK WERE DRIVING APPROXIMATELY 65-70 MPH BASED ON THE POLICE REPORT. MY FOOT WAS ON THE GAS THE ENTIRE TIME AS I SPUN OUT OF CONTROL. MY COMPLAINT IS REGARDING THE AIRBAGS. NONE OF THE SIDE OR FRONT AIRBAGS DEPLOYED AT THE TIME OF ALL IMPACTS. I HAD THE VEHICLE INSPECTED BY AN INDEPENDANT INSPECTION COMPANY AFTER CONTACTING TOYOTA. I HAVE YET TO HEAR BACK FROM TOYOTA EVEN THOUGH THEY HAVE HAD THE REPORT FOR ALMOST TWO MONTHS. I FINALLY RECEIVED THE REPORT FROM THE INSPECTOR AFTER THREATNING TO HAVE MY ATTORNEY CONTACT THEM. I DON'T KNOW HOW TO READ THE FINDINGS OF THE REPORT SINCE ITS ALL TECHNICAL ENGINEERING. I AM ATTACHING THE REPORT AND HOPE THAT SOMEONE IN YOUR DEPARTMENT CAN REVIEW IT AND FOLLOWUP. THANK YOU

THE CONTACT'S CHEST AND FACE STRUCK THE STEERING WHEEL. THE CONTACT RECEIVED INJURIES TO THE CLAVICLE AREA, SHOULDERS, FACE, EYES, HEAD/BRAIN, AND NECK AREA. EXTENSIVE MEDICAL ATTENTION WAS NEEDED. THE VEHICLE WAS NOT TOWED; HOWEVER, A POLICE REPORT WAS FILED THE VEHICLE WAS TAKEN TO THE DEALER WHERE IT WAS DIAGNOSED THAT THE AIR BAG SENSOR WAS MISSED BY A COUPLE OF MILLIMETERS, WHICH PREVENTED THEM FROM DEPLOYING. THE VEHICLE WAS DEEMED TOTALED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE. UPDATED 07/13/17*LJ *JS
DRIVING ON A EXTREMELY ROUGH STREET, MY CAR JERKED TO THE LEFT. I TRIED BRAKING, BUT UNMOVABLE OBJECTS (STREET SIGN AND PHONE JUNCTION BOX) PROVIDED THE STOP. AIRBAGS DID NOT DEPLOY, BUT FORCE OF CAR PUSHED ME HARD INTO THE STEERING WHEEL. I HAD MY SAFETY BELT ON, PLUS BACK BRACE. EMTS NOTED 60% O2, IN ICU ON RESPIRATOR FOR A WEEK, HOSPITAL FOR 2 WKS. DISTINCTLY, I REMEMBER THE STEERING WHEEL JERKED TO THE LEFT.

AS A RESULT, THE CONTACT VEERED OFF THE ROAD AND ROLLED OVER SEVERAL TIMES. THE AIR BAGS FAILED TO DEPLOY. THE CONTACT SUSTAINED A CONCUSSION, FRACTURED RIBS, AND SEVERAL LACERATIONS THAT REQUIRED MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO A SALVAGE YARD WHERE IT WAS DEEMED DESTROYED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE. THE FAILURE MILEAGE WAS 48,000.
ON 05/25/2016 AT ABOUT 9AM, WHILE IN MOTION AT A RATE OF SPEED APPROX 25 MPH ON A STREET DURING CONSTRUCTION (DON'T KNOW IF THE BUMPY ROAD HAD ANYTHING TO DO WITH IT), THE CHANGE OIL SOON LIGHT APPEARED AND SECONDS LATER ALL THE LIGHTS ON THE DASH CAME ON WITH THE MESSAGE TO CHECK THE "CHECK HYBRID SYSTEM" . I TRIED TO STOP THE VEHICLE AND IT KEPT ACCELERATING. I PRESSED ON THE BREAKS REALLY HARD AND THE TRACTION CONTROL LIGHT KEPT FLASHING ON AND OFF LIKE CRAZY BUT IT BARELY HAD ANY IMPACT. ALTHOUGH THE VEHICLE SLOWED, IT DID NOT COME TO A FULL STOP. I ENDED UP HITTING THE VEHICLE IN FRONT OF ME. THANK GOD IT WAS A TRUCK AND AS ABLE TO STOP THE CAR FROM MOVING FURTHER OR I COULD OF ENDED UP IN THE MIDDLE OF AN INTERSECTION. AFTER THE COLLISION, THE CAR WOULD NOT STOP ALTHOUGH THE PARK BUTTON WAS PRESSED AND LIGHT WAS ON. I EVEN TRIED TURNING OFF THE VEHICLE AND THE VEHICLE STAYED ON AND IN DRIVE. I ENDED UP IN THE HOSPITAL AND DON'T KNOW HOW IT EVENTUALLY GOT TURNED OFF BUT THAT WAS VERY FRIGHTENING. I HAD MY CHILDREN IN THE CAR. ON IMPACT, THE SEAT BELTS DID NOT EVEN TIGHTEN.
TL* THE CONTACT OWNED A 2010 TOYOTA PRIUS. THE CONTACT WAS DRIVING APPROXIMATELY 18 MPH WHEN ANOTHER VEHICLE DRIVING IN THE OPPOSITE DIRECTION CRASHING INTO THE DRIVER'S SIDE OF THE VEHICLE. NONE OF THE AIR BAGS DEPLOYED. THE CONTACT HAD TO BE PRIED OUT OF THE VEHICLE AND TRANSPORTED BY AMBULANCE TO THE HOSPITAL. THE CONTACT SUSTAINED THREE FRACTURED BONES IN THE RIGHT FOOT, 3 TOES, A SEVERE STRAIN IN THE RIGHT ANKLE, AND A CONCUSSION. THE VEHICLE WAS DESTROYED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE.
