DRIVING ON 81 N (MILE MARKER 160) IN PA AT 7:10 AM WHEN FRONT PASSENGER TIRE BLOWS OUT. THIS CAUSED ME TO SWERVE ACROSS BOTH LANES COME BACK HIT THE GUARD RAIL. TRUCK FLIPS OVER ONTO GUARD RAIL, SLIDES FOR A WAYS ON GUARD RAIL, THEN ROLLS OVER AS IT DESCENDS A HILL. THE BLOWOUT WAS IN MY LANE (GAUGE IN PAVEMENT) WITHOUT ANY OBJECT IN THE ROAD. TIRE LOST PRESSURE INSTANTANEOUSLY. BOTH TOW DRIVER & INSURANCE ADJUSTER POINTED OUT TIRE AS CAUSE OF ACCIDENT. THE TIRE WAS A MICHELIN LTX M/S2 WITH 33000 MILES ON IT, BUT IT HAD A 70000 MILE WARRANTY. SHAPE & DIRECTION OF TEAR IN TIRE IS IN THE OPPOSITE DIRECTION OF THE TIRE ROTATION, SO I TAKE THIS AS ADDITIONAL EVIDENCE THAT I DIDN'T RUN OVER AN OBJECT IN THE ROAD. *TR
MORNING DRIVE FROM FREDERICKSBURG, VA TO SYRACUSE NY ALONG ROUTE 81N. ROADS ARE EMPTY, DRY, & SUN IS UP. I HAD JUST DECIDED TO STOP AT NEXT EXIT FOR BREAKFAST WHEN MY 2005 TUNDRA PICKUP IS VIOLENTLY JERK TO THE RIGHT. I TRY TO FIGHT IT BACK STRAIGHT WHEN IT SUDDENLY TAKES OF TO THE LEFT, THEN JERKS BACK TO THE RIGHT. I HIT THE GUARD RAIL & THE TRUCK FLIPS OVER. THE NEXT THING I REMEMBER I'M HANGING UPSIDE DOWN & I'M HEARING A STRANGE NOISE. I TURN THE KEYS OFF & THE NOISE STOPS. I THINK I SMELL GAS AND FIGHT MY WAY OUT OF THE SEAT BELT & CRAWL OUT WHAT IS LEFT OF THE PASSENGER SIDE WINDOW SINCE THE DRIVERS SIDE IS BLOCKED. WHEN I GET OUT THERE IS SOMEONE YELLING FROM ROAD. I TELL HIM I'M OK & A TROOPER STOPS. I START COLLECTING SOME SILVER COINS THAT ARE SCATTERED WHEN I HEAR THE TROOPER YELLING AT ME. SOUNDING MAD. HE TELLS ME TO STOP & GO TO GUARD RAIL. THE AIR BAGS FAILED TO DEPLOY AND THE SEAT BELT FAILED TO PROPERLY RESTRAIN THE CONTACT. THE CONTACT WAS TRANSPORTED TO THE HOSPITAL WHERE HE RECEIVED TREATMENT FOR SEVERAL INJURIES INCLUDING INJURIES TO THE NECK AND VERTEBRAE, A PARTIAL COLLAPSED LUNG, ROTATOR CUFF INJURIES TO THE RIGHT SHOULDER, AND TEARS TO THE BICEPS. A POLICE REPORT WAS FILED OF THE INCIDENT. THE VEHICLE WAS TOWED TO A BODY SHOP AND DECLARED DESTROYED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE CONTACT STATED THAT THE SEAT BELTS FRACTURED DURING THE CRASH AND THE AIR BAGS DID NOT DEPLOY. THE VEHICLE WAS TAKEN TO A LOCAL REPAIR SHOP WHERE IT WAS DEEMED AS DESTROYED. THE MECHANIC WAS UNABLE TO DIAGNOSE THE CAUSE OF THE SEAT BELT AND AIR BAGS FAILURES. UNINTENDED ACCELERATION OCCURRED WHEN PROCEEDING THROUGH AND INTERSECTION AT ABOUT 4:50 PM 01/04/2011. BRAKES WERE APPLIED BUT THE POWER OF THE TOYOTA TUNDA SR5 V8 ENGINE HAD ENOUGH POWER TO ACCELERATE UNINTENDED TO APPROXIMATELY 50 MPH EVEN THO THE BRAKES WERE APPLIED AS HAD HARD AS IT COULD BE APPLIED. THE TRUCK CENTERED AN OAK TREE IN THE BLVD APPROXIMATELY 150-200 FT FROM THE INTERSECTION. THE TRUCK WAS TOTALED BY MY INSURANCE COMPANY. ONLY THE DRIVER WAS IN THE TRUCK AND NO OTHER VEHICLES AT THE TIME OF THE INCIDENT. THE DRIVER SUFFERED A BROKEN LEFT ARM AT THE ELBOW WHICH REQUIRED SURGERY. *LN

THE CONTACT STATED THAT THE METAL SUPPORT IN THE HEAD REST CAME THROUGH THE PADDING AND HIT HIM IN THE HEAD, CAUSING HIM TO LOSE CONSCIOUSNESS. A POLICE REPORT WAS FILED. THE HEAD REST HAD NOT BEEN REPAIRED PER THE MANUFACTURER'S REQUEST. THE FAILURE MILEAGE WAS APPROXIMATELY 113,000 AND THE CURRENT MILEAGE WAS APPROXIMATELY 115,000.

2005 TOYOTA TUNDRA EXPERIENCE ELECTRONIC FAILURE*CW THE CONSUMER STATED THE MECHANICAL OR ELECTRONIC FAILURE CAUSED AN ACCIDENT AS WELL AS INJURIES. THE CONSUMER STATED ALL HE REMEMBERED WAS EXITING THE HIGHWAY IN EVENING TRAFFIC WHEN ALL OF A SUDDEN, HIS VEHICLE RAMMED INTO THE VEHICLE IN FRONT OF HIM. THE AIR BAGS DEPLOYED PRIOR TO THE COLLISION AND HE HAD NO RECOLLECTION OF BEING ABLE TO BRAKE. HE ALSO STATED THE ENGINE FAILED IN JUNE 2009. THE CONSUMER WAS INFORMED THERE MUST HAVE BEEN SOMETHING WRONG WITH THE SEAMS IN THE RADIATOR. APPARENTLY, IT BLEW OUT THE BOTTOM, WHICH CAUSED THE INSTANTANEOUS LOSS OF THE RADIATOR FLUID. *JB
2005 TOYOTA TUNDRA LIMITED, I WAS HIT BACK RIGHT MIDDLE OF TIRE, CAVING IN TRUCK BED, LOSING COMPLETE CONTROL OF VEHICLE, AXLE AND TRANSMISSION DISMANTLED, WENT INTO A 360 SPIN WITH BACK TIRES COMPLETELY BENT AND NO CONTROL. THEN HIT HEAD ON AT A HIGH RATE OF SPEED OVER 35-45 MPH. WITH AIR-BAGS NOT GOING OFF! TRUCK WAS COMPLETELY REBUILT WITH HIDDEN COST NOT RELEASED TO ME. RESULTING IN TRUCK BEING REBUILT, WHEN IT SHOULD'VE BEEN TOTALED-OUT. STILL WAITING MAJOR BACK SURGERY AFTER 15 MONTHS, LOSS OF WORK. INSURANCE COMPANIES "HIDING" HIDDEN BILLS THAT WOULD MADE MY TRUCK OVER 80% TOTALED. I NEED ANSWERS AND HELP ON WHY MY AIR BAGS DIDN'T DEPLOY SAVING MY LIFE I ONCE HAD, AND BILLS THAT I DIDN'T HAVE! AND YET TOYOTA SIGNS OFF ON MY CAR SAYING IT'S SAFE TO DRIVE AGAIN WITH MY CHILDREN ON THE HIGHWAYS. I HAD MY 2005 TUNDRA BED LOADED TO THE MAXIMUM ADVERTISED WEIGHT CAPACITY. ONCE I ENTERED THE HWY AND REACHED A SPEED OF 45 THE VEHICLE WENT OUT OF CONTROL. I HAVE BEEN DRIVING FOR MANY YEARS AND HAVE NEVER EXPERIENCED A LOSS OF CONTROL LIKE THIS. THE VEHICLE LEFT THE ROAD AND FLIPPED UPSIDE DOWN. THE CAB HELD UP GREAT AND EVERYONE WAS WEARING THEIR SEATBELT THEREFOR THERE WERE NOT SIGNIFIGANT INJURIES. ATLEAST ONE OF THE TIRES WERE OFF THE RIM BUT I'M NOT SURE IF THIS WAS A RESULT OF THE ACCIDENT OR A CAUSE.

The contact stated his wife was driving on the highway at approximately 40 MPH when she hit a patch of ice and spun out of control crashing into an embankment on the rear driver side. As a result, the rear driver side tire was pushed back out of place about an inch and a half causing the control arm to become bent. The air bags did not deploy. The wife was not injured. A police report was filed. The vehicle was able to be driven from the scene. The vehicle was not diagnosed or repaired. A dealer was not contacted. The vehicle was currently being repaired at a body shop for cosmetic repairs. The manufacturer was not notified of the failure. The failure mileage was 215,000
THE FRAME RUSTED TO THE POINT THAT IT WILL NOT PASS A STATE INSPECTION. THE BRAKES WOULD NOT STOP THE VEHICLE WHEN IN FORWARD MOTION, WHICH CAUSED AN ACCIDENT.

THE CONTACT CALLED LOCAL DEALER MAITA TOYOTA OF SACRAMENTO LOCATED AT 2500 AUBURN BLVD, SACRAMENTO, CA 95821, (916) 481-0855 AND NOTIFIED THAT THE VEHICLE WAS REPAIRED PREVIOUSLY UNDER NHTSA CAMPAIGN NUMBER: 07V013000 (SUSPENSION) BACK IN 2007. THE MANUFACTURER WAS NOT INFORMED OF FAILURE. THE FAILURE MILEAGE WAS APPROXIMATELY 330,000.
TOYOTA TUNDRA FRAME RECALL - FRAME COATING APPLIED PER RECALL IN 2014. VEHICLE INSPECTED TWICE BY DEALER IN 2018 & 2019 AND VEHICLE DETERMINED TO BE OK. RECENTLY EXPERIENCED LOSS OF BRAKES AND CRASH INTO GARAGE. VEHICLE INSPECTED BY INDEPENDENT MECHANIC AND FOUND 2 PERFORATED AND RUSTED BRAKE LINES. SCRAPING AWAY THE UNDERCOATING UNCOVERED OTHER CORROSION IN VARIOUS DEGREES. DETERMINED THAT SPRAY UNDERCOATING CAME WAY TO LATE AS TRUCK ALREADY HAD 9 YEARS OF CORROSION. UNDERCOATING ONLY HID THE PROBLEM. SUBMITTED A CLAIM WITH TOYOTA WHO OFFERED NO ASSISTANCE.
