The contact owned a 2015 Chevrolet Equinox. The contact stated that while driving at 45 mph, another vehicle in front of the contact's made a left turn but the contact was not able to brake in time and rear-ended the other vehicle. No air bags were deployed. The contact was transported by ambulance for a compression fracture on her vertebrae. A police report was filed. The vehicle was towed to a tow yard and deemed totaled by the insurance company. The manufacturer was not made aware of the failure. The failure mileage was approximately 85,000.
ON DEC 31, 2020 I STRUCK A DEER ON A COUNTY RD GOING APPROX 45MPH. THE IMPACT WAS SO HARD THE DRIVE TRAIN WAS SHOVED INTO THE CAR AND THE RADIATOR EXPLODED AND THE ENTIRE FRONT PASSENGER SIDE WAS DESTROYED. NEITHER PASS. NOR DRIVERS SIDE DOORS WOULD OPEN. THE CAR IS A TOTAL LOSS YET THE AIRBAGS NEVER DEPLOYED THROUGHOUT THE ENTIRE VEHICLE BUT YOU COULD SEE A WHITE POWDERY SMOKE AND IT SMELL AS IF THEY DID AND THE CAR NOW SAYS SERVICE AIRBAGS. ALSO THE DRIVER AND PASSENGER SIDE SEAT BELT NEVER RETRACTED BACK AND MY DAUGHTER AND I WERE BOTH INJURED AS I WAS SLAMMED INTO THE STEERING WHEEL SUFFERING DAMAGE TO MY CHEST AND FACE AND HER HEAD/ FACE BOUNCED OFF THE DASH DUE TO THE AIR BAGS NOT DEPLOYING AND THE SEATBELTS NOT WORKING. WE ARE LUCKY WE WEREN'T KILLED AS THE ENTIRE FRONT END OF THE VEHICLE, ENGINE INCLUDED WAS PUSHED TO THE INSIDE OF THE CAR. WE WERE GOING TO LOOK INTO GETTING US ANOTHER EQUINOX AS I HAD JUST HAD THIS ONE FOR ABOUT SEVEN MONTHS BUT I ABSOLUTELY LOVED IT BUT AFTER READING ALL THESE REPORTS THAT ARE IDENTICAL TO OURS AS FAR AS SEATBELTS AND AIRBAGS GO AND REALIZING THIS IS A VERY COMMON THING WITH 2015 EQUINOX AND AFTER THIS HAPPENING TO US IT'S OBVIOUS THIS VEHICLE IS NOT SAFE TO DRIVE OR AT LEAST NOT SAFE TO GET INTO AN ACCIDENT WITH AND IT SOUNDS LIKE THERE NEEDS TO BE A RECALL BUT YET THAT HASN'T HAPPENED. I HAVE READ HUNDREDS OF REPORTS EXACTLY LIKE THIS ONE. I'M VERY DISAPPOINTED IN CHEVY FOR LETTING THIS CONTINUALLY HAPPEN WITH NO RECALL OR WARNING TO YOUR CUSTOMERS. . A POLICE REPORT WAS FILED ON THIS AND INJURIES DID OCCUR. THE POLICE OFFICER AND THE WRECKER DRIVER SAID THEY'VE NEVER SEEN A DEER DO SO MUCH DAMAGE TO A VEHICLE BEFORE AND THE OFFICER COULD NOT BELIEVE THAT THE AIRBAGS DID NOT DEPLOY ON US AND SAID THAT WE MOST DEFINITELY NEED TO CHECK INTO IT BECAUSE HE WOULD ASSUME THAT THEY WERE RECALLED BUT HERE WE ARE AND THEY'RE NOT.

TAKATA RECALL: MY CAR WAS INVOLVED IN A 3 CAR CRASH AT THE CORNER OF 28TH AVE N AND 4TH ST. N ON DECEMBER 14, 2018. I WAS IN THE MEDIAN WAITING TO CROSS ONCOMING TRAFFIC WHEN I PERCEIVED THE LANES WERE CLEAR AND PROCEEDED TO CROSS THE ROAD. I WAS T-BONED BY AN ONCOMING DODGE AVENGER, INTO THE FRONT END OF A JEEP WRANGLER AND PUSHED INTO A STOP SIGN. MY AIRBAGS FAILED TO DEPLOY BUT THE AVENGER'S PASSENGER AND DRIVER AIRBAGS DEPLOYED.I WAS TAKEN TO BAYFRONT ER BY AMBULANCE AND ADMITTED TO TRAUMA DEPARTMENT SUFFERING A STERNUM FRACTURE AND COMPRESSED CHEST INJURIES. MY LEFT HAND ALSO SUFFERED A POSSIBLE FRACTURE. THE POLICE REPORT IS NOT FACTUAL. IT STATES MY AIRBAGS WERE DEPLOYED BUT FROM THE PICTURE YOU CAN CLEARLY SEE THEY WERE NOT AND THE INSURANCE ADJUSTER WILL TESTIFY THAT THEY WERE NOT DEPLOYED.
TAKATA RECALL I WAS INVOLVED IN A CAR WRECK ON A CITY STREET. MY CAR WAS TOTALED AND NOT EVEN ONE AIRBAG DEPLOYED. IT WAS A FRONT END COLLISION. I WAS TRAVELING EAST ABOUT 20-25 MILES PER HOUR AND S VEHICLE RAN A STOP SIGN GOING EXCESSIVELY FAST. I HIT THE PASSENGER SIDE OF HER VEHICLE. MY VEHICLE SPUN AROUND 90 DEGREES AND ENDED UP IN A DITCH HITTING A STOP SIGN ON DRIVERS SIDE DOOR. FRONT OF MY EQUINOX WAS TOTALED.
VEHICLE WAS STRUCK IN THE FRONT AND RIGHT SIDE BY ANOTHER VEHICLE AS DRIVER WAS CROSSING A TWO-LANE CITY HIGHWAY. THE SEAT BELTS WERE ON,BUT NONE OF THE FRONT OR SIDE AIRBAGS DEPLOYED.THE DRIVER SUFFERED A MILD CONCUSSION AS A RESULT OF BEING THROWN AROUND AND BRUISED LEG. PASSENGER WAS THROWN AROUND,ALSO, RESULTING IN BOTH BEING CHECKED OUT AT THE ER.THE VEHICLE WAS DECLARED A TOTAL LOSS BY INSURANCE COMPANY.
AFTER AN ACCIDENT, THERE WAS SIGNIFICANT DAMAGE TO THE FRONT END INCLUDING PART OF THE FRAME. INSIDE THE VEHICLE HOWEVER, NONE OF THE AIRBAGS DEPLOYED AT TIME OF THE ACCIDENT. THE VEHICLE WAS LATER TOTALED DUE TO THE SIGNIFICANT DAMAGE TO THE MOTOR, PART OF THE FRAME AND FRONT END.
THE CAR WAS IN PARK & I WAS SITTING IN IT ON THE TOP OF MY DRIVEWAY. I WAS ABOUT TO OPEN THE DOOR WHEN IT STARTED TO ROLL BACKWARDS WHILE STILL IN PARK. IT STARTED TO PICK UP SPEED & DID NOT STOP UNTIL IT HIT A TREE. I WAS SO SHOCKED BY WHAT WAS HAPPENING & THE FACT THAT I LIVE ON THE RIVER EVEN THOUGH NEARLY 900 FEET AWAY ALL I COULD THINK OF WAS IT WAS GOING TO ROLL STRAIGHT IN THE RIVER. I JUMPED OUT OF THE CAR & SEVERELY INJURED MYSELF WITH A CONCUSSION , CRACKED RIBS & A BROKEN NOSE. THE CAR WAS IN PARK & STARTED TO ROLL WHILE STILL IN PARK. WHEN IT FINALLY CAME TO STOP BY HITTING A TREE IT WAS STILL IN PARK. MY MECHANIC BELIEVES IT IS A PROBLEM WITH THE TRANSMISSION. GENERAL MOTORS SENT SOMEONE OUT TWICE & AT THIS POINT WE ARE IN LIMBO. THE CAR IS NOT DRIVEABLE - IF WE HAVE IT REPAIRED MY INSURANCE DOES NOT FEEL IT IS A SAFE CAR TO DRIVE WITH OUT KNOWING THE REASON IT ROLLED BACK. GM HAS NOT BEEN HELPFUL OR VERY FORTHCOMING. I HAVE MANY PICTURES BUT WAS HAVING A PROBLEM UPLOADING THEM. IF YOU NEED TO SEE THEM I CAN MAIL THEM IF I STILL CAN NOT UPLOAD THEM.
TL* THE CONTACT OWNS A 2015 CHEVROLET EQUINOX. THE CONTACT WAS DRIVING APPROXIMATELY 25 MPH, THE CONTACT HEARD AN ABNORMAL NOISE AND CRASHED INTO A CURB. THE CONTACT STATED THAT THE ON-STAR READING INDICATED THAT THE AIR BAGS DEPLOYED HOWEVER, THE AIR BAGS FAILED TO DEPLOY. IN ADDITION, THE CONTACT STATED THAT AFTER THE CRASH, THE DRIVERS SIDE SEAT BELT FAILED TO RETRACT. THE DRIVER SUSTAINED BACK PAINS AND ABRASIONS FROM THE SEAT BELT BUT IT WAS UNKNOWN IF MEDICAL ATTENTION WAS REQUIRED. A POLICE REPORT WAS NOT FILED. THE VEHICLE WAS NOT DIAGNOSED OR REPAIRED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE AND THE CONTACT WAS NOT INCLUDED IN NHTSA CAMPAIGN NUMBER: 15V666000 (AIR BAGS). THE APPROXIMATE FAILURE MILEAGE WAS 10,000.
MY VEHICLE WAS STRUCK IN THE FRONT BY ANOTHER VEHICLE AS I WAS CROSSING A FOUR LANE HIGHTWAY. OTHER VEHICLE WAS RUNNING 65 MPH. MY SEAT BELT LOCKED, BUT NONE OF THE FRONT OR SIDE CURTAIN AIR BAGS DEPLOYED. I SUFFERED A MILD CONCUSSION AS A RESULT OF MY HEAD STRIKING THE DRIVER'S SIDE WINDOW. IMMEDIATELY AFTER THE WRECK, ONE COULD SMELL THE INFLATORS RELEASING VAPOR, BUT NO DEPLOYMENT OF THE BAGS OCCURRED. VEHICLE WAS A TOTAL LOSS DUE TO STRUCTURAL DAMAGE.
Hello, on September 5, 2025 I was driving my 2015 Chevy equinox. I was in a collision while driving my 2015 equinox going at a speed of 60 mph on a highway. On the day of the collision i hit a cargo truck that was carrying vehicles an it completely destroyed the front of my 2015 chevy equinox despite the severity of the impact. None of the airbags deployed. Fortunately me and my daughter did not sustain any injuries but I agree strongly that air bags should have deployed by the severity and location of the impact. This could have been more dangerous if someone else had been in the vehicle or if the wreck was slightly worse. My vehicle was viewed by the police at scene but no one else. There was no warning lights or messages prior to the accident. I am submitting this report because I believe the airbag system may have failed and I wanna make sure this is investigated I still have the vehicle and can provide photos and further details if needed.
Car received a front impact and airbags did not deploy despite the alert on the dash saying “service airbags” immediately after collision. Seat belts engaged though.
GM- Has Issued Recall on 2010-2013 Equniox's Or extended the warranty on the powertrain for those years. Unfortunately, from 2014-to 2017 same engine wasn't included in this extended 150,000 warranty. Know issues of rear main seal blowing out causing all of the oil to leak for the engine while driving not one warning light came on! This blew up the engine with my wife and daughter in the car! They could have DIED!!!!!! What's wrong with GM knowing that this can happen! Mine has 73,000 miles on it and they will not repair it! They have to do better before someone gets killed!
Without warning, windshield wipers suddenly stopped working during a significant rain storm that resulted in loosing all visual of interstate. Traffic had slowed to 45 MPH due to rain prior to the complete failure of windshield wiper transmission. As we were lowering driver and passenger windows to get a bearing of where we were in the interstate, we were sideswiped by a semi that caused significant damage to the entire drivers side of the car and a flat tire. This was a serious safety issue that was very close to causing a fatality and/or multi-car collision.
WE HIT A DEER GOING 70 MPH LAST NIGHT ON INTERSTATE 10 HEADED WESTBOUND , UPON IMPACT THE AIRBAGS DID NOT GO OFF. WE SMELLED SOMETHING THAT SMELLED LIKE IT WAS BURNING, IT WAS THE IGNITER TO THE AIRBAGS. NOW THE CAR IS GIVING US A SERVICE AIRBAG MESSAGE ON THE SCREEN. OUR AIRBAGS ALL FAILED TO DEPLOY.
I HAVE HAD PROBLEMS WITH THIS VEHICLE BROUGHT TO THE DEALERS ATTENTION AND WAS TOLD NOT COVERED. VEHICLE STALLED OUT AND I WAS REAR ENDED DUE TO ISSUE WITH POWER TRAIN
I WAS DRIVING HOME FROM WORK.WHILE MAKING A TURN MY CAR WAS UNABLE TO BRAKE. I SLAMMED INTO A TELEPHONE POLE WHICH MY AIRBAGS DID NOT DEPLOY. MY CAR HIT THE POLE A HARD RIGHT. MY HEAD AND NECK WAS PUSH FORWARD EXTREMELY FAST CAUSING TRAUMA.
THE IGNITION KEY WAS ABLE TO BE REMOVED FROM THE IGNITION WHILE THE VEHICLE WAS NOT IN THE PARK POSITION (VEHICLE WAS IN DRIVE POSITION). IN WHICH CAUSED A ROLLAWAY INCIDENT WITH THE VEHICLE. THE VEHICLE WAS STATIONARY AND BELIVED TO BE IN PARK.

A POLICE REPORT WAS FILED AND THERE WERE NO INJURIES. THE VEHICLE WAS TOWED. THE CAUSE OF THE FAILURE WAS NOT DETERMINED. THE MANUFACTURER WAS NOT NOTIFIED. THE FAILURE MILEAGE WAS 10,000.
