TL* THE CONTACT CALLED ON BEHALF OF HER SON WHO WAS THE VEHICLE OWNER/DRIVER OF A 2015 CHEVROLET TAHOE. WHILE DRIVING AT APPROXIMATELY 45-50 MPH, THE VEHICLE CRASHED INTO A TREE. THE CONTACT WAS CONCERNED THAT THE COLLISON MAY HAVE BEEN RELATED TO EXISTING RECALLS ON THE VEHICLE. THE DRIVER WAS FATALLY INJURED. THE CONTACT WAS THE MOTHER OF THE DRIVER. THE CONTACT STATED THAT WHILE HER SON WAS DRIVING BETWEEN 45-50 MPH, HE LOST CONTROL OF THE VEHICLE, JUMPING A CURB, AND CRASHED INTO A TREE. DURING THE CRASH THE AIR BAGS DID DEPLOY BUT THE DRIVER WAS NOT WEARING A SEAT BELT. THE DRIVER WAS FATALLY INJURED DURING THE CRASH AND SUSTAINED TWO BROKEN LEGS. THE CONTACT STATED THAT BASED ON THE POLICE AND EMS REPORTS RECEIVED, THE MIDDLE CONSOLE HAD BEEN RIPPED OFF BY THE DRIVER DURING THE INCIDENT IN AN ATTEMPT TO BRACE HIMSELF FOR THE COLLISION. THE REPORT ALSO MENTIONED THAT NO TIRE SKID MARKS WERE PRESENT AT THE SCENE. THE VEHICLE WAS DESTROYED AND TOWED FROM THE SCENE. THE CONTACT STATED THAT THE EVENT DATA RECORDER (EDR) WAS DESTROYED. AS A RESULT, THE CAUSE OF THE FAILURE WAS NOT DIAGNOSED. THE CONTACT DISCOVERED THAT THE VIN WAS LATER INCLUDED IN NHSTA CAMPAIGN NUMBERS: 18V586000 (STEERING) AND 19V645000 (SERVICE BRAKES, HYDRAULIC). THE MANUFACTURER WAS NOTIFIED OF THE INCIDENT. THE FAILURE MILEAGE WAS 50,000.

THE CONTACT'S WIFE WAS TRANSPORTED TO THE HOSPITAL BY AMBULANCE AS SHE SUSTAINED BROKEN RIBS AND CHEST PAINS. THE VEHICLE WAS TOWED TO A TOW YARD AND DEEMED A TOTAL LOSS. THE MANUFACTURER NOR LOCAL DEALER WAS NOT NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS 100,000.
ATTEMPTING TO BACK DOWN A HILL A SLOW SPEED LOST ALL BRAKES SLID OF HILL ROLLED 4 TIMES. CONCUSSION, LOSS OF EYESIGHT IN RIGHT EYE.
TL* THE CONTACT OWNED A 2015 CHEVROLET TAHOE. WHILE DRIVING VARIOUS SPEEDS, THE DIGITAL INSTRUMENT PANEL WOULD DISPLAY INCORRECTLY. ALSO, THE AIR CONDITIONER AND REMOTE START FAILED. THE VEHICLE WAS TAKEN TO WASHINGTON CHEVROLET (1 RAYMOND BLVD, WASHINGTON, PA 15301 (724) 222-2800), WHERE A SOFTWARE MALFUNCTION WAS DIAGNOSED AND THE SOFTWARE WAS UPDATED, BUT THE FAILURE RECURRED. SEVERAL MONTHS LATER, WHILE DRIVING 25 MPH, THE CONTACT INADVERTENTLY SWERVED TO THE LEFT AND STRUCK A UTILITY POLE AND FENCE. THE AIR BAGS FAILED TO DEPLOY AND THERE WERE NO WARNING INDICATORS ILLUMINATED. THE CONTACT SUSTAINED HEAD INJURIES AND BRUISING THAT REQUIRED MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO THE DEALER AND DEEMED A TOTAL LOSS. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE, BUT DID NOT ASSIST. THE CONTACT MENTIONED THAT THE VEHICLE WAS SERVICED PER NHTSA CAMPAIGN NUMBER: 16V651000 (AIR BAGS, SEAT BELTS). THE FAILURE MILEAGE WAS APPROXIMATELY 24,000.

THE CONTACT SUSTAINED AN INJURY TO THE LOWER BACK, BUT NO MEDICAL ATTENTION WAS REQUIRED. A POLICE REPORT WAS NOT FILED. THE VEHICLE WAS TOWED TO NISSANI BROS. CHEVROLET (6101 W SLAUSON AVE, CULVER CITY, CA 90230 (424) 216-7400), BUT IT HAD NOT BEEN DIAGNOSED. THE VEHICLE WAS NOT REPAIRED. THE CONTACT REFERENCED NHTSA CAMPAIGN NUMBER: 16V256000 (SUSPENSION). THE MANUFACTURER WAS NOTIFIED OF THE FAILURE AND THE CONTACT WAS PROVIDED CASE NUMBER: 8-3571350184. THE CONTACT WAS ADVISED TO CALL NHTSA. THE APPROXIMATE FAILURE MILEAGE WAS 25,400.
MY WIFE'S 2015 TAHOE HYDROPLANED SLID BACKWARDS INTO THE MEDIAN, HIT A GUARD RAIL AND ROLLED 3 TIMES. ALL SIDE CURTAIN AIRBAGS DEPLOYED BUT NOT THE FRONT AIRBAGS. THIS WAS ON 8/4/2017. THEN 2 WEEKS LATER WE RECEIVED A RECALL LETTER FOR THE SDM SOFTWARE WHICH CONTROLS THE FRONTAL AIRBAGS AND SEATBELT PRETENSIONERS.
I WAS HIT IN A HEAD ON COLLISION GOING THROUGH A STOP LIGHT WHICH RESULTED IN ME HITTING THE STEERING WHEEL AND MY AIR BAG DID NOT DEPLOY. FURTHERMORE SINCE THEN MY BACK WINDSHIELD WIPER WORKS ONLY OCCASIONALLY. OFCOURSE IT WORKS WHEN I CAN GET IN AT THE DEALERSHIP.

THE VEHICLE SPED FROM THE CONTACT'S DRIVEWAY INTO THE FRONT NEIGHBOR'S YARD BEFORE CRASHING INTO THE NEIGHBOR'S AIR CONDITIONING UNIT. AFTER THE CRASH, THE VEHICLE SPUN AROUND UNTIL THE PASSENGER TIRE PARTIALLY SEPARATED FROM THE WHEEL WELL. IN ADDITION, THE BRAKES FAILED AT THE TIME OF THE INCIDENT. THERE WERE DEEP SKID MARKS VISIBLE ON THE NEIGHBOR'S YARD. THE CONTACT WAS TRANSPORTED FROM THE SCENE VIA AMBULANCE. THE CONTACT WAS TREATED FOR UNKNOWN INJURIES THAT REQUIRED IMMEDIATE MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. THE VEHICLE WAS DESTROYED. THE AIR BAGS FAILED TO DEPLOY AND THE CONTACTS SEAT BELTS FAILED TO LOCK. MEDICAL ATTENTION WAS REQUIRED FOR THE PASSENGERS OF THE OTHER VEHICLE FOR UNKNOWN INJURIES. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO AN INDEPENDENT MECHANIC. THE FAILURE WAS NOT DIAGNOSED BUT THE ENTIRE FRONT PANEL NEEDED TO BE REPLACED. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 9200.
The contact owns a 2015 Chevrolet Tahoe. The contact stated while attempting to park the vehicle, the brake pedal was difficult to depress and failed to respond. To avoid crashing into the vehicle parked in front of her she crashed into the curb. There were no injuries. There were no warning lights illuminated. The vehicle was towed to the dealer and diagnosed that the vacuum pump needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 7,770.
BRAKING SYSTEMS NOT WORKING PROPERLY. THE VEHICLE FAILED TO STOP WHEN APPLYING THE BRAKE PEDAL. ALL BRAKE PADS ARE GOOD; BRAKE CALIPERS ARE NEW; BRAKE FLUID IS AT MAX. I REPLACED EVERYTHING I FOUND ONLINE, BUT THE ISSUE IS THERE ON THE VEHICLE.
THE ELECTRICAL SOFTWARE ISN'T FUNCTIONING PROPERLY. THE RADIO, ONSTAR, THE DOOR/SEAT BELT CHIMES DON'T WORK. NONE OF THE ELECTRICAL WORKS IN THE VEHICLE. I WAS DRIVING AND WHEN I PRESSED ON THE BRAKE AND TURNING, THE DOME LIGHTS START TO FLICKER MAKING IT DIFFICULT TO SEE OTHER CARS AT NIGHT. THE BACK DOORS DON'T ALWAYS LOCK WHEN LOCKING THEM.
I PULLED INTO MY DRIVEWAY... THE ENGINE SPEED STARTED INCREASING AND SIMULTANEOUSLY THE BRAKE PEDAL SUNK INTO THE FLOOR AND BRAKING ABILITYWAS LOST. THIS HAPPENED ABOUT 4 FEET FROM MY GARAGE DOOR AND THIS CAUSED MY TRUCK INTO THE GARAGE DOOR. I HAVE A DRIVEWAY VIDEO OF ME ON THE PHONE AND THEN THE CAR BEING ACCELERATED THROUGH THE GARAGE DOOR.

THE VEHICLE WAS NOT TAKEN TO A LOCAL DEALER FOR DIAGNOSIS OR REPAIRS. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND INFORMED THE CONTACT THAT NO RECALL WAS UNDER THE VIN. THE CONTACT RECEIVED NOTIFICATION OF NHTSA CAMPAIGN NUMBER: 19V645000 (SERVICE BRAKES, HYDRAULIC), THE VEHICLE WAS TAKEN TO THE DEALER FOR SERVICE HOWEVER A FAILURE OCCURRED. THE FAILURE MILEAGE WAS APPROXIMATELY 41,030.
OCTOBER 8, 2019 WAS TAKING OFF IN TRAFFIC AFTER STOPPING AT A RED LIGHT, APPROXIMATELY 5-10 MPH. THE TRAFFIC STOPPED, ATTEMPTED TO APPLY THE BRAKES AND PEDAL FELT HARD. REAR END COLLISION OCCURRED WITH THE CAR IN FRONT OF ME CAUSING REAR DAMAGE TO THAT VEHICLE AND FRONT DAMAGE TO MY VEHICLE. APPROXIMATELY 2 MONTHS LATER WAS PULLING INTO A PARKING SPOT AT A LOW SPEED AND COULD NOT STOP THE VEHICLE, FELT LIKE THE BRAKE PEDAL WAS STUCK TO THE FLOOR. PULLED THE EMERGENCY BRAKE AND WAS ABLE TO GET STOPPED WITHOUT A COLLISION. TOOK VEHICLE INTO DEALER ON JANUARY 10TH. THEY REPROGRAMMED THE HYDRAULIC BRAKE BOOST BUT SAID THE PEDAL WAS STILL HARD AND THEY WOULD NEED TO REPLACE THE MECHANICAL VACUUM OUTPUT. THE PART IS ON BACKORDER BUT THEY WOULD NOT RELEASE THE VEHICLE BACK TO ME BECAUSE OF SAFETY RISKS YET I'VE BEEN DRIVING AROUND FOR MONTHS WITH SAME PROBLEM. ALL RECALLS REVIEWED ARE SUGGESTIVE IF YOU EXPERIENCE THIS PROBLEM NOT A MANDATORY REPAIR. WHY WAIT UNTIL SOMEONE HAS A CRASH LIKE I DID?? ONCE RESEARCHED THIS HAS BEEN A KNOWN ISSUE FOR MONTHS WITH 3 ASSOCIATED RECALLS. THIS IS A HUGE SAFETY ISSUE, NEEDS TO BE MANDATORY REMEDIED. MY VEHICLE DAMAGE WAS SET TO BE REPAIRED NEXT WEEK BUT NOW IT HAS TO BE CANCELLED BECAUSE I DO NOT HAVE THE VEHICLE IN POSSESSION.
THE BRAKING SYSTEM FAILED WHILE DRIVING; I WAS APPROACHING A TRAFFIC LIGHT AND SLOWING DOWN AND AT ABOUT 15-20 MPH AND SLOWING THE BRAKE PEDAL WENT STRAIGHT TO THE FLOOR. THE VEHICLE WAS NO LONGER SLOWING DOWN SO I PUMPED THE BRAKES TO TRY AND REGAIN SOME KIND OF BRAKING AND THE ABS SYSTEM TRIED TO KICK IN BUT WAS ONLY PARTIALLY AFFECTIVE AND SLOWED THE VEHICLE DOWN ENOUGH TO COME TO A STOP AFTER I CROSSED THE INTERSECTION AND RAN OVER A CURB.
INTERIOR DOME LIGHTS FLASHES DURING NIGHT DRIVING CAUSING THE DRIVER TO LOSE VISION DURING NIGHT DRIVING.
BRAKE VACUUM PUMP LOSES PRESSURE WHEN BRAKES PRESSED REPETITIVELY. WHILE IN TRAFFIC I REAR ENDED ANOTHER MOTORIST DUE TO BRAKE PRESSURE LOSS. CAUSING DAMAGE TO BOTH VEHICLES. NO INJURIES DUE TO LOW SPEED BUT EVEN AT HIGH SPEEDS BRAKE PRESSURE IS LOST, IF THE BRAKE PEDAL IS DEPRESSED REPEATEDLY YOU LOSE ALL BRAKE ASSIST. VEHICLE WAS BROUGHT IN FOR RECALL REGARDING ABS SOFTWARE UPDATE BUT BRAKES STILL FEEL HORRIBLE. TSB WAS ISSUED TO VEHICLES UNDER 72000 BUT ONLY BEGAN THIS YEAR. CURRENT TSB DOES NOT COVER MY VEHICLE. I CONTACT THE GM REGARDING THIS ISSUE AND THEY TOLD ME THAT THEY WILL NOT COVER MY VEHICLE ONE MAKE EXCEPTIONS.THIS NEEDS TO BE A FULL RECALL OF THE VACCUM PUMP, VACUUM LINE AND MASTER CYLINDER LIKE STATED IN THE TSB. SOMEONE CAN DIE FROM THIS ISSUE IF GOING DOWN HILL AT SPEED.
2015 CHEVY TAHOE, WE PURCHASED TWO MONTHS AGO STILL UNDER FACTORY WARRANTY. I WAS TURNING AROUND IN THE DRIVEWAY, HIT THE BRAKES, AND THE BRAKE PEDAL WOULDN'T MOVE. IT WAS EXTREMELY STIFF, CAUSING ME TO CRASH THE VEHICLE INTO MY UTILITY TRAILER. I PUSHED AS HARD AS POSSIBLE AND NOTHING HAPPENED. THERE WAS ALMOST NO BRAKING PRESSURE AT ALL AND MY SUV JUST KEPT GOING. I PURCHASED THIS VEHICLE FOR MY FAMILY BUT NOW EVERYONE IS SCARED TO RIDE IN IT. I'VE FINANCED A VEHICLE FOR YEARS THAT I'VE HAD POSESSION OF FOR MONTHS, AND IT'S MOST NESSCERY SAFTEY FUNCTION HAS FAILED. I'M EXTREMELY DISAPPOINTED. I'VE ALWAYS OWNED GM VEHICLES. AS A 31 YEAR OLD FATHER OF 3 SMALL CHILDREN, THIS WAS A VERY LARGE PURCHASE FOR OUR YOUNG FAMILY. NOW I FEEL I MAY HAVE TO LOOK OUTSIDE MY TRUSTED BRAND TO FIND A SAFE VEHICLE I CAN TRANSPORT THE MOST PRECIOUS CARGO IN. IT'S VERY DISAPPOINTING . I THEN FIND OUT THIS EXACT ISSUE HAS CAUSED MULTIPLE CRASHES INVOLVING INJURIES. GM IS AWARE THIS IS A PROBLEM, LAWSUITS HAVE BEEN FILED, YET LITTLE OR NOTHING HAS BEEN DONE. HOW CAN GENERAL MOTORS KNOWINGLY TURN A BLIND EYE AND ALLOW THIS LIFE THREATING SAFTEY ISSUE TO GO ON IN A VEHICLE MARKETED TO FAMILIES?
