I placed my truck in Park, exited the vehicle to assist my daughter to the truck. As we approached the front of the truck - the truck started Rolling forward. I tried to enter the driver side door to stop the vehicle but realized my truck was going to collide with the vehicle next to it and I tried to clear my body of the two vehicles. I was caught between the two vehicles near the rear driver side fender and couldn't free myself. My daughter entered the truck from the passenger side and the truck had went from Park to Drive. My daughter put the truck in Park and somehow I was able to free myself. I received injuries to my hip and pelvic area. The entire incident was recorded on a home surveillance system.

TL* THE CONTACT OWNED A 2016 FORD F-250. THE CONTACT STATED THAT WHILE HER SON WAS DRIVING 35 MPH AND TOWING A TRAILER WITH HIS ATV ON, A VEHICLE IN FRONT WAS MAKING A LEFT TURN AND THE DRIVER DEPRESSED THE BRAKE PEDAL. AN UNKNOWN WARNING LIGHT ILLUMINATED AND THE VEHICLE STALLED. THE CONTACT LOST CONTROL OF THE VEHICLE AND CRASHED INTO THE VEHICLE IN FRONT THAT WAS MAKING A LEFT TURN. THE VEHICLE CONTINUED 125 FEET DOWN THE ROAD, CROSSED OVER TO THE OPPOSITE LANES, CRASHED INTO SOME TREES WHERE IT EVENTUALLY CAME TO A STOP. NO AIR BAGS DEPLOYED. THE DRIVER SUSTAINED A CONCUSSION FROM HIS FACE HITTING THE STEERING WHEEL DURING THE IMPACT, MUSCLE NECK AND SHOULDER INJURIES WHICH RECEIVED MEDICAL ATTENTION FROM THE HOSPITAL. THE OTHER DRIVER SUSTAINED UNKNOWN INJURIES AND WAS TAKEN TO THE HOSPITAL VIA AMBULANCE. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED BY GEICO TO THE CONTACT RESIDENTS WHERE AN ADJUSTER INSPECTED THE VEHICLE AND TOWED THE VEHICLE TO A REPAIR FACILITY WHERE IT WAS DEEMED THE VEHICLE A TOTAL LOSS. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND INFORMED THE CONTACT THAT NO RECALL WAS UNDER THE VIN. NO FURTHER INFORMATION WAS PROVIDED BY THE MANUFACTURER. NO DEALER WAS CONTACTED. THE FAILURE MILEAGE WAS APPROXIMATELY 70,000.

When driving my truck "2016 F250, FX4) on an open highway at 80mph out of no where, i experience what is known as the death wobble, and went off the road into the ditch. The dealership is telling me I need to pay 2000.00 plus dollars to address the problem which appears to be known and have not issued a recall. This same issue happened to my 2014 F250, and the base warranty covered it.

INCIDENT #1 MARCH 22ND 2021: FRONT DRIVER'S TIRE COMPLETELY BLEW ON HIGHWAY EN ROUTE TO TENNESSE AN HOUR AND A HALF OUT OF GATLINBURG, TN. SPEEDOMETER WAS ALSO NOT FUNCTIONING PROPERLY PRIOR TO SAYING WE WERE DRIVING A FASTER THAN ACTUAL MPH (CONFIRMED WHILE USING GPS). SPARE TIRE WAS PUT IN PLACE OF BLOWN OUT TIRE, CHANGED SPARK PLUG WIRE (WHICH DIDN'T REPAIR TICKING NOISE PER MECHANIC) MECHANIC ALSO SAID WE WOULD BE GOOD TO GO WITH SPARE TIRE. INCIDENT #2 MARCH 24TH 2021: WHILE EN ROUTE TO NEXT DESTINATION (NEW ORLEANS, LA) TRED CAME OFF REAR DRIVERS SIDE TIRE CAUSING TRUCK TO GO OUT OF CONTROL ON I-59 NEAR BIRMINGHAM, AL. VEHICLE SPUN INTO GRASS MEDIAN ONTO OPPOSITE SIDE OF INTERSTATE AND WAS NO LONGER DRIVABLE. A POLICE REPORT WAS MADE AND TRUCK WAS TOWED. On [XXX], I was driving westbound on [XXX], between Kerrville and Junction, Texas. I noticed a diesel fuel smell and pulled off the highway to investigate the source. Upon stopping, fire erupted from the engine compartment and quickly spread to the front dashboard and front passenger seating area. No warning lamps, no high temp or abnormal gage levels. The Mountain Home, TX volunteer fire dept arrived about 30 minutes after we called 911 and extinguished the remaining flames. They commented that a cracked / failed secondary fuel filter housing could be to blame. My insurer, State Farm, under claim #[XXX], now has possession of the vehicle remains. I told State Farm that NHTSA opened an investigation into possible fuel filter housing failures / fires. I do not know what their intentions are with the vehicle remains. On September 3, 2023, the fuel pump failed while the truck was in the corner of an on-ramp for an interstate. The truck immediately lost power while in the corner causing a loss of control of steering. This resulted in a near collision with a concrete dividing wall while attempting to maintain control of the vehicle until the vehicle came to a stop. There were no warnings, check engine lights, or other issues with the truck prior to the incident. The truck was inspected by a diesel repair company where it was determined that the Bosch CP4 fuel pump failed resulting in a catastrophic fuel system failure with metal shavings contaminating the complete fuel system. This required a complete fuel system replacement.

The failure of the APIM has completely disabled the Rearview Backup Camera, which is a federally mandated safety feature (FMVSS No. 111). Operating a heavy-duty F-250 without a functional backup camera creates a significant blind-spot hazard and an immediate safety risk to pedestrians and property. King Ford has confirmed the failure. Service Director Jamie Churchwell has acknowledged the damage as "Consequential Damage" resulting directly from the attempted software update for Safety Recall 22S35. It is currently being reviewed by the Ford District Parts and Service Manager (as reported by the dealer) for a "Related Damage" warranty claim . The vehicle’s screen and backup camera were functioning perfectly prior to the recall service. The failure occurred at the dealership during the application of the manufacturer’s recall software update.
