Vehicle was involved in an accident while starting to turn left onto a side street. The Tacoma was struck in both driver side doors by a 2005 Chevrolet Silverado K3000 driven by an 85 year old who either ran the stop sign or did not see her even though she was signaling and the truck had the option driving lights that are on all the time. The Tacomas doors were pushing in almost a foot and the truck was pushed sideways several feet. Tacoma driver injured her side and has a probable concussion. This Tacoma is equipped with side seat and side curtain airbags but neither deployed. The insurance claims adjuster stated that based on the damage, the airbags should have deployed.
TL* THE CONTACT OWNS A 2009 TOYOTA TACOMA. THE CONTACT STATED THAT HE WAS DRIVING 40 MPH AND LOST CONTROL OF THE VEHICLE, CAUSING HIM TO CRASH INTO A GUARD RAIL. THE STEERING WHEEL BROKE OFF BEFORE THE AIR BAG DEPLOYED. THE CONTACT SUFFERED HEAD INJURIES AND WAS THE ONLY OCCUPANT IN THE VEHICLE. HE WAS TAKEN TO THE HOSPITAL BY ANOTHER PERSON. THE POLICE WERE NOTIFIED AND A REPORT WAS AVAILABLE IF NEEDED. THE VEHICLE WAS THEN TOWED TO AN AUTO BODY SHOP. THE INSURANCE COMPANY WAS ALSO NOTIFIED. THE MANUFACTURER WAS NOT NOTIFIED. THE FAILURE MILEAGE WAS 28,000.

TL* THE CONTACT OWNS A 2009 TOYOTA TACOMA. WHILE AT A COMPLETE STOP WITH THE BRAKE PEDAL DEPRESSED THE VEHICLE ACCELERATED INDEPENDENTLY AND CAUSED THE CONTACT TO STRIKE ANOTHER VEHICLE. THE AIR BAGS DID NOT DEPLOY. A POLICE REPORT WAS FILED. THERE WERE NO INJURIES. THE VEHICLE WAS DRIVEN HOME. THE CONTACT STATED THAT THIS WAS THE THIRD TIME THE EXACT SAME FAILURE OCCURRED, CAUSING HIM TO STRIKE OTHER VEHICLES. PREVIOUSLY, THE VEHICLE WAS TAKEN TO A DEALER AND THE CONTACT WAS INFORMED THAT THE FAILURE OCCURRED DUE TO THE FLOOR MAT. THE ACCELERATOR PEDAL WAS CUT DOWN AND THE COMPUTER SYSTEM WAS UPDATED; HOWEVER, THE FAILURE RECURRED. THE FAILURE MILEAGE WAS 61,000. UPDATED 05/16/17*LJ *CN
AT OR AROUND 1330HRS LOCAL TIME ON APRIL11TH I WAS STATIONARY AT A RED LIGHT ON E.MAIN STREET AT FRANKLIN AVE IN ROCKAWAY BOROUGH IN DRY DAYLIGHT CONDITIONS WHEN MY TRUCK EXPERIENCED FULL ACCELERATION IN AN INSTANT PROPELLING ME FORWARD UNTIL I IMPACTED THE VEHICLE 3 FT IN FRONT OF ME ALSO STOPPED AT THE RED LIGHT. THE IMPACT INSTANTLY LOWERED THE ENGINE SPEED TO NORMAL IDLE. THIS IS THE THIRD TIME THIS HAPPENED. THE DEALER AND TOYOTA TRY TO HIDE UNDER THE FLOOR MATS INTERFERING WITH THE ACCELERATOR. THE LAST TIME THIS HAPPENED IN 2013 THE DEALER CUT THE ACCELERATOR PEDAL AND REFLASHED THE COMPUTER. IT'S STILL NOT RIGHT. TOYOTA HAD AN INDEPENDENT LAB TO LOOK AT THE EVENTS RECORDER AND REPORTED EVERYTHING IS FINE. THE ENGINE SPEEDS UP IN A RAPID MANNER AT A COMPLETE STOP GIVING THE OPERATOR JUST MILLI-SECONDS TO TRY TO REACT. A CRASH IS THE RESULT. I'M LUCKY TODAY, IT COULD HAVE BEEN PEDESTRIANS RUN OVER AND KILLED.THIS IS A SERIOUS PROBLEM, I WON'T DRIVE THE TOYOTA ANYMORE IT'S AN UNSAFE VEHICLE.
STEERING VERY STIFF, DOES NOT RETURN TO CENTER, LOCKS UP, FAILS TO MAINTAIN DIRECTION. THE UNIVERSAL JOINT ON THE STEERING COLUMN-STEERING GEAR IS NOT PROTECTED FROM THE ELEMENTS (ROAD SALT, WATER, ETC.) AND HAS SEIZED, CAUSING THE STEERING TO LOCK UP AND LEADING TO A MINOR ACCIDENT.

AFTER THE BRAKE PEDAL DEPRESSED THE VEHICLE SURGED AND CRASHED INTO ANOTHER VEHICLE. THE CONTACT SUSTAINED NO INJURIES. THE VEHICLE WAS DRIVEN FROM THE SCENE. NHTSA RECALL CAMPAIGN ID NUMBER 09V388000 (SPEED CONTROL) WAS NOT RELATED OR ASSOCIATED WITH THE RECALL. THE UNEXPECTED ACCELERATION OCCURRED TWO TIMES. THE VEHICLE WAS TAKEN TO THE DEALER WHO REPROGRAMMED THE COMPUTER SYSTEM. IN ADDITION, AN ENGINEER WAS REFERRED BY THE MANUFACTURER TO PERFORM AN INVESTIGATION RELATED TO THE DEFECT, AND GATHER INFORMATION FROM THE DATA RECORDER. THE APPROXIMATE FAILURE MILEAGE WAS 50,500.
I WAS DRIVING SOUTH ON I/S 95 IN ATTLEBORO, MA AT ABOUT 2:45 PM ON 6/6/13 IN A 2009 TOYOTA TACOMA PICKUP. MY WIFE WAS IN THE PASSENGER SEAT AND THE ONLY OTHER PASSENGER IN THE VEHICLE. I WAS TRAVELING AT ABOUT 60 MPH AND MOVED TO THE RIGHT LANE TO TAKE EXIT 2 ONTO RT 1A SOUTHBOUND. THE CRUISE CONTROL WAS ON AND ENGAGED. WHEN I FLIPPED THE LEVER TO DISENGAGE (CANCEL) THE CRUISE CONTROL THE TRUCK DID NOT START TO SLOW DOWN (MY FOOT WAS OFF THE ACCELERATOR PEDAL). IF ANYTHING, THE TRUCK STARTED TO ACCELERATE. I IMMEDIATELY APPLIED THE BRAKES VERY FORCEFULLY, WITH LITTLE, IF ANY AFFECT. I IMMEDIATELY PUT THE TRANSMISSION INTO NEUTRAL. BY THIS TIME, I WAS OFF THE HIGHWAY AND ON THE EXIT RAMP. CENTRIFUGAL FORCE WAS PUSHING ME TO THE LEFT. IT PUSHED THE TRUCK OVER THE CURB AND GRASS AND INTO THE ENTRANCE RAMP FROM 1A SOUTH ONTO I/S 95 SOUTH. AT THIS TIME, I SAW A VEHICLE ON THE ENTRANCE RAMP HEADED TOWARD ME. HAD I TRIED TO STEER TO THE RIGHT THE TRUCK WOULD MOST LIKELY WOULD HAVE SKIDDED AND BLOCKED THE RAMP ENTIRELY, SO I TURNED TO THE LEFT, TURNING INTO THE SKID, HOPING THAT THE ONCOMING VEHICLE WOULD HAVE ROOM ENOUGH TO PASS TO MY RIGHT. MY TRUCK CONTINUED SKIDDING TO MY LEFT AND I BELIEVE HIT THE CURB ON THE RIGHT SIDE OF THE ENTRANCE RAMP. IT THEN FLIPPED ONTO THE DRIVERS SIDE, SKIDDED ALONG THE PAVEMENT, HIT SOMETHING ELSE MOST LIKELY THE GUARDRAIL AND FLIPPED BACK TO THE UPRIGHT POSITION. IT CAME TO REST ON THE LEFT SIDE OF THE ENTRANCE RAMP. I DO NOT KNOW WHAT BECAME OF THE PREVIOUSLY ON-COMING AUTO. THERE WAS NOTHING TO INDICATE CONTACT BETWEEN CONTACT BETWEEN THE TWO VEHICLES. BOTH SIDE AIRBAGS DEPLOYED. BOTH OF US HAD OUR SEAT BELTS ATTACHED. THE FRONT WINDSHIELD WAS BROKEN, THE ROOF SEVERELY DENTED, SCRAPES ALONG THE DRIVER'S SIDE AND GLASS THROUGHOUT THE CAB. THE RESCUE ARRIVED AND TRANSPORTED US TO THE HOSPITAL.

THERE WAS ALSO AN ABNORMAL INCREASE IN ENGINE ROMS. THE VEHICLE CRASHED INTO THE SIDE OF THE RESIDENCE. THE MANUFACTURER WAS CONTACTED AND ADVISED THAT THEY WOULD SEND AN INVESTIGATOR TO FURTHER ASSESS THE FAILURE. THE FAILURE MILEAGE WAS UNKNOWN, BUT THE CURRENT MILEAGE WAS 20,300.
I STARTED MY TRUCK, OUTSIDE TEMPERATURE 22, PULLED IT INTO MY GARAGE AND PRESSED ON THE BRAKE TO EASE FORWARD AND THE TRUCK WOULD NOT STOP. I PRESSED AS HARD AS I COULD ON THE BRAKE BUT THE TRUCK CONTINUED FORWARD AND HIT THE FRONT WALL OF THE GARAGE. IT FELT LIKE IT WAS TRYING TO CONTINUE DRIVING FORWARD AGAINST THE GARAGE WALL. THE CONTACT STATED THAT THE BRAKE AND ACCELERATOR PEDALS WERE TOO CLOSE TOGETHER AND SHE HAS SIMULTANEOUSLY DEPRESSED BOTH PEDALS CAUSING THE ABNORMAL ACCELERATION. NEITHER THE DEALER NOR THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE VEHICLE WAS NOT REPAIRED. THE CURRENT AND FAILURE MILEAGE WAS APPROXIMATELY 8,000.
TODAY I WAS ON RT 50 IN WESTERN FAIRFAX COUNTY, VA STOPPED AND WAITING TO MAKE A LEFT HAND TURN ONTO CENTREVILLE RD. ALL OF A SUDDEN MY TOYOTA TACOMA 09 SURGED FORWARD AND GENTLY HIT THE STOPPED FULL SIZE TRUCK IN FRONT OF ME. THE GENTLEMEN IN THE FULL SIZE TRUCK GOT OUT AND SAW NO DAMAGE ON HIS TRUCK. I HAD SOME MINOR SCRATCHES ON THE FRONT BUMPER AND ON THE FRONT LICENSE PLATE. NO ONE WAS HURT SO WE DID NOT REPORT ANYTHING. I AM CONCERNED THAT THERE IS A DEFECT IN MY TRUCK THAT CAUSES THIS SUDDEN SURGE. HAD SOMEONE WALKED IN FRONT OF MY TRUCK WHEN THE SURGE OCCURRED, SOMEONE COULD HAVE BEEN SERIOUSLY INJURED. WHILE DRIVING THE VEHICLE HOME ON THE DATE IT WAS PURCHASED WHILE HE APPROACHED A STOP SIGN WITH HIS FOOT ON THE BRAKE. THE VEHICLE ACCELERATED AS HE KEPT HIS FOOT ON THE BRAKE UNTIL THE VEHICLE SLOWED DOWN. HE TOOK THE VEHICLE TO THE DEALER AND HE ASKED IF THE BRAKE PEDAL WAS TOO CLOSE TO THE GAS PEDAL. THE DEALER STATED THAT NOTHING WAS WRONG WITH THE VEHICLE. ON FEBRUARY 18, 2010 HE TOOK THE VEHICLE BACK TO THE DEALER FOR INSPECTION AND THE DEALER STATED FOR THE SECOND TIME THAT NOTHING WAS WRONG WITH THE VEHICLE. IMMEDIATELY AFTER HE LEFT THE DEALER, HE DROVE INTO A SHOPPING CENTER PARKING LOT AND THE VEHICLE SUDDENLY ACCELERATED OVER A CURB AND COLLIDED INTO AN EMPTY STORE WINDOW. THE SAINT GEORGE POLICE DEPARTMENT INCIDENT NUMBER IS 10P004639 AND THE ACCIDENT NUMBER WAS 239-10. THE VEHICLE WAS REPLACED AFTER THE CRASH OCCURRED. I HAVE A 2009 TOYOTA TACOMA. I HAVE HAD 3 INSTANCES WHERE THE TRUCK WOULD HUNCH UP AT A STOP LIGHT LIKE SOMEONE WAS PRESSING ON THE GAS PEDAL AS I WAS ON THE BRAKE PEDAL. IN THE LAST CASE THE TRUCK WAS BEING DRIVEN OUT OF A CHURCH PARKING LOT AT A LOW SPEED AND THE VEHICLE TOOK OFF AND LOST CONTROL AND HIT A CAR. I PERSONALLY INSPECTED THE FLOORMAT ENTRAPMENT THE FLOORMAT WAS NOT EVEN CLOSE TO THE PEDAL. I HAVE BEEN A TECHNICIAN FOR 25 YRS AND BELIEVE IT IS IN THE CONTROLLER. I CONTACTED TOYOTA AND THEY WANT TO SEND SOMEONE OUT TO INSPECT IT. IT HAS NOT BEEN REPAIRED FOR ALL DAMAGE YET. LUCKILY NO ONE WAS HURT. THIS VEHICLE ONLY HAS 4,825 MILES ON IT. *TR
2009 TOYOTA TACOMA ACCELERATOR POLICE REPORT ON FILE WITH SIOUX CITY IOWA POLICE DEPT. I REARENDED A MINIVAN AT AN INTERSECTION WHERE SHE WAS STOPPED FOR A RED LIGHT DUE TO THE VEHICLE LUNGING AND FAILING TO STOP DESPITE ME STANDING ON THE BREAKS.....TOYOTA DEALERSHIPS ARE NOT FIXING THE PEDAL AS RECALL IS ORDERING SINCE THEY SAY IT IS NOT ON THE RECALL LIST. WE HAVE HAD THIS VEHICLE FOR 1 YEAR ON A 3 YR LEASE. MY HUSBAND DRIVES IT AS I FEEL UNSAFE IN IT AND IT HAS "ACTED UP" FOR HIM ON SEVERAL OCCASIONS. WE ARE GETTING NO ASSISTANCE FROM TOYOTA IN FIXING THIS DEFECT OR LETTING US OUT OF THE LEASE. IN ORDER FOR US TO "TRADE" IT FOR A FORD F150 WE HAVE TO EAT $4000 OF NEGATIVE EQUITY... I STRONGLY FEEL THAT WE ARE PUTTING OUR LIVES IN DANGER BY DRIVING THIS VEHICLE BUT HAVE NO CHOICE SINCE WE BOTH HAVE A 30 MILE COMMUTE IN OPPOSITE DIRECTIONS. WE WERE GOING TO PARK THE TOYOTA TACOMA TRUCK IN OUR GARAGE AND IT ALL OF A SUDDEN ACCELERATED ,INCREASING THE SPEED WHICH CAUSED US TO HIT A TREE AND THE NEIGHBORS CEMENT WALL. *CW
WHILE ATTEMPTING TO COME TO A STOP ON A HIGHWAY EXIT RAMP, WHEN I APPLIED FIRM PRESSURE TO THE BRAKE ABOUT HALF WAY DOWN THE RAMP, THE ENGINE ACCELERATED UNEXPECTEDLY AND THE REAR WHEELS ON MY TRUCK BEGAN TO SPIN AND SLIP. ALTHOUGH THE TRUCK WAS SLOWING DOWN IT COULD NOT COME TO A COMPLETE STOP AND STRUCK A STOPPED VEHICLE AT THE END OF THE RAMP AT SLOW SPEED. WITH EXTREME PRESSURE ON THE BRAKE THE TRUCK REMAINED STOPPED BUT THE REAR WHEELS CONTINUED TO SPIN AT A HIGH RATE. I SHIFTED TO NEUTRAL AND TURNED OFF THE IGNITION KEY. THE TRUCK IS AN AUTOMATIC AND WAS IN 2 WHEEL DRIVE AND THE AUTO SPEED CONTROL WAS NOT ON. I CHECKED THE FLOOR MATS BECAUSE I HAD PREVIOUSLY RECEIVED A SAFETY RECALL INTERIM NOTICE AND HAD PREVIOUSLY MADE SURE THE MATS WERE CORRECTLY INSTALLED. THE ALL WEATHER FLOOR MAT WAS CORRECTLY INSTALLED AND SECURED TO THE 2 FLOOR HOOKS, AND WAS NOT ON THE ACCELERATOR PEDAL. WHEN THE IGNITION WAS TURNED BACK ON THE TRUCK OPERATED NORMALLY AND HAS CONTINUED TO DO SO, NOW ABOUT 2 WEEKS. *TR
