The contact owned a 2011 Toyota Corolla. The contact stated that while he was driving at 40 MPH, another vehicle that was speeding caused a multiple vehicle collision. During the collision, the contact was in the middle lane when the speeding vehicle crashed into a vehicle in the left lane, and that vehicle merged violently into the middle lane, causing the contact to rear-end the vehicle in front due to the driver abruptly stopping. During the attempt to stop, the vehicle failed to decelerate sufficiently. Upon impact, the frontal air bags failed to fully deploy, as they only partially emerged from the housing. The failure experienced was a loss of braking effectiveness at 40 MPH that resulted in a rear-end collision, during which the frontal air bags failed to deploy. The vehicle was towed to a family member's home. The tire pressure warning was the only warning light on at the time of the crash. The driver was injured. Injuries sustained were lower back, right side ribs, and neck pain that required medical attention. The vehicle was deemed totaled by insurance. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 124,000.

THE CONTACT SOUGHT MEDICAL ATTENTION LATER THAT NIGHT IN THE EMERGENCY ROOM. THE DRIVER OF THE OTHER VEHICLE HAD A BLOODY NOSE AND IT WAS UNKNOWN IF HE SOUGHT MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE VEHICLE WAS UNABLE TO BE DRIVEN AND WAS TOWED TO A TOW LOT. THE VEHICLE WAS NOT DIAGNOSED AND WAS DESTROYED. A DEALER WAS NOT CONTACTED. THE CONTACT STATED THAT SHE RECEIVED AN UNKNOWN RECALL NOTICE REGARDING THE AIR BAGS IN 2015. ON OCTOBER 6, 2017, THE VEHICLE WAS SERVICED PER AN UNKNOWN AIR BAG RECALL. MY HUSBAND WAS HIT HEAD ON BY A CAR SPINNING OUT OF CONTROL ON THE HIGHWAY. HE WAS TRAVELING 50 TO 60 MPH WHEN THE CARS IN FRONT OF HIM SWERVED TO GET OUT OF THE WAY OF THE ONCOMING VEHICLE LEAVING HIM NO TIME TO REACT. HE HAD JUST STARTED BREAKING UPON IMPACT WHICH WAS FORCEFUL ENOUGH TO PUSH HIS CAR OFF THE HIGHWAY INTO THE GUARD RAIL. YOU CAN SEE FROM THE PICTURES THE ENTIRE FRONT END OF THE CAR WAS DAMAGED, THE BUMPER COMPLETELY TORN OFF THE CAR, AND THE ENGINE FORCED BACK FROM THE FRAME OF THE CAR. NONE OF THE AIRBAGS DEPLOYED. EVEN THOUGH HE WAS WEARING HIS SEAT BELT, MY HUSBAND HIT HIS HEAD ON THE TOP OF THE CAR NEAR THE WINDSHIELD AND SUSTAINED SIGNIFICANT WHIPLASH FROM THE IMPACT. WE CANNOT BELIEVE THAT THE AIRBAGS WERE DEPLOYED FROM AN IMPACT LIKE THIS. WE CHECKED AND THERE ARE NO RECALLS REGARDING AIRBAGS FOR THIS CAR. SOMETHING IS NOT RIGHT. THEY SHOULD HAVE GONE OFF AND PROTECTED MY HUSBAND DURING THIS COLLISION.

I BROKE MY NECK DUE TO A FAULTY AIR BAG SYSTEM.. I HAD TO HAVE CERVICAL NECK FUSION, I AM LUCKY TO BE ALIVE.. I HAVE ALL PROPER DOCUMENTS STATING THE ACCIDENT. NO CRIMINAL ACTION NEEDS TO BE EXCERSIZED, I JUST WANT TO HOLD WHO IS RESPONSIBLE FOR THE DEFECTIVE ITEMS WHICH HAVE CAUSED ME A DRAMATIC LIFE CHANGE.. EMOTIONALLY AND PHYSICALLY.. GROSS SAFETY IGNORANCE BY TOYOTA. THE VEHICLE LOST POWER AND THE DRIVER BEHIND ME STRUCK ME. PROBLEM STARTED THE NEXT DAY AFTER THE SALE TRANSACTION. THIS IS MY SECOND COMPLIANT WITH NHTSA. ALL EXHIBITS ON THE FILE. I ASKED OTHER DEALERSHIPS TO FIX THE VEHICLE; THEY REFUSED TO PERFORM NEITHER TEST NOR INSPECTION DUE TO MANUFACTURE DEFECTS. I WAS SENT BACK TO THE TOYOTA CASE MANGERS WHO IGNORES THE SAFETY AND REFUSES TO PROVIDE RESOLUTIONS. AUDIO RECORDING ON THE FILE. I NOTIFIED THE SERVICER AND THE MANUFACTURE REP WHICH WAS THE TOYOTA CASE MANAGER ABOUT THE ONGOING PROBLEM. I SENT CERTIFIED REGISTERED LETTERS. BOTHE, THE MANUFACTURE REP AND THE SERVICER GAVE VERBAL GUARANTEES WHICH WERE BROKEN. THEY ARE FLIP-FLOPPING, THEIR VERBAL GUARANTEES AND PROMISES WERE BROKEN OVER AND OVER. THE VEHICLE HAS ONGOING PROBLEMS OCCURRING DAILY BUT IT IS COMPLETELY RANDOM. THE MANUFACTURE REP REFUSED TO INSTALL A CHAT RECORDER AND MONITORS THE VEHICLE SYMPTOMS, HE FUSES TO PROVIDE TECHNICAL INFORMATION ON THE ALLOWABLE TEMPERATURE BREAKS ROTOR DISK; EXCESSIVE BREAK PADS DUST BUILDUP, THE SERVICER REFUSES FURTHER TEST THE VEHICLE. THE SHORT TEST DRIVE WHICH THE SERVICER USES TO TEST THE VEHICLE IS INSUFFICIENT. TESTS ARE DURING THE RUSH HOUR AND SHORT DRIVEN DISTANCES. I OFFER TO PAY FOR THE TECHNICIAN TIME AND DRIVE THE VEHICLE OUTSIDE THE RUSH HOUR AND LONGER DISTANCE BUT IT WAS REJECTED AS WELL. MY COMPLAINT TO NHTSA HASNT BROUGHT TOYOTAS ATTENTION EITHER.
THIS CAR WAS INVOLVED IN A THREE CAR COLLISION. A VAN TURNED LEFT SUDDENLY AND WITHOUT SIGNALING. THE CAR BEHIND THE VAN SLAMMED ON BRAKES. THE NEXT CAR HIT THE FIRST CAR IN THE BACK. THE THIRD CAR (MY CAR) HIT THE SECOND CAR IN THE BACK AND PUSHED HIM INTO THE FIRST CAR. MY DAUGHTER WAS DRIVING AND I WAS IN THE FRONT PASSENGER SEAT. THE FRONT AIR BAGS AND ALL FOUR OF THE SIDE CURTAIN AIR BAGS DEPLOYED. WE BOTH HAD ON OUR SEAT BELTS BUT THEY DID NOT LOCK. I, SEATING IN THE FRONT PASSENGER SEAT WITH MY SEAT BELT ON, WAS PROPELLED INTO THE FRONT WINDSHIELD. THE WINDSHIELD HAS A CIRCULAR PATTERN OF CRACKS WHERE I HIT IT WITH MY FACE. LUCKILY, MY FACE WAS ONLY CUT AND SEVERELY BRUISED WITH NO BROKEN BONES. I AM CONCERNED THAT THE SEAT BELTS DID NOT LOCK. NEITHER THE SEAT BELT NOR THE AIR BAGS PREVENTED ME FROM HITTING THE WINDSHIELD. MY DAUGHTER'S SEAT BELT ALSO DID NOT LOCK BUT THE AIR BAG PREVENTED HER FROM BEING SEVERELY INJURED.
I WAS APPROACHING A FOUR WAY STOP AT THE APPROPRIATE SPEED LIMIT OF 35MPH BEFORE THE IMPACT. AS I WAS GETTING CLOSER, I WENT AHEAD AND DECELERATED. I'VE HAD PROBLEMS WITH THE BRAKES IN THE PAST. AS I GOT CLOSER TO THE STOP SIGN, IN A BLINK OF AN EYE, I WAS BEING DRAGGED BY A LARGE CHEVY HEAVY DUTY TRUCK ACROSS THE INTERSECTION. THE AIRBAGS DID NOT DEPLOY EITHER. THE IMPACT COMPLETELY DISMANTLED MY FRONT END. AFTER THE PARAMEDICS AND FIRE FIGHTERS ARRIVE, I WAS RUSHED TO THE ER FOR REPORTED BACK AND NECK INJURIES. I WAS LOOKED OVER AND WAS DISCHARGED BARELY BEING ABLE TO WALK. THE CAR WAS TOWED TO A REPAIR SHOP QUITE A DISTANCE AWAY FROM MY HOME. MY INSURANCE SAID THE SHOP COULD BE TRUSTED. I ASKED OVER 20 PEOPLE AND EACH PERSON SAID THAT THE DAMAGE FROM THIS CAR HAS TO RESULT IN BEING TOTALED. THE MECHANIC SAID OTHERWISE. HE BEGAN WORKING ON THE CAR WITHOUT MY CONSENT AND DEMANDED; THREATENED MY INSURANCE COMPANY TO PAY HIM $4,000 FOR A FEW DAYS OF WORK AND $7,000 LATER ON. THIS WAS AFTER I HAD ALREADY SPOKEN WITH MY INSURANCE TO FILE THIS CAR AS BEING TOTALED. THE MECHANIC THREATENED FOR MONEY SAYING HE HAD ALREADY STARTED ON THE CAR WHEN HE NEVER EVEN TOUCHED IT AFTER 3 WEEKS. I USED TO TRUST TOYOTA VEHICLES, BUT AFTER THIS ACCIDENT I'M NEVER BUYING ANOTHER TOYOTA AGAIN. THE EMERGENCY RESPONSE TEAM SAID IT WAS A MIRACLE THAT I WAS ALIVE. ON APRIL 19TH, 2014, AT ABOUT 1:30 PM, MYSELF AND MY WIFE LEFT OUR FRIEND'S HOME IN LOS ALTOS HILLS, CA. AS WE LEFT THE DRIVEWAY AT ABOUT 5 MPH ON BAKER LN, THE CAR WENT INTO A SUDDEN, UNCONTROLLABLE ACCELERATION MODE, REACHING SPEEDS 50 - 60 MPH. WITHIN ABOUT 1000 - 1500 FT WE APPROACHED A THE INTERSECTION OF BAKER LN AND PURISSIMA RD., ALL THE WHILE DESPERATELY TRYING TO PRESS THE BRAKE, APPLYING HAND BRAKE AND SHIFTING TO NEUTRAL. AGAINST THIS SUDDEN BURST OF ACCELERATION, THE CAR WENT OFF THE ROAD AT THAT INTERSECTION, AND FELL INTO A DITCH AND STOPPED BY A TREE TRUNK. BOTH OF US WERE INJURED WITH MULTIPLE INTERNAL/ EXTERNAL FRACTURES, BUT WE WERE CONSCIOUS; MY WIFE IN THE PASSENGER SEAT HAD MORE GRIEVOUS INJURIES, INCLUDING AN OPEN WOUND OF BROKEN BONE IN ANKLE AND SEVERE SPINAL CORD INJURIES, AND WAS SCREAMING WITH PAIN. ONCE I COLLECTED MYSELF, I CALLED 911 AND THE FD/ PARAMEDICS RESPONDED AND WE WERE HAULED FROM THE DITCH BY CUTTING OPEN THE CAR DOORS. WE WERE TRANSPORTED TO STANFORD MEDICAL CENTER IN PALO ALTO, AFTER WHICH WE WERE SHIFTED TO OUR HMO, AFTER ABOUT 10 DAYS IN ICU. I AM NOW DISCHARGED AND REHABILITATING/ GETTING PHYSICAL THERAPY AT HOME, WHILE MY WIFE IS STILL AT A REHAB CENTER, OVER 10 WEEKS AFTER THE ACCIDENT. AT THE TIME OF THE ACCIDENT, THE WEATHER, VISIBILITY AND ROAD CONDITIONS WERE EXCELLENT; BOTH OF US HAD OUR SEAT BELTS FIRMLY IN PLACE; NO CELL PHONE WAS BEING USED; I HAD MY FOOT ON THE BRAKE AND OFF THE ACCELERATOR ALL THE TIME; AIR BAGS DEPLOYED AS EXPECTED. SINCE BOTH OF US WERE CRITICALLY INJURED, WE COULD NOT INITIATE/ PURSUE ANY LEGAL ACTION AGAINST TOYOTA AND SURRENDERED THE VEHICLE (WHICH WAS TOTALED) TO MY INSURER, FARMERS. MY ENTIRE FRONT END WAS CRUSHED, RADIATOR AND TRANSMISSION BUSTED, AND FRONT BUMPER PULLED OFF, AND INSIDE CAR UNDER STEERING WHEEL HAD BEEN SLIGHTLY PUSHED OUT TOWARDS DRIVER SEAT. MY CAR WAS TOTALED. I BUSTED THE WINDSHIELD WITH MY HEAD WHEN I HIT IT GIVING ME A CONCUSSION AND HAD CONTUSIONS TO MY CHEST FROM HITTING STEERING WHEEL, AND CONTUSION AND SPRAIN TO MY RIGHT HAND. NO ONE INCLUDING POLICE, FIREMEN, AMBULANCE, AND WRECKING YARD COULD BELIEVE MY AIR BAGS DID NOT DEPLOY. MY HUSBAND AND I CONTACTED TOYOTA ABOUT THIS AND THEY ASSURED ME IT SHOULD NOT HAVE DEPLOYED AND SENT ME AN EMAIL LINK TO READ DESCRIBING WHEN AIR BAGS SHOULD DEPLOY. WHEN I CALLED BACK AFTER READING THE EMAIL AND TOLD THE MAN WHAT THE EMAIL SAID AND THAT MY AIR BAG SHOULD HAVE DEPLOYED HE CALLED ME A LIAR, AND SAID THAT WAS NOT WHAT THE EMAIL SAID. MY HUSBAND THEN CALLED AND REQUESTED INFORMATION FROM EDR BE DOWNLOADED AND READ. TOYOTA NEVER RETURNED OUR PHONE CALL AND NEVER RETRIEVED INFORMATION FROM EDR, AND NOW INSURANCE HAS TAKEN POSSESSION OF THE VEHICLE AND IT IS GONE. A MONTH LATER WE STILL HAVE NEVER RECEIVED A RETURN PHONE CALL OR EXPLANATION FROM TOYOTA. *TR
I CAME TO AN INTERSECTION AND STOPPED, THEN TOOK MY FOOT OFF THE BRAKE BEFORE NOTICING THAT THE LIGHT WAS STILL RED. THE OTHER VEHICLE WAS ROUGHLY 30 FEET AWAY WHEN I PUSHED ON THE BRAKES. I SHOULD HAVE EASILY BEEN ABLE TO STOP. BRAKES DID NOT ENGAGE AND I HAD BEEN PUSHING THEM SO HARD AND FOR SO LONG THAT I FRACTURED MY ANKLE. THERE WERE NOT EVEN SKID MARKS AT THE ACCIDENT SCENE. MY CAR HAD FRONT END FRAME DAMAGE FROM THE ACCIDENT (TOTALED), AND THE AIRBAGS NEVER DEPLOYED. ADDITIONALLY, IN THE PAST I HAD NOTICED THE RPMS RISING FOR NO REASON WHILE CAR WAS IDLING, AND THE ACCELERATION ISSUE, WHERE THE VEHICLE SEEMED TO ?HANG? BETWEEN SPEEDS OF ABOUT 35 AND 40 AND 60 AND 65MPH. THE RPMS I ONLY NOTICED ONCE-SO I DID NOT REPORT IT, AND THE ?HANGING? WAS MY OWN IGNORANCE (I HAVEN?T HAD A 4 CYLINDER IN A LONG TIME AND THIS WAS MY FIRST?AND PROBABLY LAST?TOYOTA). I ALSO HAD PRIOR BRAKING CONCERNS-- ABS ONLY MADE A NOISE (DID NOT VIBRATE AGAINST PEDAL--AGAIN, MY IGNORANCE OF HOW THIS VEHICLE WAS SUPPOSED TO RESPOND) AND IT WAS A VERY ICY WINTER, SO I ASSUMED THAT I HAD BEEN TRYING TO STOP ON ICE. THE BRAKES BEHAVED FINE IF I PHYSICALLY PUMPED THEM, BUT SUDDEN STOPS COULD NOT BE MADE. *TR

THE DRIVER SUSTAINED A BRUISE TO THE CHEST, INJURY TO THE NECK AND HEAD. THE VEHICLE WAS COMPLETELY DESTROYED AND TOWED TO A COLLISION CENTER. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. I PULLED OUT AT A GREEN LIGHT AND A PICKUP TRUCK RAN A RED LIGHT HIT MY CAR IN DRIVER DOOR AT 50MPH AND NONE OF MY AIRBAGS DEPLOYED ME AND MY WIFE BOTH HAD INJURIES. *TR
ON MAY 04, 2013, MY MOM WAS INVOLVED IN A BAD TRAFFIC ACCIDENT WITH HER 2011 TOYOTA COROLLA WHERE THE DRIVER'S SIDE AIRBAG DID NOT DEPLOY. THE INSURANCE ADJUSTOR WROTE IT UP AS A TOTAL LOSS AND ADVISED ME TO CONTACT TOYOTA TO INQUIRE ABOUT THE AIRBAGS NOT DEPLOYING. MY MOM IS CURRENTLY RECOVERING FROM TWO MAJOR SURGERIES FROM THIS ACCIDENT. THE FIRST SURGERY THEY HAD TO REMOVE HER LEFT KIDNEY DUE TO INTERNAL BLEEDING. THE SECOND SURGERY WAS 41/2 HOURS LONG REPAIRING HER BADLY BROKEN RIGHT ARM. .ALL PARTIES INVOLVED, AT THE SCENE OF THE ACCIDENT, TO THE MEDICAL STAFF AT THE HOSPITAL AND THE TOWING COMPANY ALL ADVISED ME TO CONTACT TOYOTA AND INQUIRE ABOUT THE AIRBAGS NOT DEPLOYING. THE DRIVER PARKED HER CAR IN A BANK PARKING LOT TO WRITE OUT A CHECK AND WANTED TO CASH IT USING THE BANK DRIVE THRU. SHE BACKED OUT OF THE PARKING SPOT, MISJUDGING THE PARKING MEDIUM / ISLAND ON HER RIGHT HAND SIDE AND STARTED TO MOVE FORWARD MAKING A RIGHT HAND TURN. AT THE TIME, HER CAR GOT STUCK ON TOP OF THE MEDIUM. AFTER SEVERAL ATTEMPTS TO FREE HER CAR BY PRESSING THE GAS PEDAL DOWN, THE DRIVER LOST CONTROL OF HER VEHICLE AFTER FINALLY BREAKING FREE FROM THE MEDIUM. THE DRIVER DROVE UP AND OVER A PARKING LOT CURB AND ONTO THE GRASS, SCRAPING A BANK SIGN, THROUGH A BLACK IRON FENCE, OFF AN (5FT. TO 6FT.) EMBANKMENT WALL AND INTO A LARGE WOOD UTILITY POLE. THIS CRASH CAUSED HER LAUNCH FROM THE DRIVER'S SEAT AND HEAD FIRST INTO THE PASSENGER SIDE WINDOW, SHE INJURED THE TOP OF HER HEAD WHERE SHE NEEDED SEVERAL STITCHES. *TR
WHILE TRAVELING ON A HIGHWAY, A VEHICLE STRUCK THE TOYOTA COROLLA AUTOMOBILE ON THE FRONT, PASSENGER SIDE. THIS COLLISION CAUSED THE COROLLA TO THEN STRIKE A MEDIAN WALL. AFTER THE SECOND IMPACT, THE COROLLA FLIPPED AT LEAST TWO (2) TIMES. THE AIRBAG NEVER DEPLOYED. THE ENTIRE FRONT SIDE WAS DAMAGED IN THIS ACCIDENT. *TR
