ON OR ABOUT OCTOBER 31, 2008, A 2005 TOYOTA SIENNA XLE ACCELERATED SUDDENLY AND UNEXPECTEDLY. THE DRIVER STEERED THE VEHICLE AROUND CARS, CURBS AND OTHER OBSTRUCTIONS WHILE THE VEHICLE CONTINUED TO ACCELERATE THROUGH A PARKING LOT. ATTEMPTS TO BRAKE THE VEHICLE WERE FUTILE. THE VEHICLE EVENTUALLY HIT A CURB WHICH DEFLATED A TIRE, THEN CONTINUED THROUGH A CHAIN-LINK FENCE AND A FIELD AND INTO A PORTABLE BUILDING, FINALLY COMING TO A STOP. THE DRIVER WAS INJURED IN THE UNINTENDED ACCELERATION INCIDENT AND DIED. UPDATED 02/26/10 *BF *TR UPDATED 09/28/10 UPDATE 9/19/2011*CW
I WAS COMPLETELY STOPPED IN MY SIENNA AWD AT A TRAFFIC LIGHT AT THE TOP OF A FREEWAY ON-RAMP IN A MAJOR CITY, AND REAR-ENDED BY A VEHICLE BEGINNING TO TRAVEL DOWN THE ON-RAMP AT AN UNKNOWN BUT HIGH RATE OF SPEED AND NOT HEEDING THE TRAFFIC SIGNAL. ON IMPACT, THE REAR CARGO COMPARTMENT OF MY SIENNA CRUSHED INWARD SO FAR THAT IT CAUSED THE THIRD ROW BENCH SEAT TO MOVE FORWARD AND PROPEL THE SEAT PART OF THE BENCH SEAT INTO THE BACKS OF THE SECOND ROW CAPTAIN'S CHAIRS IN FRONT OF IT. THIS WOULD HAVE CRUSHED THE LEGS AND MAYBE WHOLE BODY OF ANYONE SEATED IN THE OUTER TWO SEATS OF THE REAR BENCH SEAT, AND PERHAPS IN THE MIDDLE POSITION AS WELL. I FOUND A PIECE OF PAPER BAG CAUGHT BETWEEN THE BOTTOM SEAT EDGE OF THE BENCH SEAT AND THE BACK OF A CAPTAIN'S CHAIR, AND I COULD NOT PULL THAT PIECE OF PAPER FREE. I HAD TO PULL REALLY HARD TO REMOVE THE LARGE PACK OF TOILET TISSUE, SHOWN IN THE UPLOADED PHOTO, IN ORDER TO DISLODGE IT FROM THE REAR BENCH SEAT AREA. THE TISSUE PACK LIKELY IS NARROWER THAN THE TRUNK OF AN ADULT HUMAN BODY WHICH COULD HAVE BEEN CAUGHT THERE, WITH LEGS ALSO CRUSHED BY THE LOWER PORTION OF THE BENCH SEAT AS IT IMPACTED THE BACK OF THE CAPTAIN'S CHAIR IN FRONT OF IT. I BELIEVE THE CAUSE OF THIS SAFETY PROBLEM IS A POOR DESIGN; INADEQUATE REAR-CRASH PROTECTION FOR THIRD-ROW PASSENGERS, WHO USUALLY ARE CHILDREN. AS A PARENT AND VEHICLE OWNER, I WAS NEVER WARNED ABOUT THE GREAT DANGER MY CHILDREN COULD BE IN IF THEY SEATED ANYWHERE ALONG THE THIRD-ROW BENCH SEAT DURING A REAR-END COLLISION. I'M TRAUMATIZED BY THE THOUGHT OF A CHILD OR ANYONE HAVING BEEN SEATED IN THE THIRD ROW AT THE TIME OF THIS COLLISION, WHICH FORTUNATELY THEY WERE NOT IN THIS INSTANCE.

THE CONTACT STATED THAT THE WHILE THE VEHICLE WAS PARKED BUT UNSURE IF THE EMERGENCY PARKING BRAKE WAS ENGAGED, THE VEHICLE STATED ROLLING DOWN HER DRIVEWAY WHICH HAD A SMALL INCLINE. THE CONTACT STATED THAT HER 4 YEAR OLD CHILD STATED, "THAT HE SHIFTED THE GEAR SHIFT" AND THE VEHICLE STARTED ROLLING BACKWARDS. THE CONTACT STATED THAT THE KEY WAS NOT IN THE IGNITION AND NO WARNING LIGHTS WERE ILLUMINATED. THE CONTACT STATED THAT SHE ATTEMPTED TO DEPRESS THE EMERGENCY BRAKE PEDAL WHILE SLOWLY RUNNING WITH THE VEHICLE BUT FAILED TO STOP THE VEHICLE. THE CONTACT STATED WAS ABLE TO TURN THE STEERING WHEEL TO THE LEFT HITTING TWO BOULDERS AND A FENCE, THEN COMING TO A STOP. THE CONTACT STATED SUSTAINED BRUISING TO HER LEGS AND BACK INJURIES WHICH REQUIRED MEDICAL ATTENTION 3 DAYS LATER. A POLICE REPORT WAS NOT FILED. THE VEHICLE WAS DRIVEN BACK TO THE CONTACT'S HOME AND TOWED A WEEK LATER TO TOYOTA OF POWAY LOCATED AT 13631 POWAY RD, POWAY, CA 92064(858) 486-2900. THE CONTACT MENTIONED AFTER THE FAILURE, SHE RESEARCHED AND FOUND NHTSA CAMPAIGN NUMBER: 13V429000 (POWER TRAIN).THE MANUFACTURER WAS NOTIFIED OF THE FAILURE AND A CASE WAS OPENED AS WELL AS A TOYOTA FIELD TECHNICAL SPECIALIST WAS SENT TO THE DEALER TO DETERMINE THE FAILURE. THE CONTACT STATED THAT SHE WAS INFORMED BY THE MANUFACTURER THAT "NO ABNORMALITIES WERE FOUND WITH THE GEAR LEVER AND SHIFT CABLE". THE FAILURE MILEAGE WAS APPROXIMATELY 250,000.
HUSBAND PULLED INTO PARKING SPOT IN FRONT OF A STORE, HE WAS COMPLETELY STOPPED IN THE SPOT AND BEFORE HE COULD PUT IN PARK IT SUDDENLY ACCELERATED FULL REV, HOPPED THE CURB AND HIT THE BUILDING. WHILE UP AGAINST THE BUILDING IT WAS STILL IN FULL REV. FINALLY IT STOPPED AND HE BACKED IT BACK OVER THE CURB. VAN WENT ABOUT THREE FEET INTO THE BUILDING. TIRE ON FRONT PASSENGER SIDE WAS FLATTENED. VAN HAS BEEN TOTALED. THANK GOD NO ONE WAS IN FRONT OF OUR VAN.
ON MONDAY, OCT. 13, 2014, I WAS SLOWLY BACKING OUT OF MY DRIVEWAY WHEN MY 2005 TOYOTA SIENNA VAN SUDDENLY ACCELERATED RAPIDLY AND UNCONTROLLABLY. I TRIED TO STOP THE VAN, BUT IT SPED BACKWARDS ACROSS THE STREET IN FRONT OF MY HOUSE, RAN OVER A CURB, AND CRASHED INTO A PARKED CAR. THE CAR THAT MY VAN STRUCK PUSHED INTO ANOTHER CAR WHICH WAS ALSO DAMAGED. THIS WAS MOST FRIGHTENING AND UPSETTING, ESPECIALLY SINCE I HAVE LEARNED THAT THIS TYPE OF MALFUNCTION HAS BEEN REPORTED MANY TIMES TO TOYOTA. MY VAN WAS TOTALED, THE SECOND CAR MAY HAVE BEEN TOTALED AND THE THIRD CAR WILL NEED AT LEAST A NEW DOOR. A WOMAN WHO WAS IN THE THIRD CAR IS CLAIMING TO HAVE BEEN INJURED. *TR
TRIED TO AVOID ONCOMING TRUCK OVER DOUBLE LINE BY EASING TO R SHOULDER AND CAUGHT ICE. SLIDE, DRIVER SIDE TIRES WENT UP ON GUARDRAIL, WENT AIRBORNE TAKING OUT A MAILBOX, BOUNCED IN A LOWER FRONT YARD WENT BACK IN AIR. PINGED OFF LARGE TREES X3 THEN PASSED THRU 2 FIELD GOAL AND THE TREE DEAD CENTER AND MY VAN HIT HARD! CRUMPLING MY FRONT END BUT NOT DEPLOYING ANY AIR BAGS. NICE SEATBELT BRUISE AND SHAKEN BUT NO INJURIES. *TR
TL* THE CONTACT OWNED A 2005 TOYOTA SIENNA. THE CONTACT STATED THAT SHE WAS CRASHED INTO FROM THE REAR BY A VEHICLE DRIVING 50 MPH. THE CONTACT HAD BEEN BRAKING TO A STOP. THE AIR BAGS DID NOT DEPLOY DURING THE CRASH. THE CONTACT SUSTAINED CHEST AND NECK INJURIES DURING THE CRASH. THE VEHICLE WAS DESTROYED AND TOWED TO A SALVAGE FACILITY. THE FAILURE WAS NOT DIAGNOSED. ATTORNEY WRITES ON BEHALF OF CLIENTS IN REGARDS TO A COLLISION DUE TO UNINTENDED ACCELERATION *TGW AS THE CONSUMER WAS SLOWLY ATTEMPTING TO PARK THE VEHICLE, IT SUDDENLY AND UNEXPECTEDLY ACCELERATED OUT OF CONTROL. THE VEHICLE HIT A POLE DIRECTLY AHEAD, WENT AIRBORNE AND LANDED ON TOP OF A PARKED VEHICLE AND ROLLED TO THE SIDE. THE CONSUMERS LEFT HAND FINGERS WERE CRUSHED CAUSING INJURIES. *JB
2005 TOYOTA SIENNA WAS REAR-ENDED ON 6/10/10. PASSENGER SEAT BELT DID NOT RESTRAIN THE PASSENGER. PASSENGER'S BODY WAS THROWN VERY NEAR THE DASH. RESULTED IN DAMAGE TO STERNUM AND NECK. WE ARE REQUESTING TOYOTA REPLACE THE PASSENGER BELT. WE ARE ENCOUNTERING RESISTANCE FROM TOYOTA TO REPAIR THIS SAFETY DEFECT. DRIVER'S SEAT BELT DID RESTRAIN DRIVER. *TR
TL*THE CONTACT OWNS A 2005 TOYOTA SIENNA. WHILE DRIVING 60 MPH IN HEAVY RAIN, THE CONTACT ATTEMPTED TO CHANGE LANES TO EXIT THE HIGHWAY. SHE LOST CONTROL OF HER VEHICLE AND CRASHED INTO A TRACTOR TRAILER. THE VEHICLE SPUN OFF OF THE TRACTOR AND TRAVELED BACK TO THE LEFT OF THE HIGHWAY. THE AIR BAGS FAILED TO DEPLOY AND THE VEHICLE WAS DESTROYED. THE POLICE AND EMS WERE CALLED TO THE SCENE, AND THE CONTACT WAS THE ONLY PERSON TRANSPORTED TO A LOCAL AREA HOSPITAL FOR MINOR TO MODERATE CUTS, BRUISES, NECK, AND BACK PAIN. A POLICE REPORT WAS FILED. THE FAILURE MILEAGE WAS 61,000.
I PULLED INTO A PARKING LOT AND MADE A RIGHT TURN TO PULL INTO A PARKING SPACE. I WAS HALFWAY INTO THE SPACE, MY FOOT WAS ON THE BRAKE PEDAL, WHEN I FELT THE VAN JERK-IT THEN SUDDENLY ACCELERATED, WENT OVER THE RAISED CURB IN FRONT,ONTO THE EXIT LANE OF THE SHOPPING CENTER,TURNED AROUND BACK INTO THE PARKING LOT(THE WHEEL WAS TURNED TO THE RIGHT STILL),HIT THE BUILDING BREAKING A COUPLE OF WINDOWS, AND THEN CAME TO A STOP AFTER HITTING A BARRICADED DUMPSTER ALONG THE WAY. ALL THE WHILE I WAS HITTING THE BRAKES, BUT THEY DIDN'T SEEM TO WORK. THE AIRBAG DEPLOYED IN THE PROCESS,THE FRONT OF THE VAN WAS BADLY DAMAGED. THANKFULLY THERE WERE NO PEDESTRIANS IN THE LOT AND I DIDN'T HIT ANY OTHER VEHICLE AND MY SON AND I WALKED AWAY WITH MINOR INJURIES. I HAVE CONTACTED TOYOTA. THEY INSPECTED THE VAN LAST WEEK AND I AM WAITING TO HEAR THEIR REPORT. THE VAN HASN'T BEEN FIXED YET-I AM WAITING TO HEAR FROM TOYOTA AS TO THE NEXT COURSE OF ACTION. ABOUT AN HOUR BEFORE THIS INCIDENT OCCURRED, I WAS PULLING OUT OF ANOTHER PARKING SPACE IN REVERSE,WHEN THE VAN JUST STOPPED AND DIDN'T MOVE. I RESTARTED IT,PUT IT ON REVERSE AND THEN IT MOVED ALRIGHT,WAS BUMPY-I THOUGHT A TIRE WAS FLAT-STOPPED AND CHECKED TO SEE ALL OF THEM-THEY WERE FINE, AND AFTER THAT ALL WAS SMOOTH AND WELL UNTIL THE CRASH. *JB

VEHICLE WAS TURNING INTO AN INTERSECTION GOING LEFT. AN ON COMING VEHICLE GOING ABOUT 45 MPH AND HIT VEHICLE "A" ON THE DRIVER SIDE FRONT QUARTER PANEL. THE AIR BAGS FAILED TO DEPLOY. VEHICLE WAS TOWED BY THE LOCAL POLICE DEPARTMENT. PROVIDE FURTHER DETAILS. *NM
VAN WAS "T-BONED" ON PASSENGER SIDE BY LARGE PICKUP TRUCK PROBABLY GOING 45 MPH OR SO. TRUCK HIT FRONT SIDE PANEL AND PASSENGER DOOR. ESTIMATED DAMAGE AT THIS POINT OVER $14,000. PASSENGER HAD TO BE EXTRICATED BY JAWS OF LIFE AND SUFFERED FRACTURED PELVIS AND FACE LACERATIONS. NEITHER THE SIDE CURTAIN AIR BAG NOR SIDE SEAT AIR BAG WENT OFF. I CONTACTED TOYOTA YESTERDAY AND THEY SAID THEY WOULD CONTACT TO ME WITHIN 3 BUSINESS. I THINK THERE IS A PROBLEM WHETHER THEY SAY (A) IT SHOULD HAVE GONE OFF AND DIDN'T OR (B) THAT IT SHOULD NOT HAVE GONE OFF WITH THIS TYPE OF CRASH. VAN HAS NOT YET BEEN REPAIRED. ONE ISSUE IS WHETHER I SHOULD WAIT TO HAVE IT REPAIRED. DIGITAL PICTURES OF VAN ARE AVAILABLE. *NM

WHILE REVERSING OUT OF A PARKING SPOT, FELT THE VAN "JUMPED" THEN ACCELERATED ON IT'S OWN WHILE IN REVERSE SWIPED A PARKED TRUCK, AND DID A HALF CIRCLE INTO THE DRIVE-THROUGH OF A PARKING LOT ONLY TO STOPPED WHEN IT HIT ANOTHER SUV. I FELT I HAD NO CONTROL OF THE VAN WHILE THIS WAS HAPPENING.
ABS BRAKE PUMP MODULE. WENT OUT CAUSING ACCIDENT. NO WARNING. ABS LIGHTS CAME ON. TOOK TO SHOP. DRAINING BATTERY,CAUSING BRAKES TO FAIL. SHOUOD BE RECALLED DUE TO NO WARNING,COMPLETE BRAKE FAILURE,CAUSING VEHICLE TO BE DANGEROUS, NOT ROAD WORTHY.

THE VEHICLE WAS NOT DIAGNOSED OR REPAIRED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 216,000.
