WHEN THE ENGINE WAS STARTED WITH THE GEAR IN PARK AND THE PASSENGER SIDE SLIDING DOOR OPEN, THE VEHICLE SUDDENLY DROVE FORWARD AND CRASHED INTO A CONCRETE PILLAR. WHILE ATTEMPTING TO STOP THE VEHICLE, THE CONTACT BECAME TRAPPED BETWEEN THE VEHICLE AND THE CONCRETE PILLAR. THE CONTACT SUSTAINED INJURIES TO THE HEAD, LEFT HIP, LEGS, TORSO, AND BOTH ARMS. MEDICAL ATTENTION WAS NOT RECEIVED. THE CAUSE OF THE FAILURE WAS NOT DETERMINED. THE LOCAL DEALER AND MANUFACTURER WERE NOT NOTIFIED OF THE FAILURE. THE CAUSE OF THE FAILURE WAS NOT DETERMINED. THE FAILURE MILEAGE WAS 150,000.
MINIVAN PARKED BUT ROLL AWAY CAUSED INJURY. THE MINIVAN WAS PARKED TO PICK UP GROCERY BAGS BUT STARTED ROLLING AWAY AFTER THE LAST 2BAGS OF GROCEY

THE AUTONATION TOYOTA CERRITOS DEALER (18700 STUDEBAKER RD, CERRITOS, CA) WAS NOTIFIED OF THE FAILURE AND WERE CURRENTLY REPAIRING THE VEHICLE. THE MANUFACTURER WAS NOT NOTIFIED. THE APPROXIMATE FAILURE MILEAGE WAS 97,000.
I PUT MY VEHICLE IN PARK AND THEN TURNED ON THE HAZARD LIGHTS, AS I WAS PARKED AT THE ENTRANCE TO A BUSY HIGHWAY, AND JUST WANTED TO GRAB MY MAIL. I TURNED AROUND FROM GETTING THE MAIL; AND MY MINIVAN WAS ROLLING ACROSS THE HIGHWAY WITH THREE OF MY CHILDREN INSIDE OF IT. IT STOPPED WHEN IT HIT A CEMENT BARRIER ON THE OTHER SIDE OF THE HIGHWAY. ON JUNE 15, 2016 AROUND 4:30 MY WIFE WAS DRIVING TOYOTA SEINA LTD VAN WHICH IS EQUIPPED WITH SIDE CURTAIN AIRBAG ANOTHER VEHICLE HIT HER VAN ON PASSENGER SIDE FRONT WHEEL FROM SIDE WHILE CHANGING LANES THIS RESULTED TIPPING OF VAN , SEINNA VAN WAS TIPPED ON SIDE AND WAS DRAG ON ROAD OVER 100 FEET BEFORE IT STOPPED . WINDOWS AND FRONT GLASS SHATTERED WHILE SLIDING THERE IS EXTENSIVE DAMAGE ON CAR ON PASSENGER SIDE AND DRIVER SIDE AS WELL AS IN FRONT NEAR PASSENGER SIDE NONE OF THE AIRBAGS WERE INFLATED DESPITE SIGNIFICANT IMPACT

I WAS TRYING TO PARK MY VAN INSIDE A CAMPSITE, DRIVING AT APPROX 1.2 MPH WHEN MY VAN TOOK OFF LIKE A ROCKET. I COULDN'T STOP THE CAR EVEN BY BRAKING. LUCKILY MY VAN WENT INTO A DITCH, AIRBAGS OPENED UP, VAN WENT DOWN THE DITCH AND CAME UP AT AN ANGLE AND STOPPED. I WAS BADLY HURT AND PASSED OUT FOR FEW MINUTES, TAKEN TO A LOCAL HOSPITAL EMERGENCY ROOM. THE 16 PAGE REPORT ORDERED BY TOYOTA THRU' A 3RD PARTY CLEARLY SHOWED THAT THE CAR WENT FROM 1.2 MPH TO 31.5 MPH, ENGINE RPM WENT FROM 800 TO 4800 RPM IN 4.2 SECS. I CHECKED WITH A REPUTABLE AUTO MECHANIC. HE COMMENTED THAT NO ONE CAN MANUALLY RAMP THE ENGINE RPM TO 4800 RPM. IT IS CLEARLY AN ISSUE WITH TOYOTA'S ELECTRONIC SYSTEM. TOYOTA HAS DENIED ANY ISSUE WITH THEIR CAR IN A LETTER TO ME.I WANT THE NATIONAL HIGHWAY TRANSPORTATION SAFETY ADMIN TO TAKE MY CASE AGAINST TOYOTA. THE APPROXIMATE FAILURE MILEAGE WAS 20,400.
OCTOBER 2010 I PURCHASED A TOYOTA SIENNA 2011. 3 MONTHS LATER I DROVE BACK FROM WORK AND I HAVE MY 4 CHILDREN WITH ME IN THE CAR WHERE THE CAR BRAKES FAILED TO RESPOND AS WELL THE EMERGENCY BRAKE!!!! WAS THE WORST DAY IN MY LIFE!!!! I CRASHED INTO A TRUCK AND WE WERE ALL IN SHOCKED TO THIS DAY!!!!...AFTER THE CRASH I HAD RECEIVED A RECALL IN THE TOYOTA SIENNA 2011 FOR THE BRAKES? I AM WITHOUT CAR RESPONSIBLE FOR THE DAMAGES FOR THE OTHER CARS, IN PAIN, MY KIDS SUFFER SMALL INJURIES AND FOR A AUTO LOAN IN TOYOTA CRASHED. I HAD MADE CALLS BUT UNFORTUNATELY NO ONE WILL GIVE ME A ANSWER OR STAND AND FACE THEIR RESPONSIBILITIES... I was pulling my 2011 Toyota Siena into a parking space. My foot was on the brake and the car was going about 5 mph. The car made a slight sound, along with a perceptible yet small physical shift, and (while my foot was on the brake) independently accelerated with tremendous force. It was like it was ignited. The car hit a concrete pole and was totaled. The acceleration over-rode the brake. No one was hurt. The concrete pole averted what could have been a catastrophic accident, had we hit pedestrians, or had we rear ended a car or truck on the freeway. The brakes failed as the car jumped into independent acceleration. My safety and the safety of my passenger were both at risk. As I said, in a different situation, we might have hit pedestrians, or gone through the wall of the grocery store and hit customers. The concrete pole saved us and others. The mechanic at the service center where the car was towed said it was impossible to discern if there had been a failure in the system of the car - though clearly both my passenger and myself experienced that failure in the car's ability to override an independent, forceful acceleration not caused by the driver. There was no warning of any kind. On the Edmunds site, there are many, many testimonials by other drivers of the Toyota Siena of similar, frightening experiences of independent acceleration. It is terribly dangerous and should be investigated. If there is not a recall on this vehicle, there should be. This is a potentially catastrophic defect.
CHECK AIR BAGS
ON A SNOWY SATURDAY MORNING, MY HUSBAND WAS SLOWLY (DRIVING LESS THAN 5 MPH) PULLING INTO A PARKING SPOT WHILE BRAKING. AS HE SHIFTED INTO PARK, THE VEHICLE SUDDENLY REVVED AND ACCELERATED OVER THE FIVE INCH CURB AND HIT A BUILDING A FEW FEET IN FRONT OF THE CAR. HIS FOOT REMAINED FIRMLY ON THE BRAKE DURING THE INCIDENT. ALTHOUGH THE VEHICLE HAD COME TO A COMPLETE STOP ONCE IT HIT THE BUILDING AND WAS PUT INTO PARK, THE ENGINE CONTINUED TO MAKE A LOUD REVVING SOUND. THE REVVING DID NOT STOP UNTIL THE VEHICLE WAS TURNED OFF.
I WAS DRIVING MY TOYOTA SIENNA 2011(US 22W ALLENTOWN PA HIGHWAY) WHEN SUDDENLY MY VEHICLE WAS HIT HARDLY BY A CHEVY SUV, THEN I HIT LATERALLY THE WALL DIVIDING THE HIGHWAY, LATELY HIT A ALUMINIUM FENCE. I WAS ADMITTED AT LEHIGH VALLEY HEALTH NETWORK, CEDAR CREST HOSPITAL WITH HITS ALL OVER MY BODY, ESPECIALLY, AN OPEN-WOUND TRAUMA IN MY FOREHEAD. I STARTED HAVING RECURRENT HEADACHES, PAINFUL BACK AND NECK ACCOMPANY OF SWELLING, NUMBNESS AND CRAMPS IN MY RIGHT LIMB. I WAS ATTENDING PHYSICAL THERAPIES, STILL I AM TAKING MEDICATIONS BY RELIEF RECURRENT MIGRAINES, BACK AND NECK PAIN PROVOKED BY THE WHIPLASH SYNDROME LONG-TERM INJURIES. WHEN CHECKING ON THE VEHICLE WAS VERY NOTICEABLE THAT NONE OF AIR-BAGS WAS OPENED. BECAUSE OF THIS SITUATION MY INJURIES WERE WORST.
