ON 9/18/99, MY FATHER, HUSBAND, SON AND I WERE INVOLVED IN A ROLLOVER ACCIDENT ON I-10 BECAUSE OF THE TREAD SEPARATION ON THE LEFT REAR TIRE. AS A RESULT OF THE ACCIDENT, MY FATHER WAS TAKEN BY HELICOPTER TO A LOCAL HOSPITAL WHERE HE DIED 2 DAYS LATER AS A RESULT OF HIS INJURIES. MY SON WAS NOT HARMED, MY HUSBAND SUFFERED MINOR INJURIES AND WAS TRANSPORTED BY AMBULANCE TO THE SAME LOCAL HOSPITAL, AND I HAD VERY MINOR CUTS AND BRUISES.( DOT NUMBER: )
LOW SPED IMPACT TO LEFT REAR CAUSED ROLLOVER. NE-AND ONE-HALF ROTATIONS, COMING TO REST ON ITS ROOF. PASSENGER'S SIDE WINDOW SHATTERED, CAUSING PARTIAL EJECTION OF 11-YEAR -OLD PASSENGER, CRUSHING HIS SKULL AND KILLING CHILD. TWO OTHER PASSENGERS WERE INJURED. *AK

1995 FORD EXPLORER. CONSUMER STATES SEAT BACK FAILURE IN REAR -END COLLISION *TGW THE CONSUMER STATED WHILE SITTING AT A RED LIGHT, HE WAS REAR ENDED BY ANOTHER VEHICLE TRAVELING AT ABOUT 30 MPH. UPON IMPACT HIS VEHICLE WAS PROPELLED INTO THE VEHICLE IN FRONT OF HIM. THE CONSUMER STATED HE WAS THROWN BACKWARD, AFTER THE IMPACT, THE CONSUMER REALIZED HE WAS OUT OF HIS SEAT BELT, LYING ACROSS THE SECOND SEAT. THE FRONT SEAT WAS AT A 45 DEGREE ANGLE AND HAD BROKEN BRACKETS ON BOTH SIDES. THE CONSUMER SUSTAINED A SERIOUS BACK INJURY. *JB
TL*THE CONTACT OWNS A 1995 FORD EXPLORER. WHILE MAKING A TURN AT 55 MPH, THE BRAKES LOCKED WITHOUT WARNING. AS A RESULT, THE VEHICLE SLID AND CRASHED INTO A DITCH BEFORE ROLLING OVER THREE TIMES. THE CRASH DESTROYED THE VEHICLE. A POLICE REPORT WAS FILED. AS OF NOVEMBER 6, 2007, THE DEALER HAD NOT INSPECTED THE VEHICLE. THE DRIVER SUSTAINED BRUISES TO THE BACK AND SHOULDER BLADES ALONG WITH CUTS AND SCRATCHES ON HIS LEFT ARM. THE CONTACT FELT THAT THE ANTILOCK BRAKES CAUSED THE FAILURE. THE VIN WAS UNKNOWN. THE FAILURE MILEAGE WAS 120,000 AND CURRENT MILEAGE WAS 145,000.
FAILURE OF THE DRIVER'S SEAT RECLINER MECHANISM IN A CRASH. *AK
CONSUMER STATES WHILE COMING INTO LONG DRIVEWAY THE ACCELERATOR PEDAL GOT STUCK ATTEMPTED TO BRAKE WITH PEDAL AND EMERGENCY BRAKE. DROVE INTO BRICK WALL.NEITHER ONE OF THE AIR BAGS DEFLACTED. DEALER NOTIFIED.*AK *CB
I WAS DRIVING A 95 FORD EXPLORER SOUTH BOUND ON US 95 AT APPROXIMATELY 70 MPH, WHEN THE VEHICLE BEGAN ACCELERATING ON IT'S OWN. WHEN I TOOK MY FOOT OFF THE ACCELERATOR THE VEHICLE KEPT ACCELERATING ON IT'S OWN EVEN WITH THE CRUISE CONTROL OFF AND MY FOOT OFF THE ACCELERATOR PEDAL. I THEN TRIED TO APPLY THE BRAKES BUT SINCE THE VEHICLE WAS ACCELERATING THE EXPLORER JUST SHOOK. I ALSO DOUBLE CHECKED THE CRUISE CONTROL AND IT WAS IN FACT OFF AND TRIED TO PUMP THE GAS TO SEE IF THE PEDAL WAS STUCK BUT NEITHER WORKED. WHEN THE VEHICLE REACHED APPROXIMATELY 85 MPH I ATTEMPTED TO SHIFT INTO NEUTRAL WHEN THE VEHICLE SKIDDED OUT OF CONTROL. THE SKID MARK WAS ABOUT 350 FEET LONG AND THEN THE EXPLORER LEFT THE ROAD AND ROLLED A FEW TIMES UNTIL IT CAME TO A STOP ON THE DRIVER'S SIDE WITH THE ENGINE STILL ROARING. THE ENGINE ONLY STOPPED WHEN PASSENGER TURNED OFF THE IGNITION KEY SINCE THE DRIVER WAS UNCONSCIOUS. *JB
CONSUMER WAS INVOLVED IN AN ACCIDENT IN WHICH THE AIRBAGS DID NOT EXPERIENCE DEPLOYMENT AND THE SEAT BELTS DID NOT LOCK, CONSUMER STATES THE VEHICLE HAS BEEN FIXED, HOWEVER THE SEAT BELTS STILL DO NOT LOCK IN PLACE, CONSUMER WAS INJURED IN THE ACCIDENT. *SLC
THOUGHT THE GUY IN FRONT OF ME WAS GOING TO GO THROUGH A YELLOW LIGHT, SO I FIGURED I WOULD FOLLOW IN SUIT. THEN HE JUST SUDDENLY STOPPED, WITH ME HITTING HIM DIRECTLY IN THE REAREND. I REARENDED SOMEONE AFTER TRYING TO STOP AND MY SEATBELT DID NOT LOCK IN, NOR DID THE AIRBAGS GO OFF. I ENDED UP WITH BRUISED RIBS AND HEART.*AK

WHILE TRAVELING IN WET CONDITIONS AT 45MPH CONSUMER LOST CONTROL, VEHICLE WENT INTO A SKID, RESULTING IN MINOR INJURIES TO OCCUPANTS. VEHICLE WAS TOTALED. PLEASE PROVIDE FURTHER INFORMTATION. *AK
MY THREE OTHER TIRES WERE ALSO SEPARATING AND WERE REPLACED. I AM STILL WAITING FOR FIRESTONE TO PAY ME FOR MY MEDICAL AND EIGHT MONTHS LOSS TIME FROM WORK AND VEHICLE RENTAL. THEY HAD REIMBURSED ME APPROXIMATELY $2,700.00 ONLY FOR DAMAGE TO MY EXPLORER. THE TIRES THEY HAD REPLACED I BELIEVE NOW HAS A RECALL ACCORDING TO THE NEW REPORTS OF 5-21-01. THE TREAD ON THE RIGHT FRONT TIRE SEPARATED AND THE VEHICLE WENT OFF THE ROAD AND ROLLED 4 OR 5 TIMES. I WAS INJURED WITH BRUSES, SCRATCHES,AND GLASS CUTS IN MY FACE. I HAVE CONTACTED NORMA DAVIS AT FIRESTONE (800-543-7522),BUT SHE STATED THAT I NEEDED THE TIRE FOR HER TO HELP. I HAVE SINCE HAD THE VEHICLE REPOCESSED, BECAUSE MY INSURANCE(NATIONWIDE INSURANCE CO.)CHOSE TO REPAIR THE VEHICLE FOR $10000. AFTER MONTHS OF TAKING THE VEHICLE BACK TO THE REPAIR SHOP, I WAS TOLD THAT NATIONWIDE WOULD NO LONGER BE RESPONSIBLE FOR REPAIRS CAUSED BY MY ACCIDENT.
