DID NOT INFLATE DESPITE 3 GOOD "SWATS" TO THE NOSE OF THE VEHICLE. VEHICLE RUNNING GOING THRU CORNERS AT APPROXIMATELY 35 MPH
TL* THE CONTACT OWNED A 2006 CHEVROLET SILVERADO 1500. WHILE DRIVING AT AN UNKNOWN SPEED, THE CONTACT SWERVED TO AVOID HITTING A DEER. THE CONTACT LOST CONTROL OF THE VEHICLE AND IT FLIPPED OVER AND LANDED ON ITS SIDE IN A DITCH. THE AIR BAGS FAILED TO DEPLOY. AS A RESULT, THE CONTACT SUSTAINED A BROKEN NECK, HEAD TRAUMA, AND BACK AND NECK BRUISES THAT REQUIRED MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO A TOW LOT AND DEEMED A TOTAL LOSS. THE DEALER AND MANUFACTURER WERE NOT MADE AWARE OF THE FAILURE. THE FAILURE MILEAGE WAS UNKNOWN.
"TAKATA RECALL" ON 11/04/2016 ATTEMPTING TO PASS ANOTHER VEHICLE, SUDDENLY VEHICLE ON MY PASSENGER SIDE BEGIN TO SIDESWIPED MY VEHICLE, MY PASSING WAS DELAY AND HINDER; ONCOMING TRAFFIC AND SIDESWIPED CONTINUALLY FORCE ME OF THE LEFT SIDE PAVEMENT ONTO THE GRAVEL ON THE LEFT AND INTO A RESIDENT DOGWOOD TREES, NO DEPLOY FROM AIRBAGS VEHICLE WAS CONSIDER A TOTAL LOST BY MY INSURANCE COMPANY AND REPAIR SHOP
TL* THE CONTACT OWNS A 2006 CHEVROLET SILVERADO 1500. THE CONTACT STATED THAT WHILE DRIVING APPROXIMATELY 60 MPH, HE CRASHED INTO A STOPED VEHICLE ON THE HIGHWAY AND THE AIR BAGS FAILED TO DEPLOY. A POLICE REPORT WAS FILED. THE VEHICLE WAS DESTROYED. THE CONTACT AND TWO PASSENGERS IN THE SECOND VEHICLE WERE TRANSPORTED TO A HOSPITAL FOR TREATMENT OF INJURIES. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE. THE FAILURE AND CURRENT MILEAGE WAS 85,000.
INVOLVED IN A HEAD ON COLLISION WITH A 15' HIGH ROCK BERM AT 50 MPH. AFTER HITTING THE ROCKS THE TRUCK LIFTED AND FLIPPED ONTO THE PASSENGER SIDE. FOUR OF FIVE OCCUPANTS SUSTAINED MINOR INJURIES. THE AIRBAGS FAILED TO DEPLOY DURING COLLISION. *TR
1. INVOLVED IN TWO VEHICLE MOTOR VEHICLE COLLISION. MY VEHICLE WAS T-BONED ON THE PASSENGER SIDE, CAUSING THE BUMPER TO BE PUSHED IN AT LEAST 3-6 INCHES. 2. I PERSONALLY BELIEVE THE IMPACT OF THE COLLISION, WHILE BOTH VEHICLES OF SPEEDS OF LESS THAN 20 MILES PER HOUR, SHOULD HAVE CAUSED MY AIRBAGS TO DEPLOY. AIRBAGS DID NOT DEPLOY. 3. UNKNOWN. *TR
TL*THE CONTACT OWNS A 2006 CHEVROLET SILVERADO 1500. WHILE DRIVING BETWEEN 35-40 MPH, THE FRONT AIR BAGS FAILED TO DEPLOY DURING A FRONT END CRASH. GENERAL MOTORS HAS NOT BEEN NOTIFIED. THE CONTACT SUSTAINED BRUISING AND A SPRAINED ANKLE. HER DAUGHTER SUSTAINED A SEVERELY BRUISED HIP AND HER SON IS EXPERIENCING SEVERE MUSCLE ACHES. THE POLICE WAS CALLED TO THE SCENE AND A REPORT WAS FILED. THE CURRENT AND FAILURE MILEAGES WERE 43,000.
DT*: THE CONTACT STATED WHILE ATTEMPTING A U-TURN AT 40MPH, THERE WAS A LOSS OF CONTROL, THE VEHICLE STRUCK A TREE AND THE AIRBAGS DID NOT DEPLOY. THE CONTACT WAS WEARING A SEATBELT HOWEVER, MINOR INJURIES WERE SUSTAINED. THE VEHICLE SUSTAINED EXTENSIVE DAMAGE TO THE FRONT END. ALTHOUGH THERE WAS A CRASH, THE POLICE WERE NOT CONTACTED. THE VEHICLE WAS TOWED TO AN INDEPENDENT BODY REPAIR SHOP, BUT THE AIRBAGS WERE NOT INSPECTED.
On [XXX], at or around midnight, I was traveling [XXX] at approximately 60 mph. The traffic was very light, with few vehicles on the road. It was raining, and I was in the number 1 lane. Two additional lanes were to my left, reserved for toll road traffic. I did not see any other vehicles on [XXX], nor did I see any vehicles to my left in the toll lanes. There was a physical separation between [XXX] and the toll road, consisting of 36-inch plastic candlestick dividers with approximately 10-foot separations. As I was traveling, I suddenly felt a lurch, and the right rear of my 2006 Chevrolet Silverado (Vin# [XXX] ) dropped. My pickup truck immediately began to spin to my left, forcing me into the toll lanes and into the concrete center divider. When my vehicle came to rest, I was facing eastbound in the westbound lanes of the toll road. My lights were no longer except a single amber light flashing, which gave limited warning to approaching vehicles. The California Highway Patrol arrived (unknown why no traffic collision report was taken), my vehicle was removed from the roadway, the right rear wheel and tire had separated from the vehicle and rolled to the right shoulder of [XXX] 150 yards from where my pickup came to rest. The vehicle was towed to the tow yard, where a vehicle inspection was conducted on March 19, 2025. The right rear wheel and tire for some reason were not recovered from the collision scene. The inspection revealed the cause of the collision was a broken/severed axle, not related to driver error. Photographs of the broken axle were taken and are available for review. The axle clearly had a manufacturing defect, poor casting or forging. The defect appeared to be an ongoing problem and separated from the vehicle at the time of the collision for no apparent reason. Depending on the time of day, traffic conditions, and the number of passengers, the collision had the potential to be much worse. THE CONTACT STATED THAT WHILE DRIVING APPROXIMATELY 45 MPH, THE BRAKING SYSTEM HAD MALFUNCTIONED AND DID NOT STOP THE VEHICLE WHEN PRESSING THE BRAKE PEDAL CAUSING THE VEHICLE TO CRASH INTO A TREE. DURING THE CRASH, THE FRONT END EXPERIENCED SEVERE DAMAGE BUT THE FRONT AIR BAG DID NOT DEPLOY. NO INJURIES WERE REPORTED AND NO POLICE REPORT WAS FILED. THE VEHICLE WAS MANEUVERED TO A PARKING LOT AND A TOWING COMPANY WAS CALLED. THE COSMETIC DAMAGES WERE BEING REPAIRED AND THE CAUSE OF THE FAILURE WAS NOT DETERMINED AS OF YET. A LOCAL DEALER (UNKNOWN) WAS CALLED AND INFORMED ABOUT THE FAILURE. THE MANUFACTURER THEN CALLED THE CONTACT AND OPENED A CASE. THE FAILURE MILEAGE WAS 200,000.
FAILURE OF BRAKE LINE COMPLETE LOSS OF BRAKES. GOING OUT OF DRIVEWAY TURN INTO A DTE POLE TO STOP
WAS TRAVELING APPROXIMATELY 55 MPH DOWN A STATE HIGHWAY. AFTER COMING AROUND A BLIND CURVE, CAME UPON STOPPED TRAFFIC DUE TO ROAD WORK. APPLIED PRESSURE TO BRAKE PEDAL WHEN PEDAL WENT SOFT AND DEPRESSED ALL THE WAY TO THE FLOOR. AFTER RELEASING BRAKES AND PUSHING AGAIN STILL HAD NO BRAKES. HAD TO SWERVE OUT OF ROAD-WAY TO AVOID REAR ENDING MULTIPLE VEHICLES. AFTER RUNNING OUT OF ROAD, HIT 2 CULVERT PIPES ALONG WITH ROAD WORK SIGNAGE, FINALLY BRINGING THE TRUCK TO A STOP IN A RESIDENTIAL YARD. TRUCK BRAKE LINES FOR FRONT BRAKES HAD CORRODED AND RUPTURED CAUSING THE INCIDENT. FURTHER INVESTIGATION INTO THIS MATTER SHOWS AN APPARENT ISSUE WITH THIS MODEL GMS FOR BRAKE LINE CORROSION THAT GM HAS FAILED TO ADDRESS THROUGH THE RECALL PROCESS.

AS A RESULT, THE CONTACT CRASHED INTO A TREE AND THE AIR BAGS FAILED TO DEPLOY. A POLICE REPORT WAS NOT FILED AND THERE WERE NO INJURIES REPORTED. THE VEHICLE WAS TAKEN TO AN INDEPENDENT MECHANIC WHERE IT WAS DIAGNOSED THAT THE BRAKE LINE WAS RUSTED AND NEEDED TO BE REPLACED. THE VEHICLE WAS REPAIRED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS 130,000.

THE VEHICLE WAS TOWED TO A PRIVATE MECHANIC WHERE THERE WAS NO DIAGNOSTIC TEST PERFORMED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE VEHICLE WAS DESTROYED. THE APPROXIMATE FAILURE AND CURRENT MILEAGE WAS 100,000.
WHILE APPROACHING A ONE LANE BRIDGE ON DRY, CLEAN PAVEMENT AT APPROX 10 MPH I ATTEMPTED TO BRAKE TO LINE UP WITH THE BRIDGE. THE VEHICLE FAILED TO BRAKE, WITH THE ABS LIGHT COMING ON AND STAYED ON AND A LOUD NOISE CAME FROM THE PASSENGER SIDE FRONT WHEEL WELL. I CLOSELY MISSED HITTING THE RAIL OF THE BRIDGE (WHICH WOULD HAVE BEEN A 20FT PLUNGE OFF THE BRIDGE) AND THE VEHICLE CONTINUED FOR ANOTHER 20 TO 25 FT WHILE ON THE SLIGHT DOWN HILL GRADED SLOPE. NO MATTER HOW I PUSHED THE BRAKE PEDAL THE BRAKES FAILED TO RESPOND AND I UTILIZED MY TRAINING AS AN EMERGENCY APPARATUS DRIVER AND PLACED THE VEHICLE INTO A LOWER GEAR AND DRIFTED IT INTO AN EMBANKMENT TO STOP. I THEN BACKED THE VEHICLE OUT OF THE EMBANKMENT AND THE VEHICLE OPERATED FINE BUT THE ABS LIGHT REMAINS ON. AS I SEE WITH OTHER POSTS THIS IS AN OBVIOUS PROBLEM WITH THE SILVERADO THAT NEEDS TO BE ADDRESSED IMMEDIATELY. *TR

A POLICE REPORT WAS FILED OF THE INCIDENT. THE VEHICLE WAS TOWED TO AN AUTO COLLISION CENTER. THE FAILURE MILEAGE WAS APPROXIMATELY 58,000. THE VIN WAS UNAVAILABLE.
DRIVERS SIDE REAR AXLE AND TIRE CAME OFF THE VEHICLE, CAUSING 2006 SILVERADO TO WRECK. AIR BAG DID NOT DEPLOY. WE WERE ON THE ENTRANCE RAMP TO I-95 WHEN SUDDENLY TRUCK TURNED TOWARD CONCRETE GUARD RAIL. AFTER HITTING RAIL HEAD ON AND SKIDDING ACROSS ENTRANCE RAMP HITTING RAIL ( SEVERAL TIMES) VEHICLE CAME TO A COMPLETE STOP. TIRE AND AXEL WERE (LEFT REAR-DRIVER SIDE) GONE. TRUCK IS TOTALED. AIR BAG DID NOT DEPLOY. *TR
