MY FATHER WAS DRIVING A STOCK DUAL CAB CHEVY 2008 SILVERADO, NOT SURE WHAT THE STOCK TIRES FOR THIS TRUCK IS. WAS BOUGHT IN LUCEDALE, MS AT WALT MASSEY CHEVROLET. HE WAS FORCED OFF THE NORTH SIDE OF ROAD TRAVELING WEST AT APPROX. 60 MPH AND WHEN REENTERING PAVEMENT OR DURING YAWING (STILL UNCERTAIN, BUT PICTURES WERE TAKEN BY INVESTIGATORS), THE RIGHT TIRE DETACHED FROM THE AXLE WHICH CAUSED THE VEHICLE TO GO OFF OTHER SIDE OF ROAD AND HIT A LARGE OAK. THE ONSTAR SERVICES, WHICH WERE FUNCTIONING, DID NOT NOTIFY EMS OR FAMILY. THE TRUCK WAS STILL RUNNING WHEN WITNESSES WENT TO ASSIST HIM. THE VEHICLE STARTED SMOKING INSIDE THE TRUCK, WHICH MADE WITNESSES LEAVE SCENE AND THE VEHICLE BLEW UP AND NOTHING WAS LEFT TO IDENTIFY HIM OR THE VEHICLE TO NOTIFY FAMILY OF HIS WHEREABOUTS. *TR

SWERVED TO MISS A DEER AND HIT AN EMBANKMENT AND A CEMENT CULVERT ON A HIGHWAY AT NIGHT, WAS BY MYSELF. NEITHER OF MY AIRBAGS DEPLOYED AND I HIT MY CHEST EXTREMELY HARD ON THE STEERING WHEEL AND I AM IN SEVERE PAIN FROM MY RIBS. HAVE SEVERE HEMATOMA AND BRUISING.

I WAS HIT FROM FRONT DRIVER SIDE WITH A TRAIL BLAZER. AFTER REPAIR ABOUT A COUPLE WEEKS MY AIRBAG SENSOR LIT UP ON MY DASH. I BELIEVE THIS MAY BE FROM HER HITTING MY TRUCK FRONT END. MAYBE. WHEN IT WAS REPAIRED THEY MIGHT OF PULLED OR BLEW SENSOR . I PAID FOR COMPUTER DIAGNOSIS AND THEN TOLD INSURANCE COMPANY. THEY SAID I WOULD GET MONEY. BACK IF RELATED. TO ACCIDENT. RAILROAD HYRAIL VEHICLE EQUIPPED, AFTER MARKET, WITH HYRAIL WHEEL ASSEMBLIES WHICH ALLOW THE VEHICLE TO OPERATE ON RAILROAD TRACKS. ONCE HYRAIL (FLANGED) WHEELS ARE RETRACTED, THE VEHICLE IS OPERATED OVER THE HIGHWAYS AND PUBLIC ROADS LIKE ANY OTHER VEHICLE. ON 10/5/12 NEAR NEWCASTLE, WY A RAILROAD EMPLOYEE WAS OPERATING VEHICLE ON THE RAILROAD TRACKS WHEN IT STRUCK STANDING EQUIPMENT AT APPROX 15 MPH. AIRBAGS DID NOT DEPLOY AND RAIL EMPLOYEE WAS INJURED. CONCERN IS THAT THE INSTALLATION OF HYRAIL WHEEL ASSEMBLIES MIGHT BE IMPACTING THE AIRBAG SENSOR. THE HYRAIL WHEEL ASSEMBLIES EXTEND APPROX. ONE FOOT OF STEEL FRAMEWORK AHEAD OF THE FRONT AND REAR BUMPERS. THE PLACEMENT OF THESE ASSEMBLIES MIGHT INADVERTENTLY BE AFFECTING AIRBAG DEPLOYMENT IN CRASHES ON THE RAILROAD TRACK AND ON PUBLIC ROADS. *TR

THE AIR BAGS DID NOT DEPLOY AND THERE I WAS NO RECORDED INCIDENCE ON THE DATA RECORDER. THE VEHICLE WAS TOTALED BY THE INSURANCE CO. MY CONCERN IS THAT THIS VEHICLE COULD BE REBUILT AND PUT BACK ON THE ROAD AS DRIVABLE VEHICLE SINCE IT HAD ONLY 13,000 MILES ON IT. IF NO CAUSE FOR THE MALFUNCTION CAN BE DETERMINED THERE IS A DISTINCT POSSIBILITY THIS COULD HAPPEN AGAIN WITH POSSIBLE LOSS OF LIFE. THE INITIAL IMPACT WAS MY TRUCK'S FRONT TO THE PASSENGER SIDE OF THE VEHICLE THAT ENTERED THE ROADWAY. AFTER BOTH VEHICLES SEPARATED, MY VEHICLE CONTINUED AND STRUCK A MEDIAN GUARDRAIL HEAD-ON (ANOTHER FRONTAL IMPACT) BEFORE COMING TO REST. MY SEATBELT DID NOT LOCK AND NO AIRBAGS DEPLOYED. THE HIGHWAY PATROL INVESTIGATED THE ACTIVE COLLISION SCENE. FOUR DAYS LATER, AN INDEPENDENT COLLISION RECONSTRUCTION EXPERT THAT I HIRED INSPECTED MY VEHICLE AT MY LOCAL GM DEALERSHIP. THE INSPECTION REVEALED THAT: I WAS IN FACT GOING 45MPH, MY SEATBELT WAS BUCKLED, MY SEATBELT PRETENSIONER DID NOT ACTIVATE, AND NO AIRBAGS DEPLOYED. GM HAS ALSO BEEN CONTACTED BY ME, AND THEY ARE CURRENTLY INVESTIGATING THE ISSUE. *TR
