Sometime between backing into my parking spot at work, then backing into a parking spot at the dentist, My Tailgate released/lowered. When I backed in at the dentist, my back up camera seemed to malfunction, so I just used my mirrors. Apparently, the camera was just pointing at the ground. Sine you can't see your tailgate in the mirrors; I could not see that it hit the retaining wall at the back of the space.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Jul 22, 2024Old lyme, CT
Crash

This weekend while traveling on I95N I got into a wreck with automatic cruise control on. I had been in stop and go traffic for about 20 minutes and was following behind a Gatormade hauling trailer for well over a mile with no issues. Suddenly, from a dead stop, the truck attempted to accelerate to its preset speed of 50 mph. I was only one car length away from the trailer and wasn't covering the break. By the time I got my foot on the break, I slammed in the back of the trailer. At no point did the emergency brake warning or any alerts pop up.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Aug 24, 2023Denver, NC
Crash

I was driving Eastbound on highway 150 on my way to work. I have adaptive cruise control in my truck and have used it many times before on this exact road with no issues. Traffic was moving slow and the adaptive cruise control was following the car in front of me exactly how it is designed to do (ACC can be turned on at speeds as low as 5 mph in this vehicle and can come to a complete stop and restart movement from a complete stop). I drove through a section of this road right after the lake crossing where the morning sun was pretty bright directly facing us for a few seconds. During this time my foot was off of the accelerator and brake as instructed by the vehicle manual for safe operation of the ACC system. In these few seconds the truck unexpectedly accelerated aggressively with no warning and before I had a chance to react the truck rear ended another vehicle, pushing it into the car in front of them as well. I have been found liable so far by insurance. I am trying to argue on my behalf. I firmly believe I am not personally liable for this accident. The system in the truck is supposed to fail safe (meaning when meeting uncertain variables, such as low camera visibility, the vehicle should disengage the ACC instead of aggressively accelerating with no information of what is in front of it) and it did not in this circumstance. As stated, I had no input to the accelerator or the brake at the time of the accident according to the owners manual safe operation of this system. The vehicle FAILED to disengage the ACC system as it was supposed to and caused an accident I had NO TIME to react to by aggressively accelerating without any warning. The vehicle has been appraised for repair by the insurance company and was found to have $7000 in damages.

On [XXX], my 2023 Chevrolet Silverado 1500 LT caught fire while parked in my residential driveway at [XXX] . The vehicle had been started approximately 10 minutes earlier using the factory-installed remote start system. The vehicle was unattended at the time. My neighbor noticed the vehicle on fire and called 911. The Andover Fire Department responded and extinguished the fire. The fire department indicated the fire appeared to be electrical in nature. The fire appeared to originate in the dashboard and steering column area based on visible damage and fire progression. The vehicle had approximately 60,000 miles and was properly maintained. There were no prior warning lights, messages, or symptoms before the fire. The remote start system was factory-installed and not aftermarket. The vehicle is currently available for inspection by insurance, manufacturer, or investigators at [XXX] . The vehicle is likely a total loss due to the extent of the fire damage. This incident posed a serious safety risk as the vehicle caught fire spontaneously while parked near my home. The fire could have spread to my residence or caused injury if someone had been inside or near the vehicle. The fire department responded, and an official fire report is pending. Insurance claim has been opened and inspection is pending. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I am reporting another engine failure on the 6.2L L87. This engine passed the initial recall testing and the oil was changed to 0W40 on September 30, 2025. However, my engine in my 2023 LT Trail Boss started knocking on Friday, March 20, 2026 (at 22,500 miles) after I picked up my children from school. Thankfully it started as I pulled into my driveway with them so they weren’t in any danger. No loss of power on the truck or warning lights visible. However, it was loud enough and odd enough for my [XXX] olds to even notice the sound in the cab. It was dropped off at the dealership on that Saturday morning and the GM dealership was finally able to look at it on Wednesday, March 25th and confirmed what was expected. The advisor stated the engine will need to be replaced due to the engine heading towards bearing/crankshaft failure. I am still currently without my truck as of today (April 4th) with no repair date in sight. I will say that the dealership service advisor has been very good about communicating about its status. However, he stated that he has had no luck finding an new engine and is unable to give a timetable in getting one. He did offer to start a PO for a rental car through enterprise. He stated they are not giving loaners, but could get a rental car. He also stated that it would not be a truck due to GM only giving $50 per day towards a rental. So here I sit currently without my truck. I see multiple reports about the engines being on back order and even the replacement engines are failing within the fist 500 miles. Some reports indicating they are on the third or fourth engine. Something needs to done about getting these trucks bought back by GM. They have clearly failed with this engine and are replacing the failed engines with the same thing! It is a well documented manufacturing flaw and design! I am thankful it occurred near my home. I would not have been happy with a catastrophic failure on a highway with my [XXX] old children in the truck!! INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INF

The contact owns a 2023 Chevrolet Silverado 1500. The contact stated that while driving approximately 65 MPH, the vehicle started to lose motive power. There were several unknown warning lights illuminated, and the message that the vehicle would not exceed 55 MPH while driving. The contact stated that after driving for several miles, the message appeared that the vehicle would not accelerate to 30 MPH while driving. The contact had taken the vehicle to a local dealer, who diagnosed the vehicle and determined that the DEF tank heater needed to be replaced; GM Part Number: 85856213; however, the part was on a national back order. The vehicle was not repaired. The manufacturer was informed of the failure. The failure mileage was approximately 67,000.

POWER TRAIN
Mar 30, 2026Wentzville, MO

At cold start vehicle often required several seconds for the transmission to engage before moving. After traveling at highway speed (45+ MPH) vehicle began to make clunking noise as if the transmission were changing gears but it was not and the RPMs on the tachometer did not change. Instances became worse and more frequent over 6 month period, vehicle never displayed any codes and no notifications or check engine/transmission lights on driver information center. Took vehicle to O’Fallon Buick GMC and was advised torque converter and transmission valve body had failed and needed replaced. I inquired about ongoing issue with this vehicle based on my findings online and was told it was not a recall issue at this time. Truck has 74,000 miles and service records for all maintenance to date.

I'm reporting a 5mpg fuel economy reduction immediately following the 6.2L L87 engine recall (NHTSA #24V795000). I use 93 octane and monitor my fuel mileage consistently. I bragged about my fuel economy in my 6.2l until after the recall was applied. This is going to cause a financial hardship as I'm on fixed income after Servicing 29 years in the Army and 100% disabled. There is no other reason my mpg should have changed as my driving habits have not.

While driving on the freeway without warning, my engine stopped working. My vehicle was towed to the Chevy dealership where they confirmed that I was the latest victim of the ongoing engine failure problem with the 6.2 L engine. There was no warning, no sign that I had a problem. My engine just simply stopped working on a busy freeway. I was barely able to make it to the shoulder where I had merging traffic coming on the freeway behind me. I’m deeply concerned that Chevy will replace it with the same faulty engine and I will be put in the same danger situation again in the future.

FUEL SYSTEM, DIESEL

The contact owns a 2023 Chevrolet Silverado 1500. The contact stated that on two separate occasions while hauling the camper, the "High Diesel Exhaust Fluid", Exhaust Fluid Range", and "Fill DEF (Diesel Exhaust Fluid) Now" warning messages were displayed on the instrument panel. The messages indicated that the Diesel Exhaust Fluid was low and needed to be replenished. The contact stated that the failure caused the instrument panel to freeze and not operate as needed. Upon attempting to refuel the vehicle, fuel failed to enter the fuel tank since the diesel tank was full. The vehicle was taken to a dealer, where the contact was informed that the failure was an electrical glitch and that there were no issues with the Diesel Exhaust Fluid system. The contact was also informed by the dealer of Technical Service Bulletin: 22-NA-150, which the contact related to the failure. The dealer reset the vehicle, which caused the instrument panel to return to normal operation. The manufacturer was then notified of the failure, and the contact was informed that vehicle was not included in the TSB. The contact was given a case number and was referred to the NHTSA Hotline to report the failure. The vehicle was not repaired. The failure mileage was approximately 55,000.
