THIS INCIDENT WAS REPORTED BY FAMILY OF THE DRIVER. VEHICLE HAD BEEN MODIFIED FOR USE BY A DISABLED PERSON. STEERING SYSTEM WAS CONTROLLED BY AN "ELECTRONIC MOBILITY SYSTEM (MODEL DS2000)". WHILE DRIVING APPROXIMATELY 30 TO 35 MPH, DRIVER LOST ALL DIRECTIONAL CONTROL OF THE VEHICLE, AND WAS INVOLVED IN A HEAD-ON COLLISION WHEN HIS VEHICLE WANDERED INTO ONCOMING TRAFFIC. THE DRIVER WAS KILLED . PLEASE PROVIDE ANY ADDITIONAL INFORMATION / DOCUMENTATION / PHOTOGRAPHS.*AK
INSURED HAD A HEAD ON COLLISION, INSURED WASN'T WEARING A SEAT BELT, AIR BAG DIDN'T DEPLOY. INSURED SUSTAINED INJURIES TO HIS RIBS AND HAND, CLAIM 13-7256-113. *JG
CONSUMER'S VEHICLE WAS INVOLVED IN A FRONTAL CRASH TRAVELING AT AN UNKNOWN SPEED. DRIVER'A AIR BAG FAILED TO DEPLOY, AND SEAT BELT RETRACTOR FAILED TO CATCH. CONSUMERS HEAD HIT WINDSHIELD, KNOCKING OUT TWO TEETH, AND ARMS WERE INJURED. THE VEHICLE HAS NOT BEEN EXAMINED TO DETERMINE CAUSE OF PROBLEM. WHILE COMING TO A STOP LIGHT, SOMEONE RAN STOP LIGHT AND HIT CONSUMER AT ABOUT 40 MPH ON DRIVERS SIDE. CAR SPUN 180 DEGREES, AND NONE OF AIRBAGS DEPLOYED.*AK *JB
CONSUMER WAS TRAVELING 15-20 MPH AND ANOTHER VEHICLE MADE AN ILLEGAL LEFT HAND TURN AT LIGHT & HIT FRONT DRIVER'S SIDE. AIR BAGS DEPLOYED & CAUSED TRAUMA TO DRIVER'S LEFT EYE, RESULTING IN A PERMANENT INJURY. DEALER WILL BE CONTACTED. *AK
WHILE DRIVNG ABOUT 45 MPH CONSUMER'S VEHICLE WAS STRUCK BY ANOTHER VEHICLE. AIR BAGS DEPLOYED BUT, DRIVER'S SEAT BELT BECAME DETACHED FROM THE HARNESS BOLT. DRIVER AND PASSENGERS INJURED. *AK *ML
CONSUMER WAS INVOLVED IN A REAR END COLLISION AND THE AIR BAG DID NOT DEPLOY. DEALER HAS NOT BEEN CONTACTED. *AK
WHILE STOPPING, VEHICLE WAS REARENDED BY TRUCK, HIT GUARDRAIL, FRONT OCCUPANTS FORCED FORWARD/BACKWARD, BOTH FRONT SEATS SNAPPED LIKE TWIGS, SEATBACKS COLLAPSED COMPLETELY, FELL BACKWARDS WITH OCCUPANTS, ALSO NO DEPLOYMENT OF DRIVER'S AIR BAG. *AK
WHILE DRIVING 20 MPH DRIVER APPLIED THE BRAKES AND PEDAL WAS HARD. CONSUMER WAS UNABLE TO STOP THE VEHICLE, IT SKIDDED, AND HIT ANOTHER VEHICLE. UPON IMPACT, BOTH AIR BAGS DID NOT DEPLOY. NO INJURIES WERE REPORTED ABS LIGHT APPEARED ON THE DASHBOARD, AND REMAINED ON UNTIL THE VEHICLE WAS TURNED OFF. CONSUMER DROVE VEHICLE TO DEALER FOR INSPECTION, AND MECHANIC DETERMINED THAT A PART OF THE ABS MODULE NEEDED REPLACING. THE PART WAS NOT ON RECALL OR UNDER WARRANTY. *AK
WHILE DRIVING, CONSUMER MADE A RIGHT TURN. THE TURN WAS COMPLETED SUCCESSFULLY, BUT THE STEERING WHEEL WOULD NOT RECOVER, RESULTING IN A COLLISION. DEALER DIAGNOSED FAILURE OF THE SPINDLE AND CONTROL ARM.*AK
MY HUSBAND WAS REAR-ENDED BY A CAR AT APPROXIMATELY 30MPH IN OUR 1992 DODGE GRAND CARAVAN. THE BACK OF THE DRIVERS SEAT COLLAPSED. FORTUNATELY HE WAS NOT HURT, BUT HE COULD EASILY HAVE LOST CONTROL OF THE VEHICLE. ALSO, IF OUR 8 YEAR OLD CHILD HAD BEEN WITH HIM, SHE WOULD HAVE BEEN RIDING IN THE SEAT DIRECTLY BEHIND THE DRIVER AND COULD HAVE BEEN SEVERLY INJURED.
CONSUMER WAS DRIVING INTO THE GARAGE WHEN HE APPLIED BRAKES AND VEHICLE DIDN'T STOP. VEHICLE WENT INTO THE GARAGE DOOR. *AK
DURING FRONTAL IMPACT AT 36 MPH, THE DRIVER'S SIDE AIR BAG DID NOT DEPLOY. PLEASE PROVIDE DETAILS. *AK

MY 1992 DODGE GRAND CARAVAN LE COOLANT FAN FAILED AND THE RELAY DID NOT INTERUPT WHICH CONTINUED TO FEED POWER BACK TO THE FUSE PANEL WHICH IN RETURN CAUSED MY WIRING HARNESS TO IGNITE INTO FLAMES UNDER THE HOOD RIGHT OVER THE LEFT STRUT ! *AK
WHILE DRIVING VEHICLE CAUGHT ON FIRE UNDER THE DASH NEAR BASE OF STEERING COLUMN. VEHICLE WAS TOTALED. PLEASE PROVIDE FURTHER INFORMATION.*AK
SEAT BELT ANCHORAGE:CENTER SEAT/RIGHT SIDE SAFETY BELT ANCHORAGE RELEASES FREQUENTLY WHEN THE BELT IS IN USE. *AK
VEHICLE FIRE.
THE CONSUMER WAS INVOLVED IN PAST RECALL 730 BY DODGE THAT WAS POORLY HANDLED. THE REAR HATCH ATTACHMENT BOLT WAS THE ITEM ON RECALL. THE CONSUMER HAD TAKEN THE VEHICLE IN FOR REPAIR IN JULY OF 1997 BECAUSE THE BOLTS HAD BROKEN, AND HAD THEM REPLACED. THEN THEY BROKE AGAIN IN AUGUST OF 1999. ON MAY 1, 2005 ONE OF THEM BROKE AGAIN, AND ALMOST BROKE HIS WIFE'S ARM. ON 5-3-2005, AND THE DEALER SAID THAT THE CAR WAS TOO OLD. THEY WOULDN'T FIX IT UNLESS THE CONSUMER PAID. THE DEALERS RESPONSE WAS INAPPROPRIATE. THE DEALER CHECKED WITH THE DISTRICTS OFFICE AND VALIDATED THAT IT WOULDN'T DO IT. ALSO, THE CONSUMER WAS INAPPROPRIATE. THE THIRD ISSUE WAS THE ADEQUACY OF THE RECALL ITSELF. THE FOURTH ISSUE WAS THAT THE DEALER JUST TOLD HIM THAT HE WOULDN'T FIX BOTH SIDES. *AK THE OIL PAN LEAKED AND THE SERPITINE BELT. GEAR SHIFT DISENGAGED FROM PARK WHILE VEHICLE IGNITION WAS OFF. PURSE STRAP LOOPED OVER GEAR SHIFT AS OWNER WAS EXITING VEHICLE CAUSING VEHICLE GEAR SHIFT TO MOVE FROM PARK, VEHICLE BEGAN TO ROLL BACKWARDS CAUSING OWNER TO SLIP UNDER LEFT FRONT WHEEL. *AK
