EHICLE WAS INVOLVED IN A REAR IMPACT ABOUT 50 MPH WHICH CAUSED DRIVER'S SEATBACK TO COLLAPSE, AND DRIVER FELL BACKWARDS. THIS MAY HAVE CAUSED INJURIES. *AK

SEAT BELT FAILED 3 TIMES IN TWO SMALL IMPACT CRASHES TO RELEASE, RESULTING IN SEVERE CHEST/NECK AND SHOULDER INJURIES. DRIVER HAD TO BE MANUALLY REMOVED. *AK

TL* THE CONTACT OWNS A 1993 DODGE GRAND CARAVAN. WHILE DRIVING 20 MPH, THE BRAKES FAILED WITHOUT WARNING. THE CONTACT DEPRESSED THE BRAKE PEDAL, BUT THE VEHICLE WOULD NOT STOP AND CRASHED INTO THE PRECEDING VEHICLE. THE AIR BAGS DID NOT DEPLOY. A POLICE REPORT WAS NOT FILED. THERE WERE NO INJURIES SUSTAINED. THE VEHICLE WAS ABLE TO BE DRIVEN AWAY FROM THE SCENE. DICK HANNAH CHRYSLER DODGE JEEP RAM IN VANCOUVER, WASHINGTON WAS MADE AWARE OF THE FAILURE AND STATED THAT THEY DID NOT HAVE THE PARTS TO REPAIR THE VEHICLE. IN ADDITION, THE DEALER ACKNOWLEDGED THAT A BRAKE FAILURE HAD OCCURRED. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS MADE AWARE OF THE ISSUE. THE FAILURE MILEAGE WAS APPROXIMATELY 222,400. *JS

WHILE APPLYING BRAKES PEDAL WENT TO THE FLOOR, RESULTING IN EXTENDED STOPPING WHICH CAUSED A MINOR COLLISION. PRIOR TO THE COLLISION, DEALER HAD REPAIRED THE VEHICLE UNDER A SAFETY RECALL FOR BRAKES. *AK
DESIGN FLAW OF HAIR SPRING IN THE AIRBAG SENSOR HAS BROKEN, CAUSING THE AIRBAG TO FAIL. DEALER HAS REPAIRED THE VEHICLE. *AK *YC THE CLOCK SPRING ASSEMBLY IS THE ELECTRICAL HARNESS FOR THE CRUISE CONTROL AND AIR BAG. THE CLOCK SPRING ASSEMBLY FAILURE CAUSED THE AIR BAG INOPERABLE. THE CLOCK SPRING ASSEMBLY WAS REPLACED BUT FAILED AGAIN AND AIR BAG FAILED TO DEPLOY DURING COLLISION. THE CLOCK SPRING ASSEMBLY HAD BEEN DIAGNOSED TO BE DEFECTIVE AND SHOULD BE RECALLED. *YH
WHILE DRIVING DOWN HIGHWAY CONSUMER STRUCK A DEER WHICH CAUSED AN ACCIDENT, BUT AIR BAGS DID NOT DEPLOY. *AK
ABS SYSTEM FAILURE - DEALER REFUSES TO FIX PROBLEM. THERE IS A RECALL FOR THIS PROBLEM DATED APR 19 1996 - NHTSA CAMPAING # 96V09900 - OVER 320,000 VEHICLES AFFECTED.
THE VEHICLE WAS INVOLVED IN AN ACCIDENT, DURING IMPACT THE ANCHOR WHICH ATTACHES THE SEATBELT TO THE FLOOR BECAME UNATTACHED, THIS RESULTED IN FAILURE TO HOLD CHILDS CAR SEAT IN PLACE CAUSING HER TO STRIKE THE FRONT ROW PASSENGER SEAT, THE CHILD SUSTAINED A BLOODY LIP AND BRUISED CHEEK. NLM

WHEN CONSUMER OPENED THE HATCH DOOR IN THE REAR OF VEHICLE THE ROD BROKE FREE FROM ATTACHMENT, FALLING ON CONSUMER'S HAND, SHE WAS INJURED. *AK

The contact stated that while driving at an undisclosed speed and attempting to slow the vehicle with the brake pedal depressed, the brake pedal required excessive effort to bring the vehicle to a complete stop. The ABS warning light was illuminated. The vehicle was taken to an independent mechanic, who diagnosed a failure with the brake actuator piston assembly. The vehicle was not repaired. After investigating the failure, the contact related the failure to NHTSA Campaign Number: 96V099000 (Service Brakes, Hydraulic) but the VIN was not included. The manufacturer was notified of the failure but provided no assistance. The failure mileage was 132,000.

THE VEHICLE WAS TAKEN TO AN INDEPENDENT MECHANIC WHERE THE TECHNICIAN STATED THAT THE ABS PUMP FAILED AND NEEDED TO BE REPLACED. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE AND STATED THAT THE VEHICLE WAS PREVIOUSLY SERVICED PER NHTSA CAMPAIGN NUMBER: 96V099000 (SERVICE BRAKES, HYDRAULIC). THE FAILURE MILEAGE WAS 190,000.

THE VEHICLE WAS TAKEN TO A DEALER WHERE IT WAS DIAGNOSED THAT THE BRAKE ACTUATOR FAILED AND NEEDED TO BE REPLACED. IT WAS UNKNOWN IF THE VEHICLE WAS REPAIRED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURES. THE FAILURE MILEAGE WAS APPROXIMATELY 140,000. UPDATED 09/20/16*LJ THE BRAKES WORKED WITH THE RELAY CONTINUOUSLY OPERATING, WHICH ACTIVATED THE BRAKE PUMP AND MOTOR ASSEMBLY. THE PROBLEM WAS THE PUMP/MOTOR ASSEMBLY WOULD BURN OUT IF THE VAN WERE DRIVEN FOR ANY LENGTH OF TIME. UPDATED 09/21/ *JB
