STATE OF NEVADA TRAFFIC ACCIDENT REPORT. 2004 FORD F-150. COLLISION OCCURRED IN WHICH THE PASSENGERS WERE EJECTED FROM THE VEHICLES. *MC *TR UPDATED*JB
The vehicle rolled and killed the occupant because the roof caved in. The shoulder portion of the seat belt did not hold the occupant (only the lap portion) and the occupant was partially ejected out of the rear of the cab. The vehicle was inspected and purchased by State Farm insurance since the vehicle was totaled. The 2000's Ford F-150s have roofs that are too weak to support the weight of the vehicle, thereby putting the occupants in danger of injury or death. This is well documented. There were no warnings.
TL* THE CONTACT OWNS A 2004 FORD F-150. THE CONTACT WAS DRIVING 50 MPH WHEN HE CRASHED INTO ANOTHER VEHICLE. THE AIR BAGS DID NOT DEPLOY AND THE SEAT BELTS FAILED TO RESTRAIN THE CONTACT. THE CONTACT WAS THROWN INTO THE STEERING WHEEL AND SUSTAINED A FRACTURED DISC IN HIS BACK. THE VEHICLE WAS DESTROYED. THE VEHICLE WAS TOWED BUT CAUGHT ON FIRE WHILE IN TOW. THE MANUFACTURER WAS CONTACTED IN REGARDS TO THE FAILURE BUT ADVISED THAT THEY WERE UNABLE TO ASSIST. A POLICE REPORT WAS AVAILABLE. THE FAILURE MILEAGE WAS 135,000.

DRIVER LOST CONTROL OF LIGHT DUTY TRUCK, ESTIMATED SPEED AT 50 TO 60 MILES PER HOUR, TELEPHONE POLE WAS SHEARED COMPLETELY OFF FROM GROUND, TRUCK CONTINUED SEVERAL MORE FEET AND CAME TO REST AFTER HITTING A TREE. NEITHER THE DRIVER OR PASSENGER AIR BAG WAS DEPLOYED ON IMPACT. CRASH RESULTED IN DEATH OF DRIVER. *JB
DT: THE CONTACT'S WIFE WAS DRIVING THE VEHICLE AND THE BRAKES WERE NOT WORKING PROPERLY. WHILE TRAVELING DOWN HILL THERE WHERE NO BRAKES. THIS, IN TURN, CAUSED THE CONTACT'S VEHICLE TO REAR END ANOTHER VEHICLE. THIS ACCIDENT INVOLVED FOUR OTHER VEHICLES WHICH SUSTAINED DAMAGES. THIS CRASH WAS A FRONTAL IMPACT .UPON IMPACT, THE AIR BAGS DID NOT DEPLOY. THERE WERE NO WARNING LIGHTS PRIOR TO THE ACCIDENT. THE VEHICLE WAS TOWED TO A BODY SHOP FOR INSPECTION .THE VEHICLE WAS UNDER WARRANTY.
35 TO 40 MILE PER HOUR TREE HIT TO A 2004 FORD F150 4WD PICKUP, NO AIR BAG DEPLOYMENT RESULTED IN FATAL ACCIDENT. *NM
TL* THE CONTACT OWNS A 2004 FORD F-150. THE CONTACT'S VEHICLE HAS GENERAL AMERITRAC TR TIRES, SIZE 8T25570R17. THE TIRES BLEW OUT TWICE WHILE DRIVING AT 60 MPH. THE SIDEWALL OF THE PASSENGER SIDE TIRE BLEW OUT ONE YEAR AFTER THE VEHICLE WAS PURCHASED. THE SECOND BLOWOUT WAS ON THE PASSENGER SIDE REAR TIRE AND OCCURRED THE SAME WAY AS BEFORE. THE FIRST BLOWOUT WAS REPAIRED. THE CURRENT MILEAGE IS 30,000 AND FAILURE MILEAGE WAS 20,000.*AK
My Teenage daughter was in a car accident and the airbags did not deploy and her seatbelt did not lock , and smashed into her steering wheel.
The contact owns a 2004 Ford F-150. The contact stated while starting his parked vehicle, he found that the vehicle would not start. The contact self-diagnosed and bypassed the starter solenoid using a screwdriver and started the vehicle. The vehicle slipped out of the park and rolled backward. The contact was backed over by the vehicle and had received injuries to his ribs and stomach. He had lacerations and abrasions to his back and chest. The contact was able to get out from under the vehicle and attempted to stop the vehicle which was rolling. The contact jumped into the passenger side and attempted to brake but could not. The contact attempted to push the shift lever into "park" but the gear would not engage. The vehicle rolled and crashed into the rear end of a parked vehicle. The contact's vehicle had damage to the rear bumper and the rear end of the other vehicle had the bumper and trunk crumpled. The police were not called and the contact did not seek medical attention. The contact was able to start the vehicle and drove it to his residence. The contact then examined the undercarriage and determined that the transmission linkage cable had been resting on the doughnut of the exhaust system. The contact stated that the outer cable shell had melted and the metal cable was exposed. The contact diagnosed that the exposed cable had contributed to the vehicle slipping out of gear. The contact had not taken the vehicle to a dealer or an independent mechanic. The vehicle had not been repaired. The manufacturer had been informed of the failure. The failure mileage was approximately 260,000.

THERE WERE NO WARNING INDICATORS ILLUMINATED. THE DRIVER SUSTAINED A BROKEN LEFT WRIST AND STRUCK HIS HEAD ON THE WINDSHIELD. THE DRIVER RECEIVED MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO AN IMPOUND LOT AND DEEMED DESTROYED. THE CONTACT CALLED CONWAY FORD (LOCATED AT 2393 CHURCH ST, CONWAY, SC 29526, (843) 365-5596) AND INFORMED THEM OF THE FAILURE. ON SEPTEMBER 16, 20018 I WAS INVOLVED IN A TRAFFIC COLLUSION. MY VEHICLE WAS IMPACTED ENOUGH FOR A TOTAL LOSS. MY AIRBAGS NEVER DEPLOYED.IN THE ATTACHED POLICE REPORT I AM VEHICLE 2, I WAS HEADING FROM NORTH TO SOUTH ON OWENSMOUTH STREET, VEHICLE 1 WAS TRAVELING FROM WEST TO EAST ON VICTORY. I WAS WAITING AT A RED LIGHT. ANOTHER VEHICLE (VEHICLE 3)WAS WAITING AT THE RED LIGHT NEXT TO ME. WHEN THE LIGHT TURNED GREEN I BEGAN TO DRIVE AND THE VEHICLE 1 TRAVELING EAST ON VICTORY RAN A RED LIGHT AND SLAMMED INTO THE FRONT OF MY VEHICLE. MY AIRBAG NEVER WENT OFF AND I NOW SUFFER FROM EXTREME PAIN IN MY RIGHT RIB, LEFT HIP, MAJOR HEADACHES AND VERY SEVERE NECK PAIN.
TRUCK DIED, LOST ALL POWER WHILE DRIVING DOWN THE ROAD. THIS HAS HAPPENED 5 - 6 TIMES THROUGH THE YEARS STARTING AROUND 2010. SOMETIMES DRIVING DOWN FREEWAY SOMETIMES DRIVING SLOWLY INTO DRIVEWAY. BATTERY WAS CHECKED, IT WAS OKAY AND CONNECTED FINE. VARIOUS OTHER ENGINE PROBLEMS LATER INCLUDE A RATTLE NOISE AND LOW RPM. RUNNING HOT, CHUGGING ALONG LIKE RUNNING OUT OF GAS, ETC. HAVING TO PULL OVER TILL IT COULD GO AGAIN. CURRENTLY JUST A LITTLE OVER 100,000 MILES.
TAKATA RECALL I WAS IN A ACCIDENT LAST WEEK. NONE OF MY AIRBAGS DEPLOYED IT WAS A FRONT END ACCIDENT WHICH RESULTED IN THE WINDSHIELD AND NEARLY ALL GALSS BEING BROKEN AS WELL AS THE OTHER VEHICLES AIRBAGS DEPLOYING. NOT A SINGLE AIRBAG IN MY VEHICLE DEPLOYED. I WAS DOING APPROXIAMATELY 43 MPH AND THE OTHER VEHICLE PULLED OUT IN FRONT OF ME CAUSING ME TO T-BONE THE OTHER VEHICLE. NONE OF MY AIRBAGS DEPLOYED.

THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS 100,000.

THE VEHICLE WAS TOWED TO THE CONTACT'S RESIDENCE. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 170,000.

IMPACT WAS IN THE FRONT CENTER OF THE PICKUP WITH THE DAMAGE 24 INCHES INTO THE ENGINE AREA, THE DRIVER DID HAVE HIS SEAT BELT ON BUT HE STILL HIT THE REAR VIEW MIRROR WITH HIS HEAD AND BROKE HIS LEFT WRIST AND INCURRED MANY OTHER INJURIES AS A RESULT. HIS INJURIES WOULD HAVE BEEN LESSENED IF THE AIRBAG AND THE SEATBELT HAD WORKED THE WAY THERE SUPPOSED TO. *TR

THE MANUFACTURER WAS NOT MADE AWARE OF THE DEFECT. THE FAILURE MILEAGE WAS UNKNOWN.

THE CONTACT'S INSURANCE COMPANY DID NOT INVESTIGATE THE AIR BAG FAILURE. A POLICE REPORT WAS FILED AND THE VEHICLE WAS DESTROYED. THE FAILURE AND THE CURRENT MILEAGES WERE 70,000. THE VIN WAS UNAVAILABLE.
