The contact owned a 2013 Ford F-150. The contact stated that the son was driving approximately 93 MPH, and while approaching a stop light, the vehicle continued to drive into an intersection and crashed into a commercial Semi truck. During the crash, the three front-seat occupants sustained fatal injuries. The rear driver-side passenger sustained head trauma and was rushed to the hospital. A police report was filed and indicated a possible brake malfunction had occurred before the crash. During the incident, the vehicle was destroyed and later towed away. The exact cause of the failure was not determined. The manufacturer and local dealer were notified of the failure. The contact was later informed that the VIN was not included in the NHTSA Campaign Number: 16V345000 (SERVICE BRAKES, HYDRAULIC). The contact indicated that the vehicle had experienced the same failure listed in the recall. Also, the vehicle was inspected and determined that the brake booster was corroded. Vehicle accelerated without driver pressing gas pedal and RPM gauge jumped up to 5 (which is red zone). Brake pedal went to the floor when brakes were applied instead of causing the vehicle to stop. Emergency brake was used, and driver lost control of the vehicle resulting in a terrible accident. The crash caused injuries to driver and the death of a bicyclist.

The contact stated that while driving at approximately 50 MPH, the vehicle unexpectedly downshifted from 6th gear into a lower gear, and the check engine and transmission warning lights illuminated. As a result of the failure, the vehicle crashed into bushes. The contact also stated that the air bags did not deploy during the incident. The contact sustained a left foot injury; however, no medical attention was sought. A police report was not filed. The vehicle was not towed, and the contact was able to restart the vehicle. The contact further stated that while driving at an undisclosed speed on another occasion, the vehicle unexpectedly entered limp mode and remained in 3rd gear. The contact owns a 2013 Ford F-150. The contact stated while driving at various speeds, the vehicle was running rough and there was a strong burning smell. The vehicle was taken to an independent mechanic, who diagnosed that the ABS fuse was overheating, was damaging the brake pads and rotors, and was causing the vehicle to overheat. The mechanic advised the contact to remove the ABS fuse, and the contact removed the fuse. The vehicle was later taken to another independent mechanic, where it was determined that the failure was related to the ABS module. The vehicle was taken to a transmission specialist, where the same assessment was made. The vehicle was taken to an unknown dealer, where it was diagnosed that the ABS module had failed and needed to be replaced. The vehicle was not repaired due to the cost. The contact stated while driving approximately 50 MPH and approaching the stop light, the vehicle in front of her vehicle stopped prematurely. The contact stated that upon depressing the brake pedal to stop the vehicle, the vehicle decelerated but failed to stop as intended and crashed into the rear of the other vehicle. The contact stated that the ABS and the traction control warning lights were illuminated. The air bags did not deploy. The contact hit the right leg shin area against the dashboard and sustained a punctured wound on the leg. The contact also sustained a swollen right leg and back pain. The emergency units physically examined the contact and recommended medical assistance for the right leg injury. The contact later went to urgent care but could not afford the cost of the medical care. The contact then treated the injuries at home. It was unknown whether the other driver sustained injuries. There was no property damage. A police report was filed but the contact did not have the information available. The police determined that it was a no-fault accident. I have had numerous brake issues on my 2013 Ford F-150 SuperCrew. One repair was done in the warranty period and 2-3 were not. This vehicle had less than 81,000 miles on it when all of this work was done. It was recently repaired and still isn’t solved. I’ve had two major accidents where my vehicle rear ended other vehicles because of its failure to immediately stop on braking. The dealership can’t solve the problem or don’t know how and I’m afraid to continue to drive this unsafe vehicle.

9/16/22 My wife was driving about 50 mph when the transmission suddenly shifted into 1st gear causing the rear end of the truck to slide sideways and hit a curb. The force of the collision with the curb caused the right rear tire to blow out and my wife was left with bruises from the seatbelt. The was no warning prior to the incident. The truck was at the dealership on 7/13/22 for a recall for this same problem, they flashed the PCM and said that would fix the problem. I have since brought it back to the dealership they said the lead frame speed sensor in the transmission needs to be replaced and that is not covered by the recall and would cost over $2000. I also called Ford to report the incident Case # [XXX]. Ford stated this was normal wear and tear and they would not cover the repair. The contact stated that while his daughter was driving at highway speed,her USB port had initially failed to operate as needed and that soon after, the display screen had drawn completely blank. The contact's daughter had pulled over and restarted the vehicle multiple times but the failure persisted. As she proceeded to drive the vehicle at 40 MPH, the vehicle independently downshifted as the vehicle stalled without warning. Due to the failure, the contact hit another vehicle from behind. The air bags did not deploy upon impact. No medical attention was given at the scene; however, his daughter did suffer minor bruising. The whereabouts of the other driver was unknown. A police report was filed. Due to the failure, the vehicle was towed to a dealer where they have yet to inspect or diagnose the failure. TWO PROBLEMS: I) FROM A STOP, SUCH AS FOLLOWING TRAFFIC CONTROL DEVICE INSTRUCTION LIKE AT A TRAFFIC LIGHT, YIELD (BEFORE MERGING ONTO A HIGHWAY), STOP SIGN, PARKING SPOT, ETC, THE TRUCK ATTEMPTS TO START MOVING FORWARD IN 5TH GEAR. THE TRUCK DOES NOT MOVE FORWARD UNLESS SHIFTED TO NEUTRAL OR PARK AND THEN BACK INTO DRIVE. TRUCK WILL NOT ACCELERATE FROM A STOPPED POSITION IN 5TH GEAR. II) MORE CONCERNING IS THAT THE TRUCK DECIDES TO DOWNSHIFT INAPPROPRIATELY (E.G. IT SHOULD UPSHIFT OR STAY IN GEAR, BUT DOWNSHIFTS ERRATICALLY CALLING). THIS LEADS TO STOPPING SHORT AND CAUSES THE TRUCK TO BUCK. IT RESULTS IN ALMOST TOTAL LOSS OF CONTROL OF THE VEHICLE AND PRESENTS A HORRIBLE RISK TO DRIVER, PASSENGERS, AND OTHERS ON THE ROAD. HAD TO PULL OFF SIDE OF ROAD OVER A CURB CAUSING PROPERTY DAMAGE AND ALMOST A HEART ATTACK! I HAVE 68,000 MILES ON A TRUCK THAT SHOULD LAST FOR AT LEAST DOUBLE THAT. THE TRUCK HAS BEEN BABIED AND WELL-MAINTAINED SINCE IT WAS PURCHASED. THIS IS SAD THAT FORD WON'T ACCEPT RESPONSIBILITY FOR USING A FAULTY TRANSMISSION WITH KNOWN ISSUES.
