TL* THE CONTACT STATED HER SON WAS DRIVING A 2018 FORD F-150 THAT HE BORROWED FROM A FRIEND. THE CONTACT STATED WHILE HER SON WAS DRIVING AT AN UNKNOWN AND REAR ENDED ANOTHER VEHICLE WHILE IT WAS MAKING A LEFT TURN. THE CONTACT STATED AS A RESULT BOTH VEHICLE HIT COLLIDED WITH A TREE. THE CONTACT STATED THAT THE VEHICLE HER SON WAS DRIVING CAUGHT FIRE AS THE FUEL TANK COLLIDED WITH THE TREE. AS A RESULT, THE CONTACT STATED HER SON LOST HIS LIFE. THE CONTACT WAS PROVIDING ALL INFORMATION FROM THE POLICE REPORT FILED. THE VEHICLE WAS A TOTAL LOSS. THE VEHICLE WAS TOWED TO TOW LOT. THE MANUFACTURER HAD NOT BEEN INFORMED OF THE FAILURE.THE FAILURE MILEAGE WAS UNKNOWN.

The contact's husband owned a 2018 Ford F-150. The contact stated that while her husband was driving at 62 MPH, he hit the back of the semi-truck on the front driver which sent the vehicle to tailspin and caused her husband to hit the semi-truck for a second time. The second collision sent the vehicle into another tailspin before coming to a complete halt. A police report was filed. According to the police department investigation, the airbag inflator had deployed which struck her husband's forehead. The investigation also determined that her husband's injuries were not consistent with proper airbag deployment. The report also noticed that both the air bag and air bag inflator were found on the floor near the rear passenger side seat. In addition, the report showed bruising constant with the lower lap belt locking but the driver did not have any bruising/burns from the upper seatbelt. The contact was pronounced dead on the scene and was taken to a moratorium. The vehicle was taken to an independent tow yard and deemed a total loss by insurance. The dealer nor the manufacturer had yet to be notified of the failure. The failure mileage was 36,667.

This vehicle was involved in an offset rear-end collision with a semi-trailer, with the driver's side making the primary impact. The crash resulted in the death of the driver and only occupant. Both the driver's airbag and left passenger curtain airbags deployed. During the police investigation of the crash, it was determined that the steering wheel-mounted airbag assembly completely dislodged from the steering wheel, with the airbag and its assembly being located in the rear floorboard post-impact. The driver sustained a head injury possibly consistent with the airbag assembly striking their forehead area. Two out of three of the plastic retaining tabs were found to have sheered off, likely leading to the airbag assembly being ejected from the steering wheel. Police investigators documented this incident with photographs and have taken the airbag assembly as evidence.

WE GOT INTO AN ACCIDENT OVER THE WEEKEND. WE WERE T-BONES BY A BUS. IT WRECKED THE PASSENGER SIDE OF THE CAR. HOWEVER, WHEN I WENT INTO THE PASSENGER SIDE TO GET MY PURSE OUT I NOTICED THAT THERE WERE FLAMES COMING FROM THE SIDE OF THE DRIVERS SEAT. OUR F150 IS NOW BURNT TO A CRISP. THERE'S NO REASON FOR THAT TRUCK TO HAVE CAUGHT FIRE LIKE THAT.

AFTER I WAS REAR ENDED MY 2018 F150 STARTED SMOKING AND CAUGHT FIRE AT THE BASE OF THE POST BESIDE THE DRIVER SEAT WHERE THE CARPET AND PLASTIC SIDE COVER COME TOGETHER. THIS HAPPENED WITHIN 10 MINUTES OF THE CRASH. I WAS TRAVELING APPROXIMATELY 10 MPH ON A 65 MPH 4 LANE, SLOWING DUE TO TRAFFIC CONGESTION WHEN I WAS REAR ENDED BY A 2007 CHEVY PICKUP TRAVELLING AT ABOUT 60 MPH. HITTING MY TRUCK HARD ENOUGH TO LIFT THE FRONT END AND LAND ON THE 4 FOOT HIGHWAY BARRIER IN THE FRONT, MY TRUCK CONTINUED AND THEN STUCK THE TRUCK IN FRONT OF ME WITH MY PASSENGER DOOR CAUSING THE REAR OF THE TRUCK TO ALSO LAND ON THE CONCRETE BARRIER. I WAS HELPED FROM THE TRUCK THRU THE RIGHT SIDE BACK DOOR BY THE DRIVER OF THE TRUCK MY VEHICLE IMPACTED. OFFICER WAS ON SITE IN ABOUT 3 MINUTES AND WANTED TO MOVE MY TRUCK FIRST, HE ASKED IF I HAD ANYTHING I NEEDED OUT OF THE VEHICLE. TROOPER AND I WENT TO GET MY BELONGINGS WHEN WE NOTICED THE SMOKE IN THE CAB. I WAS NOT ABLE TO GET ANYTHING FROM THE TRUCK BECAUSE THE FIRE STARTED AND SPREAD QUICKLY. FIRE WAS ONLY ON THE INSIDE OF THE VEHICLE.

JUNE 21, 2018 WE WERE DRIVING DOWN HWY 1 BYPASS IN FORREST CITY, AR AND WERE HIT ON THE DRIVER'S SIDE, PASSENGER DOOR AND GAS TANK AREA. THE TRUCK TOOK THE IMPACT WELL AND WE SPUN 180 DEGREES TO THE OPPOSITE SIDE OF THE ROAD. THE SIDE AND CURTAIN AIRBAGS DEPLOYED AND MY TWO SMALL CHILDREN IN THE BACK SEAT WERE UNHARMED, INCLUDING THE ONE WHOSE DOOR TOOK MOST OF THE IMPACT. MY HIP WAS INJURED FROM THE AIR BAG THAT DEPLOYED FROM THE BACK OF MY SEAT. AS I WAS SITTING IN THE SEAT IMMEDIATELY FOLLOWING THE ACCIDENT CALLING 911, THE SEAT BEGAN SMOKING. I TRIED TO EXIT OUT OF THE DRIVER'S SIDE DOOR AND IT WAS JAMMED. SMOKE CONTINUED TO GET THICKER AND SMELLED LIKE AN ELECTRICAL/PLASTIC BURNING SMELL. I THEN HAD TO CRAWL OVER THE CENTER CONSOLE AND EXIT THROUGH THE FRONT PASSENGER DOOR. AT THAT POINT, THICK BLACK SMOKE WAS COMING FROM MY SEAT AND THE LEFT SIDE OF THE CAB BEGAN TO BURN. THE ACTUAL IMPACT DAMAGE TO MY VEHICLE WOULD NOT HAVE TOTALED IT OUT, BUT THE FIRE, SMOKE, AND WATER DAMAGE FROM THE FIRE BEING PUT OUT, DID. I HAVE ALSO FILED A CASE WITH FORD MOTOR COMPANY, AS I BELIEVE THIS WAS A MECHANICAL/ELECTRICAL DEFECT IN THE VEHICLE. HAD WE NOT BEEN ABLE TO EXIT THE VEHICLE, MY ENTIRE FAMILY WOULD HAVE DIED FROM THIS FIRE, NOT THE IMPACT OF THE ACCIDENT. FROM OBSERVATIONS FROM THE POLICE, FIRE DEPARTMENT, WRECKER SERVICE, AND MY HUSBAND WHO IS A MECHANIC, THEY BELIEVE THAT THE FIRE WAS IGNITED WHEN THE AIR BAGS DEPLOYED. I DID HAVE HEATED SEATS, AND THE VEHICLE WAS CLEARLY ON WHEN I WAS HIT AND THE AIR BAGS DEPLOYED.

ABOUT 5-10 MINUTES AFTER THE WRECK THE INSIDE OF THE TRUCK WAS ON FIRE. THE FIRE DEPARTMENT WAS CALLED AND PUT THE FIRE OUT. AT THAT TIME THE FIREMAN STATED THEY FELT THE AIRBAG ON THE DRIVERS SIDE HAD STARTED THE FIRE. UPON LOOKING AT THE TRUCK YESTERDAY, I COULD SEE THAT IT WAS REALLY THE SEAT BACK OF THE DRIVERS SEAT THAT BURNED THE MOST. AT THE TIME OF THE FIRE, THE TRUCK WAS NOT RUNNING AND WAS STILL SITTING IN THE SPOT IT CAME TO REST IN AFTER THE ACCIDENT.

THE VERMONT STATE POLICE TOWED THE VEHICLE TO THEIR YARD FOR INVESTIGATION. THE VEHICLE WAS LATER INCLUDED IN NHTSA CAMPAIGN NUMBER: 18V568000 (STRUCTURE, SEAT BELTS), ALTHOUGH THE CONTACT WAS NOT NOTIFIED. IMMEDIATE EVACUATION WAS NEEDED AND A SECOND 911 CALL WAS PLACED. THE TRUCK WENT UP IN COMPLETE FLAMES IN A MATTER OF MINUTES AND IS A COMPLETE LOSS.

The contact stated that not a single air bag deployed during the impact, and no brake collision warning was activated. No warning lights illuminated on the instrument panel prior to the crash. The contact stated he received a pre-collision notification on his phone approximately 30 seconds after the accident occurred, but he was unable to avoid the collision. I was rear ended by a driver going approximately 35-45mph while I was coming to a near stop at a stop light, driver struck me totalling my vehicle and gaming me up to the right and down into the ditch alongside the road, my airbags never deployed but at fault party aurbags did. At some point during the accident most likely during the initial impact the driver side seat of my f150 broke resulting in my seat laying flat and me laying on top of it. The seat was a powered movement seat, the seat appears to have broke the back rest movement or retaining device. The seat failing to keep me in the correct upright position may have attributed to a severe back injury. Vehicle was taken to insurance salvage yard by state farm, was also investigated by state farm insurance adjuster but any additional info was not provided to me beyond the vehicle is a total loss At the time of the accident no warning lights were present however a emissions light was intermittent for 02 sensor, after the accident several lights were present for airbag/accident and rear parking aid sensors
